    Quick-Mixing Studies Under Reacting Conditions

    The low-NO(x) emitting potential of rich-burn/quick-mix/lean-burn )RQL) combustion makes it an attractive option for engines of future stratospheric aircraft. Because NO(x) formation is exponentially dependent on temperature, the success of the RQL combustor depends on minimizing high temperature stoichiometric pocket formation in the quick-mixing section. An experiment was designed and built, and tests were performed to characterize reaction and mixing properties of jets issuing from round orifices into a hot, fuel-rich crossflow confined in a cylindrical duct. The reactor operates on propane and presents a uniform, non-swirling mixture to the mixing modules. Modules consisting of round orifice configurations of 8, 9, 10, 12, 14, and 18 holes were evaluated at a momentum-flux ratio of 57 and jet-to-mainstream mass-flaw ratio of 2.5. Temperatures and concentrations of O2, CO2, CO, HC, and NO(x) were obtained upstream, down-stream, and within the orifice plane to determine jet penetration as well as reaction processes. Jet penetration was a function of the number of orifices and affected the mixing in the reacting system. Of the six configurations tested, the 14-hole module produced jet penetration close to the module half-radius and yielded the best mixing and most complete combustion at a plane one duct diameter from the orifice leading edge. The results reveal that substantial reaction and heat release occur in the jet mixing zone when the entering effluent is hot and rich, and that the experiment as designed will serve to explore satisfactorily jet mixing behavior under realistic reacting conditions in future studies

    Aloimunisasi sel darah merah ditakrifkan sebagai pembentukan antibodi di dalam respons kepada antigen asing sel merah melalui proses transfusi atau mengandung. Di kalangan wanita hamil yang tidak mempunyai sejarah transfusi darah, mereka boleh mengalami aloimunisasi pada kehamilan yang lalu atau semasa disebabkan oleh kehadiran antigen sel merah bapa yang diwarisi kepada janin. Kajian ini bertujuan untuk menentukan prevalens aloimunisasi sel darah merah di kalangan wanita hamil tanpa sejarah transfusi darah di PPUKM dan juga untuk mengaitkan kelazimannya dengan nombor kehamilan dan sejarah komplikasi obstetrik. Ini adalah satu kajian keratan rentas di mana 150 wanita hamil telah dipilih secara rawak di klinik antenatal PPUKM. Sepuluh ml darah periferal telah diperolehi selepas persetujuan diambil dari setiap pesakit. Ujian yang dijalankan ke atas sampel-sampel darah tersebut termasuk ujian jenis kumpulan darah ABO dan Rh D dan saringan antibodi dengan menggunakan teknik ujian antiglobulin tidak langsung. Sampel positif terus dikenalpasti untuk pengkhususan antibodi. Dalam kajian kami, majoriti (37.3%) daripada wanita adalah primigravida. Aloantibodi sel darah merah telah dikesan pada dua daripada 150 (1.3%) pesakit yang kemudiannya dikenalpasti sebagai anti-C dan anti-D. Namun tiada seorang pun daripada primigravida yang mengalami aloimunisasi. Seorang wanita dari gravida 2 (2.9%) dan gravida 3 (3.6%) didapati positif bagi aloimunisasi. Salah seorang daripada mereka juga mempunyai sejarah obstetrik buruk. Kajian ini menunjukkan bahawa kelaziman aloimunisasi sel darah merah di kalangan wanita hamil adalah rendah di pusat ini.     A liver fibrosis cocktail? Psoriasis, methotrexate and genetic hemochromatosis

    BACKGROUND: Pathologists are often faced with the dilemma of whether to recommend continuation of methotrexate therapy for psoriasis within the context of an existing pro-fibrogenic risk factor, in this instance, patients with genetic hemochromatosis. CASE PRESENTATIONS: We describe our experience with two male psoriatic patients (A and B) on long term methotrexate therapy (cumulative dose A = 1.56 gms and B = 7.88 gms) with hetero- (A) and homozygous (B) genetic hemochromatosis. These patients liver function were monitored with routine biochemical profiling; apart from mild perivenular fibrosis in one patient (B), significant liver fibrosis was not identified in either patient with multiple interval percutaneous liver biopsies; in the latter instance this patient (B) had an additional risk factor of partiality to alcohol. CONCLUSION: We conclude that methotrexate therapy is relatively safe in patients with genetic hemochromatosis, with no other risk factor, but caution that the risk of fibrosis be monitored, preferably by non-invasive techniques, or by liver biopsy

    Gravitational Waves From Known Pulsars: Results From The Initial Detector Era

    We present the results of searches for gravitational waves from a large selection of pulsars using data from the most recent science runs (S6, VSR2 and VSR4) of the initial generation of interferometric gravitational wave detectors LIGO (Laser Interferometric Gravitational-wave Observatory) and Virgo. We do not see evidence for gravitational wave emission from any of the targeted sources but produce upper limits on the emission amplitude. We highlight the results from seven young pulsars with large spin-down luminosities. We reach within a factor of five of the canonical spin-down limit for all seven of these, whilst for the Crab and Vela pulsars we further surpass their spin-down limits. We present new or updated limits for 172 other pulsars (including both young and millisecond pulsars). Now that the detectors are undergoing major upgrades, and, for completeness, we bring together all of the most up-to-date results from all pulsars searched for during the operations of the first-generation LIGO, Virgo and GEO600 detectors.     The 100 most cited articles investigating the radiological staging of oesophageal and junctional cancer: a bibliometric analysis

