    The Limitations of “Information Privacy” in the Network Environment

    The modern concept of privacy as a right to be let alone was developed in the context of print media. With the advent of digital technology, the focus of the privacy concept has changed to individuals’ right to control their information. In this essay, we explore why the individual right to control personal information is not enough to protect privacy in a meaningful way in a networked environment, given the interactive nature of the Internet and the voluntary nature of information activities of individual users. The greatest difficulty for individuals who become the object of surveillance in the current technological environment lies in the fact that as individual users' identities become more and more exposed, subjects of surveillance and their activities become less and less identifiable. Given the power disparity regarding identifiability that always has existed between individuals and institutions and among different individuals, we argue that privacy should not only be an individual's right but also a social good with concerns for the public interest

    Acrolein is known to be involved in acute lung injury and other pulmonary diseases. A number of studies have suggested that acrolein-induced toxic effects are associated with depletion of antioxidants, such as reduced glutathione and protein thiols, and production of reactive oxygen species. Mitochondrial NADP+-dependent isocitrate dehydrogenase (idh2) regulates mitochondrial redox balance and reduces oxidative stress-induced cell injury via generation of NADPH. Therefore, we evaluated the role of idh2 in acrolein-induced lung injury using idh2 short hairpin RNA- (shRNA-) transfected Lewis lung carcinoma (LLC) cells and idh2-deficient (idh2−/−) mice. Downregulation of idh2 expression increased susceptibility to acrolein via induction of apoptotic cell death due to elevated mitochondrial oxidative stress. Idh2 deficiency also promoted acrolein-induced lung injury in idh2 knockout mice through the disruption of mitochondrial redox status. In addition, acrolein-induced toxicity in idh2 shRNA-transfected LLC cells and in idh2 knockout mice was ameliorated by the antioxidant, N-acetylcysteine, through attenuation of oxidative stress resulting from idh2 deficiency. In conclusion, idh2 deficiency leads to mitochondrial redox environment deterioration, which causes acrolein-mediated apoptosis of LLC cells and acrolein-induced lung injury in idh2−/− mice.     Whats Happening in the Jury Room? : Analyzing Shadow Jury Deliberations in Korea

    This paper looked into the jury deliberation process by examining shadow jury deliberations in 18 actual cases between November 2010 and July 2011 in Korea. Based on the direct observation and the content analysis of the videotaped deliberation, we examined four key areas in jury deliberation in order to gain insights and implications for the institutional design of the jury system: (1) the binding effect of the jury verdict, (2) the number of jurors, (3) the jurors deliberations regarding both conviction and sentencing, and (4) the judges intervention in jury deliberation. The results demonstrate that the shadow jurors in general actively participated in the deliberation process by speaking in turn, and were respectful toward other jurors in debate. The jury forepersons positively played their role by giving jurors equal chance to talk and managed the discussion well. Misunderstanding of law and the intermingling of facts relevant to conviction or sentencing were not as frequent as many people expected: when such problems occurred, they were most often corrected through the intervention of other jurors or judges. Most judges were helpful in jurors reaching a verdict in the jury room. Also there was no definitive relationship between the size of the jury and the quality of deliberation. On the other hand, the shadow jurors tended to state their initial positions early in the deliberation process without fully discussing the issues first. They oftentimes made arguments not based on evidence. In addition, jurors emotions affected decision-making in some instances. Although encouraging aspects as well as areas for improvement coexist, the overall quality of jury deliberation in Korea, as evidenced by this study, is positive.     Inter- and Intra-observer Reliability of MRI for Lumbar Lateral Disc Herniation

    Background: The authors analyzed inter- and intra-observer agreement with respect to interpretation of simple magnetic resonance T1- and T2-weighted axial and sagittal images for the diagnosis of lumbar lateral disc herniation, including foraminal and extraforaminal disc herniations.Methods: Forty-two patients in whom lumbar lateral disc herniation was suspected or confirmed by simple magnetic resonance imaging at one institute between May 2003 and December 2004 were included. The magnetic resonance images consisting of T1- and T2-weighted axial and sagittal images, and these were reviewed blindly and independently by three orthopaedic spine surgeons in a random manner. The images were interpreted as positive or negative for lateral disc herniation on 2 different occasions 3 months apart. Results were analyzed using Cohens kappa statistic, and strengths of agreements were determined using the Landis and Koch criteria.Results: The kappa values for inter-observer agreement averaged 0.234 (0.282, 0.111, and 0.308 respectively) on the first occasion, and 0.166 (0.249, 0.111, and 0.137 respectively) on the second occasion, with an overall mean value of 0.200. Thus, the strength of agreement was only slight-to-fair according to the Landis and Koch criteria. Kappa values for intra-observer agreement averaged 0.479 (0.488, 0.491, and 0.459 respectively), indicating moderate agreement.Conclusions: The present study indicates that simple magnetic resonance imaging is not a reliable imaging modality for diagnosing lumbar lateral disc herniation.     Volumetric change of the latissimus dorsi muscle after immediate breast reconstruction with an extended latissimus dorsi musculocutaneous flap

    Background In immediate breast reconstruction using an extended latissimus dorsi musculocutaneous (eLDMC) flap, the volume of the flap decreases, which causes a secondary deformity of the breast shape. Since little research has investigated this decrease in muscle volume, the authors conducted an objective study to characterize the decrease in muscle volume after breast reconstruction using an eLDMC flap. Methods Research was conducted from October 2011 to November 2016. The subjects included 23 patients who underwent mastectomy due to breast cancer, received immediate reconstruction using an eLDMC flap without any adjuvant chemotherapy or radiotherapy, and received a computed tomography (CT) scan from days 7 to 10 after surgery and 6 to 8 months postoperatively. In 10 patients, an additional CT scan was conducted 18 months postoperatively. Axial CT scans were utilized to measure the volumetric change of the latissimus dorsi muscle during the follow-up period. Results In the 23 patients, an average decrease of 54.5% was observed in the latissimus dorsi muscle volume between the images obtained immediately postoperatively and the scans obtained 6 to 8 months after surgery. Ten patients showed an average additional decrease of 11.9% from 6–8 months to 18 months after surgery. Conclusions We studied changes in the volume of the latissimus dorsi muscle after surgery using an eLDMC flap performed after a mastectomy without adjuvant chemotherapy or radiotherapy. In this study, we found that immediate breast reconstruction using a latissimus dorsi muscle flap led to a decrease in muscle volume of up to 50%

    The Relationship between Subjective and Objective Parameters in CT Phantom Image Evaluation

    Objective: To evaluate whether there is a relationship between subjective parameters determined by a reviewer (spatial resolution, low contrast resolution, and artifacts) and objective parameters (the CT number of water, noise, and image uniformity) in CT phantom image evaluations. Materials and Methods: We reviewed the CT results of phantom image evaluations conducted by Korean Institute for Accreditation of Medical Image (KIAMI) from May 2007 to June 2007. We compared the objective parameters against the pass or fail groups for the subjective parameters. We also evaluated whether there is a relationship between the artifact types and the other subjective parameters. Results: The mean noise value was significantly higher in the fail groups for the subjective parameters compared to the pass groups (p = 0.006). Specifically, noise and low contrast resolution were found to have a statistically significant positive correlation (r = 0. 183, p < 0.001). In the fail group for low contrast resolution, the failure due to artifacts was significantly higher than the pass group (p < 0.001). In contrast, no statistically significant differences were found for the mean CT number of water, noise, or image uniformity based on the types of artifacts. Conclusion: Subjective CT image parameters evaluated by a reviewer correlate with objectively measured parameters, especially noise.
