    Sporadic E layer has essential influences on propagation of radio waves in decametric range, both expanding range of working frequencies, and shielding reflection of radio waves from overlying ionosphere. In report considered reflection of radio waves from Es layer on a line Cyprus-Kazan in the extent of 2600 km, at range of propagation limiting one-hop reflection for heights of arrangement of layer. Researches were spent by method of LFM sounding.     Application of HF Doppler measurements for the investigation of internal atmospheric waves in the ionosphere

    We present results of the spectral analysis of data series of Doppler frequency shifted signals reflected from the ionosphere, using experimental data received at Kazan University, Russia. Spectra of variations with periods from 1 min to 60 days have been calculated and analyzed for different scales of periods. The power spectral density for spring and winter differs by a factor of 3-4. Local maxima of variation amplitude are detected, which are statistically significant. The periods of these amplitude increases range from 6 to 12 min for winter, and from 24 to 48 min for autumn. Properties of spectra for variations with the periods of 1-72 h have been analyzed. The maximum of variation intensity for all seasons and frequencies corresponds to the period of 24 h. Spectra of variations with periods from 3 to 60 days have been calculated. The maxima periods of power spectral density have been detected by the MUSIC method for the high spectral resolution. The detected periods correspond to planetary wave periods. Analysis of spectra for days with different level of geomagnetic activity shows that the intensity of variations for days with a high level of geomagnetic activity is higher. © 2009 COSPAR

    Childhood intussusception in Uzbekistan: Analysis of retrospective surveillance data

    <p>Abstract</p> <p>Background</p> <p>Estimates of baseline incidence of childhood intussusception could help safety monitoring after the introduction of rotavirus vaccines. We studied the incidence of intussusception in Uzbekistan, a GAVI-fund eligible state in Central Asia.</p> <p>Methods</p> <p>We retrospectively reviewed intussusception cases in children <2 years of age treated during 2004-2008 at 15 hospitals in the Bukhara region of Uzbekistan. Demographic and clinical data as well as information on diagnostic and treatment practices were obtained from hospital records. We categorized cases using the Brighton collaboration clinical case definition and calculated the national incidence rate.</p> <p>Results</p> <p>Over a 5-year study period, 67 confirmed cases were identified, of which 67% were boys. The median age was 12 months, and no seasonal trend in the distribution of cases was observed. The diagnostic methods used included abdominal radiography (87%) and ultrasonography (57%). Intussusception reduction by air enema was successful in 33 (49%) patients and 34 (50%) cases underwent surgery. A total of 4 deaths occurred, including 3 deaths in infants aged 0-6 months. The median length of hospital stay was 7.3 (range 0-37) days. The incidence of intussusception is estimated at 23 (95% CI 13.6-32.4) cases per 100,000 child-years, corresponding to approximately 237 cases annually.</p> <p>Conclusions</p> <p>This is the first study to estimate the incidence of childhood intussusception prior to the introduction of the rotavirus vaccination in Uzbekistan. A prospective surveillance system using a standardized case definition is needed in order to better examine the occurrence of intussusception in developing countries.</p

    Трансплантация легких при первичной легочной артериальной гипертензии: особенности периоперационного периода

    Lung transplantation (LT) for idiopathic pulmonary arterial hypertension (IPAH) now is the only radical treatment of this disease.Aim: to analyze own experience of performing LT in patients with IPAH.Materials and methods. 8 adult IPAH patients, who underwent LT between 2014 and october 2018, were included. In 7 of 8 patients undergoing bilateral lung transplantation on intraoperative venoarterial extracorporeal membrane oxygenation (VA ECMO) with prolongation into the postoperative period.Results. VA ECMO support was prolonged into postoperative period 6 and 7 days respectively in 2 (25,0%) patients and 3 days in 6 (75,0%) patients. Hospital mortality in IPAH patients was 1.Conclusions. Own experience demonstrates that LT is an effective method of treatment in patients with IPAH. Hospital, 1- and 3-year survival rates for the patient collective were 87.5, 75.0 and 75.0% respectively.Первичная легочная артериальная гипертензия (ПЛАГ) – необратимое заболевание, единственным методом радикального лечения которого является трансплантация легких (ТЛ).Цель исследования: анализ собственного опыта выполнения ТЛ у пациентов с ПЛАГ.Материалы и методы. В исследование включили 8 взрослых реципиентов, которым была выполнена ТЛ в период с 2014-го по октябрь 2018 г. Всем реципиентам была выполнена двусторонняя последовательная трансплантация легких. У 7 из 8 реципиентов ТЛ осуществляли в условиях периферической ВА ЭКМО.Результаты. В послеоперационном периоде первичная дисфункция 3-й степени по классификации ISHLT развилась у 2 (25,0%) из 8 реципиентов, которая проявилась невозможностью прекращения ВА ЭКМО. Общая продолжительность применения ЭКМО у этих пациентов (n = 2) составила 6 и 7 суток. У других 6 (75,0%) реципиентов проявления ранней дисфункции пересаженных легких не носили столь напряженного характера (2-я степень по классификации ISHLT), и продолжительность послеоперационной ВА ЭКМО составила 3 суток. Госпитальная летальность у реципиентов легких, оперированных по поводу ЛАГ, составила 1 наблюдение.Заключение. Собственный опыт демонстрирует, что ТЛ является эффективной лечебной мерой у пациентов с ПЛАГ с показателями госпитальной, 1- и 3-летней выживаемости соответственно 87,5; 75,0 и 75,0%
