    Total dietary intake and health risks associated with exposure to aflatoxin B<sub>1</sub>, ochratoxin A and fuminisins of children in Lao Cai Province, Vietnam

    The health burden of foodborne mycotoxins is considerable, but particularly for children due to their lower detoxification capacity, rapid growth and high intake of food in proportion to their weight. Through a Total Dietary Study approach, the objective was to estimate the dietary exposure and health risk caused by mycotoxins for children under 5 years living in the Lao Cai province in northern Vietnam. A total of 40 composite food samples representing 1008 individual food samples were processed and analyzed by ELISA for aflatoxin B1, ochratoxin A and fumonisins. Results showed that dietary exposure to aflatoxin B1, ochratoxin A and total fumonisins were 118.7 ng/kgbw/day, 52.6 ng/kg bw/day and 1250.0 ng/kg bw/day, respectively. Using a prevalence of hepatitis of 1%, the risk of liver cancer related to exposure of aflatoxin B1 was 12.1 cases/100,000 individual/year. Age-adjusted margin of exposure (MOE) of renal cancer associated with ochratoxin A was 127, while MOE of liver cancer associated with fumonisins was 542. Antropometric data show that 50.4% (60/119) of children were stunted, i.e. height/length for age z-scores (HAZ) below &ndash;2, and 3.4% (4/119) of children were classified as wasted, i.e. weight for height z-scores (WHZ) below &ndash;2. A significant negative relationship between dietary exposure to individual or mixture of mycotoxins and growth of children was observed indicating that the high mycotoxin intake contributed to stunning in the children studied

    Child stunting in Vietnam has reduced substantially since the turn of the century but has remained relatively high for several years. We analysed data on children 6–59 months (n = 85,932) from the Vietnam Nutritional Surveillance System, a nationally representative cross‐sectional survey. Multivariable Poisson regression models were used to estimate relative risk (RR) of stunting, stratified by child age and ecological region. Covariates at the child, maternal, household, and environmental levels were included based on available data and the World Health Organization conceptual framework on child stunting. Among children 6–23 months, the strongest associations with child stunting were child age in years (RR: 2.49; 95% CI [2.26, 2.73]), maternal height < 145 cm compared with ≥150 cm (RR: 2.04; 95% CI [1.85, 2.26]), living in the Northeast compared with the Southeast (RR: 2.01; 95% CI [1.69, 2.39]), no maternal education compared with a graduate education (RR: 1.77; 95% CI, [1.44, 2.16]), and birthweight < 2,500 g (RR: 1.75; 95% CI [1.55, 1.98]). For children 24–59 months, the strongest associations with child stunting were no maternal education compared with a graduate education (RR: 2.07; 95% CI [1.79, 2.40]), living in the Northeast compared with the Southeast (RR: 1.94; 95% CI [1.74, 2.16]), and maternal height < 145 cm compared with ≥150 cm (RR: 1.81; 95% CI [1.69, 1.94]). Targeted approaches that address the strongest stunting determinants among vulnerable populations are needed and discussed.     The influence of human genetic variation on early transcriptional responses and protective immunity following immunization with Rotarix vaccine in infants in Ho Chi Minh City in Vietnam : a study protocol for an open single-arm interventional trial [awaiting peer review]

    Background: Rotavirus (RoV) remains the leading cause of acute gastroenteritis in infants and children aged under five years in both high- and low-middle-income countries (LMICs). In LMICs, RoV infections are associated with substantial mortality. Two RoV vaccines (Rotarix and Rotateq) are widely available for use in infants, both of which have been shown to be highly efficacious in Europe and North America. However, for unknown reasons, these RoV vaccines have markedly lower efficacy in LMICs. We hypothesize that poor RoV vaccine efficacy across in certain regions may be associated with genetic heritability or gene expression in the human host. Methods/design: We designed an open-label single-arm interventional trial with the Rotarix RoV vaccine to identify genetic and transcriptomic markers associated with generating a protective immune response against RoV. Overall, 1,000 infants will be recruited prior to Expanded Program on Immunization (EPI) vaccinations at two months of age and vaccinated with oral Rotarix vaccine at two and three months, after which the infants will be followed-up for diarrheal disease until 18 months of age. Blood sampling for genetics, transcriptomics, and immunological analysis will be conducted before each Rotarix vaccination, 2-3 days post-vaccination, and at each follow-up visit (i.e. 6, 12 and 18 months of age). Stool samples will be collected during each diarrheal episode to identify RoV infection. The primary outcome will be Rotarix vaccine failure events (i.e. symptomatic RoV infection despite vaccination), secondary outcomes will be antibody responses and genotypic characterization of the infection virus in Rotarix failure events. Discussion: This study will be the largest and best powered study of its kind to be conducted to date in infants, and will be critical for our understanding of RoV immunity, human genetics in the Vietnam population, and mechanisms determining RoV vaccine-mediated protection.     Jeokimyasal verilerin saha dağılım desenlerinin belirlenmesi çalışmalarında uzamsal küme ve aykırı değerler büyük önem taşır. Bunlar, sadece cevherleşmenin temel özelliklerini değil, aynı zamanda maden yatağının ve cevher mineralinin alansal dağılımını da karakterize ederler. Bu çalışmada, çok değişkenli verinin alansal dağılım deseni konusunda yeni bir yöntem tanıtılacak olup bu yöntemde stabil Mahalanobis uzaklığı ile lokal Moran’s I1 değeri baz alınmıştır. Uzamsal matriksi oluştururken, aralığı belirlemek üzere öncelikle Moran’s I uzamsal korelogramı kullanılmıştır. Daha sonra, belirli bir elementler topluluğu için stabil Mehalanobis aralığı hesaplanmıştır. Son olarak, hem uzamsal birlik derecesini ölçmek hem de, uzamsal küme ve aykırı değerler dahil olmak üzere Cu, Au, Mo, Ag, Pb, Zn, As ve Sb birliklerinin uzamsal dağılım desenlerini belirlemek için yersel Moran’s I1 istatistiği uygulanmıştır. Hem ham veri hem de Box-Cox dönüşüm verisi üzerinde altı değişik ölçekte (2km, 4km, 6km, 8km, 10km ve 12km) uzamsal desen analizi yapılmıştır. Elde edilen sonuçlar, Moran’s I1 ve stabil Mahalanobis ile tanımlanan uzamsal küme ve uzamsal aykırı değer alanlarının nesnel gerçeklik ile uyum sağladıklarını, ayrıca çalışma alanında bilinen maden yatakları ile de uyumlu bir ilişki gösterdiklerini ortaya koymuştur