    Objectives Accurate staging of oesophageal cancer (OC) is vital. Bibliometric analysis highlights key topics and publications that have shaped understanding of a subject. The 100 most cited articles investigating radiological staging of OC are identified. Methods The Thomas Reuters Web of Science database with search terms including “CT, PET, EUS, oesophageal and gastro-oesophageal junction cancer” was used to identify all English language, full-script articles. The 100 most cited articles were further analysed by topic, journal, author, year and institution. Results A total of 5,500 eligible papers were returned. The most cited paper was Flamen et al. (n = 306), investigating the utility of positron emission tomography (PET) for the staging of patients with potentially operable OC. The most common research topic was accuracy of staging investigations (n = 63). The article with the highest citation rate (38.00), defined as the number of citations divided by the number of complete years published, was Tixier et al. investigating PET texture analysis to predict treatment response to neo-adjuvant chemo-radiotherapy, cited 114 times since publication in 2011. Conclusion This bibliometric analysis has identified key publications regarded as important in radiological OC staging. Articles with the highest citation rates all investigated PET imaging, suggesting this modality could be the focus of future research

    Breast cancer epithelial-to-mesenchymal transition: examining the functional consequences of plasticity

    The epithelial-to-mesenchymal transition (EMT) is a critical developmental process that has recently come to the forefront of cancer biology. In breast carcinomas, acquisition of a mesenchymal-like phenotype that is reminiscent of an EMT, termed oncogenic EMT, is associated with pro-metastatic properties, including increased motility, invasion, anoikis resistance, immunosuppression and cancer stem cell characteristics. This oncogenic EMT is a consequence of cellular plasticity, which allows for interconversion between epithelial and mesenchymal-like states, and is thought to enable tumor cells not only to escape from the primary tumor, but also to colonize a secondary site. Indeed, the plasticity of cancer cells may explain the range of pro-metastatic traits conferred by oncogenic EMT, such as the recently described link between EMT and cancer stem cells and/or therapeutic resistance. Continued research into this relationship will be critical in developing drugs that block mechanisms of breast cancer progression, ultimately improving patient outcomes

    Seven classes of mitogen-activated protein kinase (MAPK) intracellular signalling cascades exist, four of which are implicated in breast disease and function in mammary epithelial cells. These are the extracellular regulated kinase (ERK)1/2 pathway, the ERK5 pathway, the p38 pathway and the c-Jun N-terminal kinase (JNK) pathway. In some forms of human breast cancer and in many experimental models of breast cancer progression, signalling through the ERK1/2 pathway, in particular, has been implicated as being important. We review the influence of ERK1/2 activity on the organised three-dimensional association of mammary epithelial cells, and in models of breast cancer cell invasion. We assess the importance of epidermal growth factor receptor family signalling through ERK1/2 in models of breast cancer progression and the influence of ERK1/2 on its substrate, the oestrogen receptor, in this context. In parallel, we consider the importance of these MAPK-centred signalling cascades during the cycle of mammary gland development. Although less extensively studied, we highlight the instances of signalling through the p38, JNK and ERK5 pathways involved in breast cancer progression and mammary gland development

    Comparative effectiveness and safety of non-vitamin K antagonists for atrial fibrillation in clinical practice: GLORIA-AF Registry

    Background and purpose: Prospectively collected data comparing the safety and effectiveness of individual non-vitamin K antagonists (NOACs) are lacking. Our objective was to directly compare the effectiveness and safety of NOACs in patients with newly diagnosed atrial fibrillation (AF). Methods: In GLORIA-AF, a large, prospective, global registry program, consecutive patients with newly diagnosed AF were followed for 3 years. The comparative analyses for (1) dabigatran vs rivaroxaban or apixaban and (2) rivaroxaban vs apixaban were performed on propensity score (PS)-matched patient sets. Proportional hazards regression was used to estimate hazard ratios (HRs) for outcomes of interest. Results: The GLORIA-AF Phase III registry enrolled 21,300 patients between January 2014 and December 2016. Of these, 3839 were prescribed dabigatran, 4015 rivaroxaban and 4505 apixaban, with median ages of 71.0, 71.0, and 73.0 years, respectively. In the PS-matched set, the adjusted HRs and 95% confidence intervals (CIs) for dabigatran vs rivaroxaban were, for stroke: 1.27 (0.79–2.03), major bleeding 0.59 (0.40–0.88), myocardial infarction 0.68 (0.40–1.16), and all-cause death 0.86 (0.67–1.10). For the comparison of dabigatran vs apixaban, in the PS-matched set, the adjusted HRs were, for stroke 1.16 (0.76–1.78), myocardial infarction 0.84 (0.48–1.46), major bleeding 0.98 (0.63–1.52) and all-cause death 1.01 (0.79–1.29). For the comparison of rivaroxaban vs apixaban, in the PS-matched set, the adjusted HRs were, for stroke 0.78 (0.52–1.19), myocardial infarction 0.96 (0.63–1.45), major bleeding 1.54 (1.14–2.08), and all-cause death 0.97 (0.80–1.19). Conclusions: Patients treated with dabigatran had a 41% lower risk of major bleeding compared with rivaroxaban, but similar risks of stroke, MI, and death. Relative to apixaban, patients treated with dabigatran had similar risks of stroke, major bleeding, MI, and death. Rivaroxaban relative to apixaban had increased risk for major bleeding, but similar risks for stroke, MI, and death. Registration: URL: https://www.clinicaltrials.gov.
