    Multicultural Competence: A Literature Review Supporting Focused Training for Preservice Teachers Teaching Diverse Students

    This paper focuses on an in depth literature review based on preservice teachers perceptions of their multicultural competence in teaching diverse students. More specifically, the literature review was framed around findings from a study looking at the gap between increased diversity of students and the level of multicultural competence of preservice teachers at a local university was investigated in this qualitative case study. Inductive data analysis results indicated that preservice teachers perceived a need for additional multicultural competence including increased awareness, knowledge, and skills in working with diverse students. A professional development training workshop design was created to address four dominant themes from the analysis.  The literature review supports the prominent components addressed by the professional development training workshop. Adequately preparing preservice teachers for teaching  diverse students continues as a focus in teacher preparation programs. As a faculty member at the local university, school of education, ongoing research focused on building preservice teachers multicultural competence remains essential.     Multicultural Competence for Teaching Diverse Students as Experienced by Preservice Teachers

    The gap between increased diversity of students and the level of multicultural competence of preservice teachers at a local university was investigated in this qualitative case study. The purpose of this study was to describe preservice teachers\u27 experiences with multicultural competence in teaching diverse students. The study\u27s conceptual framework was based on Dewey\u27s theory of experience, Knowles\u27s theory of adult learning, Kolb\u27s theory of experiential learning, and Gay\u27s culturally responsive teaching. Two models incorporating cultural competence by Mason, Benjamin, and Lewis and Pedersen were used to frame professional practice and develop understanding, acceptance, and skills in working with diverse students. Inquiry into how preservice teachers characterized their multicultural competence in relation to their experiences teaching diverse students formed the guiding research question. Data collection included semistructured, individual interviews with 10 preservice teachers selected by purposeful sampling. Lesson plans, class profiles from participants, and handwritten notes of participants\u27 nonverbal expressions during interviews were also analyzed. Inductive data analysis results indicated that preservice teachers perceived a need for additional multicultural competence including increased awareness, knowledge, and skills in working with diverse students. A 3-day professional development workshop training project was developed to address cultural awareness, knowledge, and skills among preservice teachers in grades K-12. Preservice teachers\u27 experiences revealed the need to build and strengthen multicultural competence in order to bring about social change by improving educational outcomes for minority culture students

    Accounting’s role in providing data suggests its impact in making things governable and knowable and this paper reflects on its role in making violence invisible. As critical researchers the consequences of accounting practices on vulnerable populations is of significant import and reporting on women and violence is assessed in this research using global reports, testimonies and data. We find a discourse in global reports minimizing and silencing violence against women while counter accounts reveal a contrasting reality. As such, the dynamics of knowledge creation challenges us to explore the neoliberal construction of data. We suggest alternative formulations adding to an emerging literature of counter-accounts and we reject an inevitability of silence. Rather critical accounts provide pathways for thinking differently and aspiring for change and social justice. O papel da contabilidade no fornecimento de dados sugere seu impacto em tornar as coisas governáveis e conhecidas e esse artigo reflete sobre o seu papel em tornar a violência invisível. Como pesquisadoras e pesquisadores críticas, as consequências das práticas contábeis em populações vulneráveis são de grande importância e os reportes sobre mulheres e violência são avaliados nessa pesquisa utilizando os relatórios globais, testemunhos e dados. Nós encontramos um discurso nos relatórios globais que minimiza e silencia a violência contra as mulheres, enquanto a counter accounting revela uma realidade contrastante. Como tal, a dinâmica da construção do conhecimento nos desafia a explorar a construção neoliberal dos dados. Nós sugerimos formulações alternativas adicionando à uma literatura emergente de counter accounting e nós rejeitamos a inevitabilidade do silêncio. Os relatos críticos provêem caminhos para pensar diferentemente e aspirar pela mudança e pela justiça social

    Discharge destination from an acute care for the elderly (ACE) unit

    Older adults age 65 and over account for a disproportional number of hospital stays and discharges compared to other age groups. The objective of this paper is to describe placement and characteristics of older patients discharged from an acute care for the elderly (ACE) unit. The study sample consists of 1,351 men and women aged 65 years or older that were discharged from the ACE Unit during a 12-month period. The mean number of discharges per month was 109.2 ± 28.4. Most of the subjects were discharged home or home with home health 841, 62.3%. The oldest elderly and patients who had been admitted from long term care institutions or from skilled nursing facilities to the ACE unit were less likely to return to home

    Implementing Electronic Tablet-Based Education of Acute Care Patients

    Poor education-related discharge preparedness for patients with heart failure is believed to be a major cause of avoidable rehospitalizations. Technology-based applications offer innovative educational approaches that may improve educational readiness for patients in both inpatient and outpatient settings; however, a number of challenges exist when implementing electronic devices in the clinical setting. Implementation challenges include processes for "on-boarding" staff, mediating risks of cross-contamination with patients' device use, and selling the value to staff and health system leaders to secure the investment in software, hardware, and system support infrastructure. Strategies to address these challenges are poorly described in the literature. The purpose of this article is to present a staff development program designed to overcome challenges in implementing an electronic, tablet-based education program for patients with heart failure

