    Constructing a dental electronic information resource (DEIR)

    This paper discusses the construction of a dental electronic information resource (DEIR). Development is based on the Internet gopher client/server model. Information is organized in a series of hierarchical menus. A DEIR can be browsed by traversing the menu hierarchy or by performing keyword searches. Searches can result in a menu of found items from across the Internet. Initially, contents of the DEIR will be text files. Electronic copy will be given to an editorial board that will review submissions for conformity to publication standards. A current estimate for electronic storage costs of a DEIR is $1.25/Mb per year. There will be additional costs associated with any purchases of data (e.g. Current Contents), production of data (e.g. development time), and editorial and managerial activities. The DEIR will be accessible from any computer or terminal that can connect to the Internet. Primary user groups will be faculty, staff, and students of dental schools.     A Community-Based Dental Program for Older Adults

    The planning and implementation of a community-based outreach program for older adults is described. Objectives of the program were to provide dental health education to older persons at their place of residence, to improve access to dental care for that population, and to increase the number of older adults treated at a dental facility administered by the Department of Community Dentistry, University of Michigan. Data collected during encounters with participants are reported to supplement the description of the program. In the first year, 98 older adults (mean age 71.3 years)participated in the outreach program which was directed by a dental hygienist. Of those persons whose initial encounter was with the outreach program, 47 percent eventually contacted the dental care facility and 36 percent completed treatment. Persons who elected to seek treatment averaged 3.9 encounters with the hygienist during the program; persons who did not seek treatment averaged 2.2 encounters.     Preventive Behaviors as Correlates of Periodontal Health Status

    Objectives: This investigation examined current practices of brushing, flossing, and periodic dental visits and their association with periodontal health status. Methods : Data were collected using face-to-face interviews and 40-minute in-home dental examinations with a probability sample of adults 18 years of age or older, having at least one tooth, and living in housing units in the Detroit tricounty area. Complete examinations were performed on 319 individuals. Results : On average, subjects reported brushing their teeth about twice a day. About one-third of the population reported flossing at least once a day. Loss of periodontal attachment was related to frequency of brushing while subjects who exhibited acceptable flossing ability had less plaque and calculus, shallower pocket depths, and less attachment loss. Subjects reporting a periodic dental visit at least once a year had less plaque, gingivitis, and calculus than subjects reporting less frequent visits. In regression analyses, brushing thoroughness, flossing ability and frequency, and dental visit frequency were predictors of lower plaque, gingivitis, and calculus scores. In turn, these scores were predictors of shallower pocket depths and less attachment loss. Conclusions : Brushing, flossing, and periodic dental visits were correlated with better periodontal health.     Fissure Sealant Knowledge and Characteristics of Parents as a Function of Their Child's Sealant Status

    A survey was conducted to identify and compare sealant knowledge and sources of sealant information of parents whose children had and had not received fissure sealants. The socioeconomic characteristics of these individuals were also compared. The sealant group was composed of the parents of children found to have a sealant on at least one permanent tooth during dental examinations of 2,036 elementary schoolchildren in southwestern Michigan. Another group of children was selected from the same population and matched to the children with sealants by child's age, sex, school location, and community. Surveys were returned by 210 of 260 sets of parents (81% response rate). Significant differences were found between the two groups with regard to parents' ages and levels of income. Parents of children with sealants had more correct information about the procedure and 74 percent of these individuals reported that the dental office was their primary source of information. For the group without sealants, 48 percent of respondents reported no source of sealant information.     Educating Dentists about Fissure Sealants: Effects on Knowledge, Attitudes, and Use

    This study evaluated the effects of three modes of education on dentists' knowledge, attitudes, and use of pit and fissure sealants. A randomly selected group of dentists was invited to participate in a sealant education initiative. During a 12-month period, a total of 662 dentists either (1) attended continuing education courses, (2) received written materials and videotapes by mail, or (3) received only written materials by mail. A comparison group (n=337) received no materials until after the education phase and evaluation had been completed. Pre-and postintervention surveys were used to measure changes in knowledge, attitudes, and sealant use. Response rates to the two surveys were 62 percent and 76 percent, respectively. Preintervention values for knowledge scores, an attitude scale, and sealant use were similar among the four groups. At followup, the three education groups had significantly higher knowledge scores than the comparison group. Attitude values for all but one group were not significantly different, and sealant use by all groups was identical. The numbers of respondents not using sealants declined slightly between surveys in the three education groups while rising slightly in the No-Education Group. Because program outcomes were similar to those of another sealant initiative, it can probably be concluded that continuing education will increase dentists' knowledge about sealants, but have little effect upon their attitudes or their use of the technique.     Evaluation and Use of an Index of Oral Health Status

    Objectives: The goals of this investigation were (1) to evaluate the Oral Health Status Index in relation to demographic characteristics, socioeconomic status, and preventive behaviors of an adult population; and (2) to understand how individual index components performed as indicators of oral health status compared to the composite index. Methods : The Oral Health Status Index (OHSI) was used on a probability sample of adults, aged 18–93 years, living in the Detroit tricounty area. Data were collected on 509 subjects via in-home dental examinations. Bivariate and multivariate analyses were used to compare the OHSI and its components, including decayed, missing, and replaced teeth, free ends, and moderate and severe periodontal disease measures. Results : The mean OHSI score for subjects was 77.3 (se=1.83) with a range of -8.0 to 100.0. In regression analyses, OHSI scores were positively correlated with subjects' education level, self-rated oral health scores, and frequency of dental checkups and negatively correlated with age, nonwhite race, and smoking. Of the index components, missing teeth performed well as an indicator of oral health status. Missing teeth were positively correlated with age, nonwhite race, and smoking and negatively correlated with education level, self-rated oral health, and use of Medicaid. About 53 percent of variance in OHSI scores was explained by the multivariate models, compared to 46 percent for missing teeth. Conclusions : Choosing an indicator of oral health status likely will depend upon the characteristics of the population to be studied. As a composite measure of oral health status, the OHSI performed acceptably; however, missing teeth, an index component, also worked well.     Polarimetric and Multi-Doppler Radar Observations of Sprite-producing Storms

