    Importância da Avaliação Funcional Respiratória em Idade Precoce

    As provas de função respiratória são instrumentos de quantificação objectiva do compromisso pulmonar utilizadas na confirmação diagnóstica, monitorização após terapêutica e avaliação do prognóstico de diversas patologias. Tem sido possível nos últimos anos a sua aplicação em crianças mais jovens, com benefícios evidentes, pela possibilidade de atempadamente monitorizar e alterar o curso natural de certas doenças. As provas de função respiratória no lactente e em idade pré-escolar revestem-se de particularidades metodológicas, tendo ocorrido recentemente um grande interesse da comunidade científica neste âmbito, com grandes progressos nesta área. Pretende-se no presente artigo enumerar as técnicas de função respiratória disponíveis em idade precoce, explicitando a sua fundamentação teórica, metodologia e aplicabilidade. Será dado particular destaque às técnicas mais inovadoras aplicadas no lactente – técnicas de compressão torácica rápida em volume corrente e volume aumentado, bem como em idade pré-escolar – pletismografia sem oclusão e espirometria animada. Embora as provas funcionais do lactente permaneçam no domínio da investigação, pela sua controversa aplicabilidade individual, as provas de função respiratória em idade pré-escolar são muito promissoras para o futuro

    Viral Respiratory Infections in Children

    Introdução: Os vírus respiratórios são uma importante causa de morbilidade e constituem a principal causa de dificuldade respiratória na infância. Os agentes mais frequentes são o vírus sincicial respiratório (VSR) e rínovirus humano (RV). Outras agentes menos comuns incluem os vírus influenza, parainfluenza, adenovírus e os mais recentemente identificados coronavírus, metapneumovírus humano e bocavírus humano. O objectivo foi descrever as infecções por vírus respiratórios numa amostra de crianças internadas. Métodos: Foi feita uma revisão dos pedidos de pesquisa de vírus respiratório em crianças abaixo dos 5 anos, internadas por infecção respiratória entre 1 de Outubro de 2010 e 15 de Fevereiro de 2012 e dos respectivos processos clínicos. Resultados: Foi realizada pesquisa de vírus respiratórios por imunoflorescência directa em 664 crianças, com resultados positivos em 268 (40.4%): VSR (n=240, 89.6%), metapneumovírus (n=10), influenza A (n=7), parainfluenza (n=6), adenovírus (n=2) e 3 casos de co-infecção. O maior número de casos positivos ocorreu entre Dezembro 2010 e Janeiro 2011 (n=263, 39.6% do total de casos positivos) e Dezembro 2011 e Janeiro 2012 (n=183, 27.6% do total de casos positivos). A maioria das crianças apresentava infecções adquiridas na comunidade (n=605, 91.1%), com dificuldade respiratória em 422 casos (69.8%). Os restantes casos correspondiam a infecções nosocomiais (n=59, 8.9%). A infecção por VSR foi mais frequente em crianças abaixo dos seis meses (65.2%, p<0.0001) e associou-se de forma estatisticamente significativa a dificuldade respiratória (96.3%, p<0.0001), hipoxémia e corticoterapia sistémica (35.6%, p=0.0001). A maioria das crianças com sibilância recorrente apresentava dificuldade respiratória (91.9%, p<0.0001). Discussão: Nas infecções respiratórias na infância com necessidade de internamento destaca-se a preponderância de infecções por VSR, com padrão sazonal típico (com pico de incidência nos meses de Inverno) e o maior risco de internamento em infecções por VSR abaixo dos 6 meses de idade

    Ketamine induction of p53-dependent apoptosis and oxidative stress in zebrafish (Danio rerio) embryos

