    The role of cost-effectiveness analysis in the development of indicators to support incentive-based behaviour in primary care in England

    In England, general practitioners are incentivized through a national pay-for-performance scheme to adopt evidence-based quality improvement initiatives using a portfolio of Quality and Outcomes Framework (QOF) indicators. We describe the development of the methods used to assess the cost-effectiveness of these pay-for-performance indicators and how they have contributed to the development of new indicators. Prior to analysis of new potential indicators, an economic subgroup of the National Institute for Health and Care Excellence (NICE) Indicator Advisory Committee is formed to assess evidence on the cost-effectiveness of potential indicators in terms of the health benefits gained, compared to the cost of the intervention and the cost of the incentive. The expert subgroup is convened to reach consensus on the amounts that could potentially be paid to general practices for achieving new indicators. Indicators are also piloted in selected general practices and evidence gathered about their practical implementation. The methods used to assess economic viability of new pilot indicators represent a pragmatic and effective way of providing information to inform recommendations. Current policy to reduce QOF funding could shift the focus from national (QOF) to local schemes, with economic appraisal remaining central

    This policy brief, "Towards a National Housing Strategy for Homeless Youth", is part of a series developed by the Hollywood Homeless Youth Partnership (HHYP) to advance policy and practice recommendations focused on preventing and ending youth homelessness. This brief emerges from "No Way Home: Understanding the Needs and Experiences of Homeless Youth in Hollywood", a report released by the HHYP in November 2010 presenting findings from a multi-method needs assessment conducted with 389 homeless youth ages 12 to 25 in the Hollywood community.The purpose of this brief is to address the inadequacies of prioritizing permanent housing as the only solution for homeless youth, identify the major limitations of our existing housing programs, and advocate for developing a national housing strategy and funding a full housing continuum for homeless young people that is responsive to their unique needs and circumstances. This brief is being released at a time of unprecedented interest in the issue of youth homelessness -- we hope it will inform federal and local planning and decision-making and help advance our national agenda of preventing and ending youth homelessness

    Identifying Multiply-Scattered Wavepaths in Strongly Scattering and DispersiveMedia

    The ability to extract information from scattered waves is usually limited to singly scattered energy even if multiple scattering might occur in the medium. As a result, the information in arrival times of higher-order scattered events is underexplored. This information is extracted using fingerprinting theory. This theory has never previously been applied successfully to real measurements, particularly when the medium is dispersive. The theory is used to estimate the arrival times and scattering paths of multiply scattered waves in a thin sheet using an automated scheme in a dispersive medium by applying an additional dispersion compensation method. Estimated times and paths are compared with predictions based on a sequence of straight ray paths for each scattering event given the known scatterer locations. Additionally, numerical modelling is performed to verify the interpretations of the compensated data. Since the source also acts as a scatterer in these experiments, initially, the predictions and the numerical results did not conform to the experimental observations. By reformulating the theory and the processing scheme and adding a source scatterer in the modelling, it is shown that predictions of all observed scattering events are possible with both prediction methods, verifying that the methods are both effective and practically achievable.     Monsoon drought over Java, Indonesia, during the past two centuries

    Monsoon droughts, which often coincide with El Nino warm events, can have profound impacts on the populations of Southeast Asia. Improved understanding and prediction of such events can be aided by high-resolution proxy climate records, but these are scarce for the tropics. Here we reconstruct the boreal autumn (October-November) Palmer Drought Severity Index (PDSI) for Java, Indonesia (1787-1988). This reconstruction is based on nine ring-width chronologies derived from living teak trees growing on the islands of Java and Sulawesi, and one coral delta O-18 series from Lombok. The PDSI reconstruction correlates significantly with El Nino-Southern Oscillation (ENSO)-related sea surface temperatures and other historical and instrumental records of tropical climate, reflecting the strong coupling between the climate of Indonesia and the large scale tropical Indo-Pacific climate system.</p

    Space Perception in Virtual Environments: Displacement from the Center of Projection Causes Less Distortion than Predicted by Cue-Based Models

