    Seroprevalence of Pandemic Influenza H1N1 in Ontario from January 2009–May 2010

    We designed a seroprevalence study using multiple testing assays and population sources to estimate the community seroprevalence of pH1N1/09 and risk factors for infection before the outbreak was recognized and throughout the pandemic to the end of 2009/10 influenza season.Residual serum specimens from five time points (between 01/2009 and 05/2010) and samples from two time points from a prospectively recruited cohort were included. The distribution of risk factors was explored in multivariate adjusted analyses using logistic regression among the cohort. Antibody levels were measured by hemagglutination inhibition (HAI) and microneutralization (MN) assays.Residual sera from 3375 patients and 1024 prospectively recruited cohort participants were analyzed. Pre-pandemic seroprevalence ranged from 2%-12% across age groups. Overall seropositivity ranged from 10%-19% post-first wave and 32%-41% by the end of the 2009/10 influenza season. Seroprevalence and risk factors differed between MN and HAI assays, particularly in older age groups and between waves. Following the H1N1 vaccination program, higher GMT were noted among vaccinated individuals. Overall, 20-30% of the population was estimated to be infected.Combining population sources of sera across five time points with prospectively collected epidemiological information yielded a complete description of the evolution of pH1N1 infection

    Assessing secondary attack rates among household contacts at the beginning of the influenza A (H1N1) pandemic in Ontario, Canada, April-June 2009: A prospective, observational study

    <p>Abstract</p> <p>Background</p> <p>Understanding transmission dynamics of the pandemic influenza A (H1N1) virus in various exposure settings and determining whether transmissibility differed from seasonal influenza viruses was a priority for decision making on mitigation strategies at the beginning of the pandemic. The objective of this study was to estimate household secondary attack rates for pandemic influenza in a susceptible population where control measures had yet to be implemented.</p> <p>Methods</p> <p>All Ontario local health units were invited to participate; seven health units volunteered. For all laboratory-confirmed cases reported between April 24 and June 18, 2009, participating health units performed contact tracing to detect secondary cases among household contacts. In total, 87 cases and 266 household contacts were included in this study. Secondary cases were defined as any household member with new onset of acute respiratory illness (fever or two or more respiratory symptoms) or influenza-like illness (fever plus one additional respiratory symptom). Attack rates were estimated using both case definitions.</p> <p>Results</p> <p>Secondary attack rates were estimated at 10.3% (95% CI 6.8-14.7) for secondary cases with influenza-like illness and 20.2% (95% CI 15.4-25.6) for secondary cases with acute respiratory illness. For both case definitions, attack rates were significantly higher in children under 16 years than adults (25.4% and 42.4% compared to 7.6% and 17.2%). The median time between symptom onset in the primary case and the secondary case was estimated at 3.0 days.</p> <p>Conclusions</p> <p>Secondary attack rates for pandemic influenza A (H1N1) were comparable to seasonal influenza estimates suggesting similarities in transmission. High secondary attack rates in children provide additional support for increased susceptibility to infection.</p

    Machine Learning Identifies Clinical and Genetic Factors Associated With Anthracycline Cardiotoxicity in Pediatric Cancer Survivors

    BACKGROUND Despite known clinical risk factors, predicting anthracycline cardiotoxicity remains challenging. OBJECTIVES This study sought to develop a clinical and genetic risk prediction model for anthracycline cardiotoxicity in childhood cancer survivors. METHODS We performed exome sequencing in 289 childhood cancer survivors at least 3 years from anthracycline exposure. In a nested case-control design, 183 case patients with reduced left ventricular ejection fraction despite low-dose doxorubicin (\u3c= 250 mg/m(2)), and 106 control patients with preserved left ventricular ejection fraction despite doxorubicin \u3e250 mg/m(2) were selected as extreme phenotypes. Rare/low-frequency variants were collapsed to identify genes differentially enriched for variants between case patients and control patients. The expression levels of 5 top-ranked genes were evaluated in human induced pluripotent stem cell-derived cardiomyocytes, and variant enrichment was confirmed in a replication cohort. Using random forest, a risk prediction model that included genetic and clinical predictors was developed. RESULTS Thirty-one genes were differentially enriched for variants between case patients and control patients (p \u3c 0.001). Only 42.6% case patients harbored a variant in these genes compared to 89.6% control patients (odds ratio: 0.09; 95% confidence interval: 0.04 to 0.17; p = 3.98 x 10(-15)). A risk prediction model for cardiotoxicity that included clinical and genetic factors had a higher prediction accuracy and lower misclassification rate compared to the clinical-only model. In vitro inhibition of gene-associated pathways (PI3KR2, ZNF827) provided protection from cardiotoxicity in cardiomyocytes. CONCLUSIONS Our study identified variants in cardiac injury pathway genes that protect against cardiotoxicity and informed the development of a prediction model for delayed anthracycline cardiotoxicity, and it also provided new targets in autophagy genes for the development of cardio-protective drugs

    Plant maturity and nitrogen fertilization affected fructan metabolism in harvestable tissues of timothy (Phleum pratense L.)