    Effect of Covered Metallic Stents Compared With Plastic Stents on Benign Biliary Stricture Resolution A Randomized Clinical Trial

    Importance Endoscopic placement of multiple plastic stents in parallel is the first-line treatment for most benign biliary strictures; it is possible that fully covered, self-expandable metallic stents (cSEMS) may require fewer endoscopic retrograde cholangiopancreatography procedures (ERCPs) to achieve resolution. Objective To assess whether use of cSEMS is noninferior to plastic stents with respect to stricture resolution. Design, Setting, and Participants Multicenter (8 endoscopic referral centers), open-label, parallel, randomized clinical trial involving patients with treatment-naive, benign biliary strictures (N = 112) due to orthotopic liver transplant (n = 73), chronic pancreatitis (n = 35), or postoperative injury (n = 4), who were enrolled between April 2011 and September 2014 (with follow-up ending October 2015). Patients with a bile duct diameter less than 6 mm and those with an intact gallbladder in whom the cystic duct would be overlapped by a cSEMS were excluded. Interventions Patients (N = 112) were randomized to receive multiple plastic stents or a single cSEMS, stratified by stricture etiology and with endoscopic reassessment for resolution every 3 months (plastic stents) or every 6 months (cSEMS). Patients were followed up for 12 months after stricture resolution to assess for recurrence. Main Outcomes and Measures Primary outcome was stricture resolution after no more than 12 months of endoscopic therapy. The sample size was estimated based on the noninferiority of cSEMS to plastic stents, with a noninferiority margin of −15%. Results There were 55 patients in the plastic stent group (mean [SD] age, 57 [11] years; 17 women [31%]) and 57 patients in the cSEMS group (mean [SD] age, 55 [10] years; 19 women [33%]). Compared with plastic stents (41/48, 85.4%), the cSEMS resolution rate was 50 of 54 patients (92.6%), with a rate difference of 7.2% (1-sided 95% CI, −3.0% to ∞; P < .001). Given the prespecified noninferiority margin of −15%, the null hypothesis that cSEMS is less effective than plastic stents was rejected. The mean number of ERCPs to achieve resolution was lower for cSEMS (2.14) vs plastic (3.24; mean difference, 1.10; 95% CI, 0.74 to 1.46; P < .001). Conclusions and Relevance Among patients with benign biliary strictures and a bile duct diameter 6 mm or more in whom the covered metallic stent would not overlap the cystic duct, cSEMS were not inferior to multiple plastic stents after 12 months in achieving stricture resolution. Metallic stents should be considered an appropriate option in patients such as these

    Locally advanced breast cancers are more likely to present as Interval Cancers: results from the I-SPY 1 TRIAL (CALGB 150007/150012, ACRIN 6657, InterSPORE Trial)

    Interval cancers (ICs), defined as cancers detected between regular screening mammograms, have been shown to be of higher grade, larger size, and associated with lower survival, compared with screen-detected cancers (SDCs) and comprise 17% of cancers from population-based screening programs. We sought to determine the frequency of ICs in a study of locally advanced breast cancers, the I-SPY 1 TRIAL. Screening was defined as having a mammogram with 2 years, and the proportion of ICs at 1 and 2 years was calculated for screened patients. Differences in clinical characteristics for ICs versus SDCs and screened versus non-screened cancers were assessed. For the 219 evaluable women, mean tumor size was 6.8 cm. Overall, 80% of women were over 40 and eligible for screening; however, only 31% were getting screened. Among women screened, 85% were ICs, with 68% diagnosed within 1 year of a previously normal mammogram. ICs were of higher grade (49% vs. 10%) than SDCs. Among non-screened women, 28% (43/152) were younger than the recommended screening age of 40. Of the entire cohort, 12% of cancers were mammographically occult (MO); the frequency of MO cancers did not differ between screened (11%) and non-screened (15%). ICs were common in the I-SPY 1 TRIAL suggesting the potential need for new approaches beyond traditional screening to reduce mortality in women who present with larger palpable cancers

    Superhelical Duplex Destabilization and the Recombination Position Effect

    The susceptibility to recombination of a plasmid inserted into a chromosome varies with its genomic position. This recombination position effect is known to correlate with the average G+C content of the flanking sequences. Here we propose that this effect could be mediated by changes in the susceptibility to superhelical duplex destabilization that would occur. We use standard nonparametric statistical tests, regression analysis and principal component analysis to identify statistically significant differences in the destabilization profiles calculated for the plasmid in different contexts, and correlate the results with their measured recombination rates. We show that the flanking sequences significantly affect the free energy of denaturation at specific sites interior to the plasmid. These changes correlate well with experimentally measured variations of the recombination rates within the plasmid. This correlation of recombination rate with superhelical destabilization properties of the inserted plasmid DNA is stronger than that with average G+C content of the flanking sequences. This model suggests a possible mechanism by which flanking sequence base composition, which is not itself a context-dependent attribute, can affect recombination rates at positions within the plasmid
