Techniques of Inflow Occlusion for Liver Resection
Limited resection can be a therapeutic approach in patients with cirrhosis with very low
remnant hepatic function after resection. In this study, two hilar vascular clamping
methods (hilar selective clamping [n=13] and hilar lobar clamping method [n=8]), which were used for resection ofhepatocellular carcinoma in patients with cirrhosis, were
compared based on cardiovascular stability during clamping, intraoperative bleeding,
operative time and postoperative course. In the past, the Pringle method had been used
(n=19) and those instances were included for comparison. The mean operation time of
the lobar clamping group was 209 ± 44 minutes, which was significantly less than that of
the selective clamping group (259 ± 44 minutes, p < 0.05). Furthermore, the mean
intraoperative blood loss of the lobar clamping group was 920 ± 400 milliliters, which
was significantly less than that of the selective clamping group (1,640 ± 590 milliliters,
p < 0.01). The postoperative total bilirubin and glutamine-oxaloacetic transaminase
levels tended to be high in the Pringle group, but there was no significant difference
between the groups. Although the blood pressure during clamping significantly decreased
in all groups, the decrease was profound in the Pringle group as compared with those in
the other two groups. Thus, as a method for controlling afferent bloodflow during hepatic
resection in patients with cirrhosis, we recommend the lobar clamping method as a
simple, safe and effective way to minimize bleeding and maintain cardiovascular
stability
Superconducting diamagnetic fluctuations in ropes of carbon nanotubes
We report low-temperature magnetisation measurements on a large number of
purified ropes of single wall carbon nanotubes. In spite of a large
superparamagnetic contribution due to the small ferromagnetic catalytical
particles still present in the sample, at low temperature () and low
magnetic field (), a diamagnetic signal is detectable. This low
temperature diamagnetism can be interpreted as the Meissner effect in ropes of
carbon nanotubes which have previously been shown to exhibit superconductivity
from transport measurements.Comment: 10 pages 3 figure
Evidence for distinct polymer chain orientations in KC60 and RbC60
The KC60 and RbC60 polymer phases exhibit contrasting electronic properties
while powder diffraction studies have revealed no definite structural
difference. We have performed single crystal X-ray diffraction and diffuse
scattering studies of these compounds. It is found that KC60 and RbC60 possess
different chain orientations about their axes, which are described by distinct
space groups Pmnn and I2/m, respectively. Such a structural difference will be
of great importance to a complete understanding of the physical properties.Comment: To be published in Phys. Rev. Let
Modification of hyperacute canine renal homograft and pig-to-dog heterograft rejection by the intra-arterial infusion of citrate
Hyperacute rejection was studied in seven presensitized canine recipients of renal homografts and in 22 canine recipients of pig kidneys. Under both experimental circumstances, untreated animals rejected the grafts within a few minutes. With the intra-arterial infusion of sodium citrate, hyperacute heterograft rejection was forestalled for the duration of therapy, and with homografts the benefit outlasted treatment by many hours. The protection of heterografts was not due solely to anticoagulation by the citrate, since heparin in large doses did not accomplish the same thing. A contributory factor may have been complement inhibition since removal of complement by the grafts was greatly reduced in the citrate-treated animals. © 1971
Delayed biliary duet obstruction after orthotopic liver transplantation
After orthotopic liver transplantation and biliary reconstruction by cholecystoduodenostomy, four of 40 patients developed delayed obstruction of the cystic duct. The recipients had the clinical syndrome of fulminating cholangitis with jaundice, fever, leukocytosis, toxemia, and bacteremia. All four patients died; of the four, two patients died despite late reoperation and re-establishment of bile drainage by choledochoenterostomy. In all four cases, a factor contributing to the biliary obstruction may have been infection of the extrahepatic biliary ducts with or without ulceration, and in three of the livers, there was evidence of infection of the ducts with CMV. If cholecystoduodenostomy is used in future cases, prompt re-exploration and conversion to choledochoenterostomy should be considered if the diagnosis of duct obstruction, cholangitis, and persistent bacteremia are made. © 1972
Far-infrared vibrational properties of high-pressure-high-temperature C60 polymers and the C60 dimer
We report high-resolution far-infrared transmission measurements of the 2 + 2 cycloaddition C-60 dimer and two-dimensional rhombohedral and one-dimensional orthorhombic high-pressure high-temperature C60 polymers. In the spectral region investigated(20-650 cm(-1)), we see no low-energy interball modes, but symmetry breaking of the linked C-60 balls is evident in the complex spectrum of intramolecular modes. Experimental features suggest large splittings or frequency shifts of some IhC60-derived modes that are activated by symmetry reduction, implying that the balls are strongly distorted in these structures. We have calculated the vibrations of all three systems by first-principles quantum molecular dynamics and use them to assign the predominant IhC60 symmetries of observed modes. Pur calculations show unprecedentedly large downshifts of T-1u(2)-derived modes and extremely large splittings of other modes, both of which are consistent with the experimental spectra. For the rhombohedral and orthorhombic polymers, the T-1u(2)-derived mode that is polarized along the bonding direction is calculated to downshift below any T-1u(1)-derived modes. We also identify a previously unassigned feature near 610 cm(-1) in all three systems as a widely split or shifted mode derived from various silent IhC60 vibrations, confirming a strong perturbation model for these linked fullerene structures
Far-infrared vibrational properties of linear C60 polymers: A comparison between neutral and charged materials
We report the far-infrared transmittance spectrum of a pure phase of the orthorhombic high-temperature and high-pressure C-60 polymer and compare the results with a previously published spectrum of the charged RbC60 orthorhombic polymer. Assignments for both spectra are made with the aid of first-principles quantum molecular dynamics simulations of the two materials. We find that the striking spectral differences between the neutral and charged linear fullerene polymers can be fully accounted for by charge effects on the C-60 ball