    Sprites are caused by luminous electrical breakdown of the upper atmosphere, and frequently occur over large mesoscale precipitation systems. Two spriteproducing storms (on 8 and 25 June) were observed in Colorado during the summer of 2012. Unlike most past studies of sprites, these storms were observed by a polarimetric radar the CSUCHILL facility which provided both PPI and RHI scans of the cases. Also available were multipleDoppler syntheses from CSUCHILL, local NEXRAD radars, and the CSUPawnee radar; as well as data from the Colorado Lightning Mapping Array (COLMA), high speed cameras, and other lightningdetection instrumentation. This unique dataset provided an unprecedented look at the detailed kinematic and microphysical structures of the thunderstorms as they produced sprites, including electrical alignment signatures in the immediate location of the charge layers neutralized by spriteparent positive cloudtoground lightning strokes. One of the spriteproducing cases (25 June) featured an anomalous charge structure and may serve as a model for how sprites can be produced over convection rather than the more typical stratiform regions. Also to be presented will be evidence for advection of charge into a common stratiform precipitation region (on 8 June), which was then tapped by lightning originating from multiple different convective cores to produce sprites. Depending on the outcome of the 2013 convective season, polarimetric data from additional storms that produce sprites and other transient luminous events (TLEs) may be presented

    Decision-making among experts in advanced Hodgkin Lymphoma.

    BACKGROUND In the treatment of advanced Hodgkin Lymphoma (aHL), based on guidelines a multitude of treatment options are available. The availability of PET guided decision-making and new therapeutic agents increase the complexity of the decision-making process. METHODS Thirteen experts of Swiss university and cantonal hospitals in Switzerland were asked to describe their institutional decision-making practice in aHL. Variables influencing the decision-making process were identified, standardized and converted into decision trees for analysis of consent and discrepancies. The algorithms of all participating experts were analyzed with the objective consensus methodology. RESULTS Four decision criteria (age, fertility preservation, fitness, interim PET) and 12 unique treatment regimens were identified. Consensus for the treatment of aHL for young and fit, as well as for older patients without comorbidity was found. Large heterogeneity was identified with use of a variety of different regimens for unfit patients with aHL and for young female patients with a desire of fertility preservation. CONCLUSION Four major decision criteria were identified allowing the representation of expert's approach to first-line treatment of aHL. Among Swiss experts, consensus for a PET guided curative treatment of aHL was identified. The use of a multitude of treatment regimens was observed for older and comorbid (unfit) aHL patients, highlighting the need for clinical trials and recommendations for this group of patients

    The First High Redshift Quasar from Pan-STARRS

    We present the discovery of the first high redshift (z > 5.7) quasar from the Panoramic Survey Telescope and Rapid Response System 1 (Pan-STARRS1 or PS1). This quasar was initially detected as an i dropoutout in PS1, confirmed photometrically with the SAO Widefield InfraRed Camera (SWIRC) at Arizona's Multiple Mirror Telescope (MMT) and the Gamma-Ray Burst Optical/Near-Infrared Detector (GROND) at the MPG 2.2 m telescope in La Silla. The quasar was verified spectroscopically with the the MMT Spectrograph, Red Channel and the Cassegrain Twin Spectrograph (TWIN) at the Calar Alto 3.5 m telescope. It has a redshift of 5.73, an AB z magnitude of 19.4, a luminosity of 3.8 x 10^47 erg/s and a black hole mass of 6.9 x 10^9 solar masses. It is a Broad Absorption Line quasar with a prominent Ly-beta peak and a very blue continuum spectrum. This quasar is the first result from the PS1 high redshift quasar search that is projected to discover more than a hundred i dropout quasars, and could potentially find more than 10 z dropout (z > 6.8) quasars.Comment: 8 pages, 7 figure

    PHAT Stellar Cluster Survey I. Year 1 Catalog and Integrated Photometry

    The Panchromatic Hubble Andromeda Treasury (PHAT) survey is an on-going Hubble Space Telescope (HST) multi-cycle program to obtain high spatial resolution imaging of one-third of the M31 disk at ultraviolet through near-infrared wavelengths. In this paper, we present the first installment of the PHAT stellar cluster catalog. When completed, the PHAT cluster catalog will be among the largest and most comprehensive surveys of resolved star clusters in any galaxy. The exquisite spatial resolution achieved with HST has allowed us to identify hundreds of new clusters that were previously inaccessible with existing ground-based surveys. We identify 601 clusters in the Year 1 sample, representing more than a factor of four increase over previous catalogs within the current survey area (390 arcmin^2). This work presents results derived from the first \sim25% of the survey data; we estimate that the final sample will include \sim2500 clusters. For the Year 1 objects, we present a catalog with positions, radii, and six-band integrated photometry. Along with a general characterization of the cluster luminosities and colors, we discuss the cluster luminosity function, the cluster size distributions, and highlight a number of individually interesting clusters found in the Year 1 search.Comment: 26 pages, 22 figures, Accepted by Ap