    International audienceTimothy (Phleum pratense L.) is an important grass forage used for pasture, hay, and silage in regions with cool and humid growth seasons. One of the factors affecting the nutritive value of this grass is the concentration of non-structural carbohydrates (NSC), mainly represented by fructans. NSC concentration depends on multiple factors, making it hardly predictable. To provide a better understanding of NSC metabolism in timothy, the effects of maturity stage and nitrogen (N) fertilization level on biomass, NSC and N-compound concentrations were investigated in the tissues used for forage (leaf blades and stems surrounded by leaf sheaths) of hydroponically grown plants. Moreover, activities and relative expression level of enzymes involved in fructan metabolism were measured in the same tissues. Forage biomass was not altered by the fertilization level but was strongly modified by the stage of development. It increased from vegetative to heading stages while leaf-to-stem biomass ratio decreased. Total NSC concentration, which was not altered by N fertilization level, increased between heading and anthesis due to an accumulation of fructans in leaf blades. Fructan metabolizing enzyme activities (fructosyltransferase-FT and fructan exohydrolase-FEH) were not or only slightly altered by both maturity stage and N fertilization level. Conversely, the relative transcript levels of genes coding for enzymes involved in fructan metabolism were modified by N supply (PpFT1 and Pp6-FEH1) or maturity stage (PpFT2). The relative transcript level of PpFT1 was the highest in low N plants while that of Pp6-FEH1 was the highest in high N plants. Morevoer, transcript level of PpFT1 was negatively correlated with nitrate concentration while that of PpFT2 was positively correlated with sucrose concentration. This distinct regulation of the two genes coding for 6-sucrose: fructan fructosyltransferase (6-SFT) may allow a fine adequation of C allocation towards fructan synthesis in response to carbon and N availability. Contrary to fructans, starch content increased in low N plants, suggesting different regulatory mechanisms and/or sensitivity of starch and fructan metabolism in relation to the N status

    Timothy (Phleum pratense L.) is an important forage grass used for pasture, hay and silage in regions with cool and humid growth seasons. Harvesting conditions could reduce its nutritive value, particularly with extended wilting periods. To understand how daytime or night-time wilting influences the nutritive value of timothy, this study investigated the metabolism of non-structural carbohydrates, including fructan and starch, together with total soluble protein and amino acid patterns in timothy plants harvested at two maturity stages (heading and anthesis) and wilted under controlled conditions for 24h at two temperatures (15 degrees C, 20 degrees C) and two light regimes (darkness, light) by simulating different wilting management practices. Correlation analysis with the whole dataset showed that soluble protein, glucose and starch contents declined in plant tissues concomitantly with water loss, while amino acid, sucrose and fructose contents increased. Transient increase in amino acid content suggests that the decrease in protein content was due to proteolysis during wilting. Sucrose and fructose contents generally increased in plant tissues harvested at anthesis and wilted in light whereas they were unaffected in plants wilted in darkness. Fructan content remained stable. Fructan exohydrolase (FEH) and soluble acid invertase (INV) activities were well preserved during the first 12h of wilting and might facilitate the fermentation process at the beginning of ensiling by supplying fructose from fructans and hexoses from sucrose to the fermentive bacteria

    PSYM-WIDE: A Survey for Large-separation Planetary-mass Companions to Late Spectral Type Members of Young Moving Groups

    We present the results of a direct imaging survey for very large separation (>100 au), low-mass companions around 95 nearby young K5–L5 stars and brown dwarfs. They are high-likelihood candidates or confirmed members of the young (150 Myr) ÎČ Pictoris and AB Doradus moving groups (ABDMG) and the TW Hya, Tucana–Horologium, Columba, Carina, and Argus associations. Images in iÂą and zÂą filters were obtained with the Gemini Multi-Object Spectrograph (GMOS) on Gemini South to search for companions down to an apparent magnitude of zÂą ∌ 22–24 at separations 20″ from the targets and in the remainder of the wide 5 5 × 5 5 GMOS field of view. This allowed us to probe the most distant region where planetary-mass companions could be gravitationally bound to the targets. This region was left largely unstudied by past high-contrast imaging surveys, which probed much closer-in separations. This survey led to the discovery of a planetary-mass (9–13 MJup) companion at 2000 au from the M3V star GU Psc, a highly probable member of ABDMG. No other substellar companions were identified. These results allowed us to constrain the frequency of distant planetary-mass companions (5–13 MJup) to 0.84 0.66 6.73-+% (95% confidence) at semimajor axes between 500 and 5000 au around young K5–L5 stars and brown dwarfs. This is consistent with other studies suggesting that gravitationally bound planetary-mass companions at wide separations from low-mass stars are relatively rare.SkyMapper data products were used.
