    Scoping review : intergenerational resource transfer and possible enabling factors

    We explore the intergenerational pattern of resource transfer and possible associated factors. A scoping review was conducted of quantitative, peer-reviewed, English-language studies related to intergenerational transfer or interaction. We searched AgeLine, PsycINFO, Social Work Abstracts, and Sociological Abstracts for articles published between Jane 2008 and December 2018. Seventy-five studies from 25 countries met the inclusion criteria. The scoping review categorised resource transfers into three types: financial, instrumental, and emotional support. Using an intergenerational solidarity framework, factors associated with intergenerational transfer were placed in four categories: (1) demographic factors (e.g., age, gender, marital status, education, and ethno-cultural background); (2) needs and opportunities factors, including health, financial resources, and employment status; (3) family structures, namely, family composition, family relationship, and earlier family events; and (4) cultural-contextual structures, including state policies and social norms. Those factors were connected to the direction of resource transfer between generations. Downward transfers from senior to junior generations occur more frequently than upward transfers in many developed countries. Women dominate instrumental transfers, perhaps influenced by traditional gender roles. Overall, the pattern of resource transfer between generations is shown, and the impact of social norms and social policy on intergenerational transfers is highlighted. Policymakers should recognise the complicated interplay of each factor with different cultural contexts. The findings could inform policies that strengthen intergenerational solidarity and support.</jats:p

    Preventable in-hospital medical injury under the "no fault" system in New Zealand

    Objectives: To describe the pattern of preventable in-hospital medical injury under the "no fault" system and to assess the level of serious preventable patient harm. Design: Cross sectional survey using a two stage retrospective assessment of medical records conducted by structured implicit review. Setting: General hospitals with over 100 beds providing acute care in New Zealand. Participants: A sample of 6579 patients admitted in 1998 to 13 hospitals selected by stratified systematic list sample. Main outcome measures: Occurrence, preventability, and impact of adverse events. Results: Over 5% of admissions were associated with a preventable in-hospital event, of which nearly half had an element of systems failure. The elderly, ethnic minority groups, and particular clinical areas were at higher risk. The chances of a patient experiencing a serious preventable adverse event subsequent to hospital admission were just under 1%, a figure close to published results from comparable studies under tort. On average, these events required an additional 4 weeks in hospital. System related issues of protocol use and development, communication, and organisation, as well as requirements for consultation and education, were pre-eminent. Conclusions: The risk of serious preventable in-hospital medical injury for patients in New Zealand, a well established "no fault" jurisdiction, is within the range reported in comparable investigations under tort

    Coronary events and exposure to environmental tobacco smoke: a case-control study from Australia and New Zealand

    OBJECTIVES—To estimate the relative risk of coronary heart disease (CHD) associated with exposure to environmental tobacco smoke (ETS).
DESIGN—Population-based case-control study.
SUBJECTS—Cases were 953 people identified in a population register of coronary events, and controls were 3189 participants in independent community-based risk factor prevalence surveys from the same study populations.
SETTING—Newcastle, Australia and Auckland, New Zealand.
MAIN OUTCOME MEASURES—Acute myocardial infarction or coronary death.
RESULTS—After adjusting for the effects of age, education, history of heart disease, and body mass index, women had a statistically significant increased risk of a coronary event associated with exposure to ETS (relative risk (RR) = 1.99; 95% confidence interval (CI) = 1.40-2.81). There was little statistical evidence of increased risk found in men (RR = 1.02, 95% CI = 0.81-1.28).
CONCLUSION—Our study found evidence for the adverse effects of exposure to ETS on risk of coronary heart disease among women, especially at home. For men the issue is unclear according to the data from our study. Additional studies with detailed information on possible confounders and adequate statistical power are needed. Most importantly, they should use methods for measuring exposure to ETS that are sufficiently accurate to permit the investigation of dose-response relationships.

    Marine biofouling field tests, settlement assay and footprint micromorphology of cyprid larvae of Balanus amphitrite on model surfaces

    Atomic force microscopy (AFM), laboratory settlement assays and field tests were used to correlate cyprid footprint (FP) morphology with the behaviour of cyprids on different substrata. AFM imaging under laboratory conditions revealed more porous and larger FPs on glass exposing a CH3-surface than on aminosilane functionalised (NH2-) surfaces. The secreted FP volume was found to be similar on both substrata (2.1-2.6 µm3). Laboratory settlement assays and marine field tests were performed on three substrata, viz. untreated clean glass, NH2-glass, and CH3-glass. The results distinguished settlement preferences for NH2-glass and untreated glass over CH3-terminated surfaces, suggesting that cyprids favour settling on hydrophilic over hydrophobic surfaces. On combining observations from different length scales, it is speculated that the confined FP size on NH2-glass may induce a higher concentration of the settlement inducing protein complex. Settlement may be further facilitated by a stronger adherence of FP adhesives to the NH2-surface via Coulombic interactions