    Childhood overweight and obesity amongst primary school children in Hai Phong City, Vietnam

    BACKGROUND AND OBJECTIVES: Childhood obesity is a rising health concern in Vietnam, however, research in this area is not extensive. The aim of this study was to determine the prevalence of childhood overweight and obesity, and to study associations between weight status and selected lifestyle factors, such as diet and physical activity levels, among children aged 6-10 years in Hai Phong City, Vietnam.METHODS AND STUDY DESIGN: Two hundred and seventy-six children from an urban and a rural primary school participated in this cross-sectional study. Data on weight, height and waist circumference were used to calculate BMI, and waist-height ratio to determine the proportion of children who were overweight, obese and had high central adiposity. Information on diet, physical activity and socioeconomic status of families was collected using questionnaires.RESULTS: Prevalences of overweight, obesity and high abdominal adiposity were 11.2%, 10.1% and 19.9%, respectively. Children who completed >=60 minutes of physical activity daily were 50% and 80% less likely to be overweight and have high abdominal adiposity, respectively. Computer usage increased the odds of overweight and high abdominal adiposity by 4.5 and 3.9 times, respectively. Mothers with higher education and income levels increased the risk of their children being overweight (pCONCLUSIONS: Physical inactivity and high maternal education and income levels increased the risk of childhood overweight and obesity in this cohort. Future interventions should target parents and their children by providing both with educational modules centred on healthy eating habits, parental feeding practices and strategies for increasing physical activity.</p

    Mapping and Assessing Landfills Surface Temperature Using Landsat 8 Satellite Data. A Case Study in Vietnam : научное издание

    Статья из журнала. Научное издание.In recent years, rapid urbanization and population growth have led to an overload of waste in big cities of Vietnam. Waste decomposition is always associated with the effects of heat generation as a result of the continuous aerobic and anaerobic processes, resulting in landfill surface temperature is significantly higher than the surrounding area. This paper presents the results of assessment of land surface temperature distribution in two largest landfills in Vietnam, including Nam Son landfill (Hanoi city, northern Vietnam) and Da Phuoc landfill (Ho Chi Minh city, southern Vietnam). Landsat 8 satellite image data is used to calculate land surface temperatures based on the split-window method. The results showed that the landfill surface temperature was much higher than the surrounding area, even compared to the areas characterized by impervious surfaces. The difference between the landfill surface temperature and temperature in areas covered by vegetation was about 20 degrees C. The results obtained in this study provide input information to develop monitoring models of pollution at landfills

    Vietnam launched the national Expanded Program on Immunization in 1981. Since then, this program has contributed signi cantly to the improvement of child health and to the reduction of child mortality rate. Despite of the fact that the coverage of the national EPI keeps expanding, the number of children who complied with the recommended immunization schedule remains low. This article studies the progress of the timeliness childhood immunization compliance among children between 0-5 years of age in Vietnam from 2006 to 2014 and analyzes the socio-economic factors that account for the changes of the compliance rate during this period. The dataset is extracted from the Multiple Indicator Cluster Survey in 2006 and 2014. We rst identify the socio-economic factors that impact on the vaccination compliance rate using a logistic regression model. Next, we apply the decomposition method to determine the contribution of each factor on the evolution of the timeliness childhood immunization compliance. The progress of the timeliness childhood immunization has been positive and the major contribution comes from the structure e ect (unmeasured e ect). Rural areas show a stronger improvement as of 2014. Among the socio-economic factors, mother education and birth order are the ones that have the larger in uence on the childhood immunization compliance rate. However, these factors have di erent implications in urban and rural areas. These ndings are critical to the current context of Vietnam where the government is designing a strategy focusing on the e ectiveness rather than the traditional coverage indicator