    Ketamine is a widely used pharmaceutical that has been detected in water sources worldwide. Zebrafish embryos were used in this study to investigate the oxidative stress and apoptotic signals following a 24h exposure to different ketamine concentrations (0, 50, 70 and 90 mg L-1). Early blastula embryos (∼2 h post fertilisation-hpf) were exposed for 24 h and analysed at 8 and 26 hpf. Reactive oxygen species and apoptotic cells were identified in vivo, at 26 hpf. Enzymatic activities (superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GPx), lactate dehydrogenase (LDH) and acetylcholinesterase (AChE)), glutathione levels (oxidised (GSSG) and reduced (GSH)), oxidative damage (lipid peroxidation (LPO) and protein carbonyls (CO)) as well as oxidative stress (gclc, gstp1, sod1 and cat), apoptosis (casp3a, casp6, casp8, casp9, aifm1 and tp53) and cell proliferation (pcna) related-genes were evaluated at 8 and 26 hpf. Caspase (3 and 9) activity was also determined at both time-points by colorimetric methods. Superoxide dismutase (SOD), catalase (CAT), glutathione levels (GSSG), caspase-9 and reactive oxygen species (ROS) were shown to be affected by ketamine exposure while in vivo analysis showed no difference in ROS. A significant up-regulation of superoxide dismutase (sod1) and catalase (cat) genes expression was also perceived. Ketamine-induced apoptosis was observed in vivo and confirmed by the apoptotic-related genes up-regulation.     Validation of the Multidimensional Sense of Humor Scale in People with Chronic Kidney Disease

    Multidimensional Sense of Humor Scale (MSHS) was developed by Thorson and Powell and it was validated in Portuguese, but not in people with chronic kidney disease (CKD). This study examined the psychometrics of the MSHS in people with CKD on hemodialysis. A random sample of 171 people with CKD undergoing hemodialysis was selected. Exploratory Factor Analysis revealed a structure with three factors, “Humor Production and Social Use of Humor”, “Adaptive Humor and Appreciation Humor” and “Attitude Towards Humor”, with Alpha Cronbach values of 0.93, 0.90 and 0.83 respectively. It revealed stability in both interview and questionnaire methods. It showed moderate positive correlations with Positive Affect, Subjective Happiness and Wellbeing Personal Index, and moderate negative correlation with Negative Affect.     Euthanasia using gaseous agents in laboratory rodents

    Several questions have been raised in recent years about the euthanasia of laboratory rodents. Euthanasia using inhaled agents is considered to be a suitable aesthetic method for use with a large number of animals simultaneously. Nevertheless, its aversive potential has been criticized in terms of animal welfare. The data available regarding the use of carbon dioxide (CO2), inhaled anaesthetics (such as isoflurane, sevoflurane, halothane and enflurane), as well as carbon monoxide and inert gases are discussed throughout this review. Euthanasia of fetuses and neonates is also addressed. A table listing currently available information to ease access to data regarding euthanasia techniques using gaseous agents in laboratory rodents was compiled. Regarding better animal welfare, there is currently insufficient evidence to advocate banning or replacing CO2 in the euthanasia of rodents; however, there are hints that alternative gases are more humane. The exposure to a volatile anaesthetic gas before loss of consciousness has been proposed by some scientific studies to minimize distress; however, the impact of such a measure is not clear. Areas of inconsistency within the euthanasia literature have been highlighted recently and stem from insufficient knowledge, especially regarding the advantages of the administration of isoflurane or sevoflurane over CO2, or other methods, before loss of consciousness. Alternative methods to minimize distress may include the development of techniques aimed at inducing death in the home cage of animals.     Anaesthesia and analgesia in laboratory adult zebrafish: a question of refinement

    Anaesthesia is used daily in fish experimental procedures; however, the use of an inadequate anaesthetic protocol can compromise not only the animal's welfare but also the reliability of results. The use of zebrafish (Danio rerio) in biomedical research has increased in the last decades, highlighting the importance of appropriate anaesthetic regimens for this species. This article reviews the main anaesthetic agents and protocols used in laboratory adult zebrafish, and some of the analgesic methods to be used in this species that still need more research.     Analyticity and criticality results for the eigenvalues of the biharmonic operator

    We consider the eigenvalues of the biharmonic operator subject to several homogeneous boundary conditions (Dirichlet, Neumann, Navier, Steklov). We show that simple eigenvalues and elementary symmetric functions of multiple eigenvalues are real analytic, and provide Hadamard-type formulas for the corresponding shape derivatives.