    Virtual reality systems commonly include both monocular and binocular depth cues, which have the potential to provide viewers with a realistic impression of spatial properties of the virtual environment. However, when multiple viewers share the same display, only one viewer typically receives the projectively correct images. All other viewers experience the same images despite displacement from the center of projection (CoP). Three experiments evaluated perceptual distortions caused by displacement from the CoP and compared those percepts to predictions of models based on monocular and binocular viewing geometry. Leftward and rightward displacement from the CoP caused virtual angles on the ground plane to be judged as larger and smaller, respectively, compared to judgments from the CoP. Backward and forward displacement caused rectangles on the ground plane to be judged as larger and smaller in depth, respectively, compared to judgments from the CoP. Judgment biases were in the same direction as cue-based model predictions but of smaller magnitude. Displacement from the CoP had asymmetric effects on perceptual judgments, unlike model predictions. Perceptual distortion occurred with monocular cues alone but was exaggerated when binocular cues were added. The results are grounded in terms of practical implications for multiuser virtual environments

    A Warp in Progress : H I and Radio Continuum Observations of the Spiral NGC 3145

    Date of Acceptance: 16/06/2015We present VLA H I and 6 cm radio continuum observations of the spiral NGC 3145 and H I observations of its two companions, NGC 3143 and PGC 029578. In optical images NGC 3145 has stellar arms that appear to cross, forming "X"-features. Our radio continuum observations rule out shock fronts at 3 of the 4 "X"-features. In the middle-to-outer disk, the H I line-profiles of NGC 3145 are skewed. Relative to the disk, the gas in the skewed wing of the line-profiles has z-motions away from us on the approaching side of the galaxy and z-motions of about the same magnitude (about 40 km/s) towards us on the receding side. These warping motions imply that there has been a perturbation with a sizeable component perpendicular to the disk over large spatial scales. Two features in NGC 3145 have velocities indicating that they are out-of-plane tidal arms. One is an apparent branch of a main spiral arm; the velocity of the branch is 150 km/s greater than the spiral arm where they appear to intersect in projection. The other is an arm that forms 3 of the "X"-features. It differs in velocity by 56 km/s from the disk at the same projected location. Based on its SFR and H I properties, NGC 3143 is the more likely of the two companions to have interacted with NGC 3145 recently. A simple analytic model demonstrates that an encounter between NGC 3143 and NGC 3145 is a plausible explanation for the observed warping motions in NGC 3145.Peer reviewe

    Adverse prognostic and predictive significance of low DNA-dependent protein kinase catalytic subunit (DNA-PKcs) expression in early-stage breast cancers

    Background: DNA-dependent protein kinase catalytic subunit (DNA-PKcs), a serine threonine kinase belonging to the PIKK family (phosphoinositide 3-kinase-like-family of protein kinase), is a critical component of the non-homologous end joining (NHEJ) pathway required for the repair of DNA double strand breaks. DNA-PKcs may be involved in breast cancer pathogenesis. Methods: We evaluated clinicopathological significance of DNA-PKcs protein expression in 1161 tumours and DNA-PKcs mRNA expression in 1950 tumours. We correlated DNA-PKcs to other markers of aggressive phenotypes, DNA repair, apoptosis and cell cycle regulation. Results: Low DNA-PKcs protein expression was associated with higher tumour grade, higher mitotic index, tumour de-differentiation and tumour type (ps<0.05). Absence of BRCA1, low XRCC1/SMUG1/APE1/Polβ were also more likely in low DNA-PKcs expressing tumours (ps<0.05). Low DNA-PKcs protein expression was significantly associated with worse breast cancer specific survival (BCCS) in univariate and multivariate analysis (ps<0.01). At the mRNA level, low DNA-PKcs was associated with PAM50.Her2 and PAM50.LumA molecular phenotypes (ps<0.01) and poor BCSS. In patients with ER positive tumours who received endocrine therapy, low DNA-PKcs (protein and mRNA) was associated with poor survival. In ER negative patients, low DNA-PKcs mRNA remains significantly associated with adverse outcome. Conclusions: Our study suggests that low DNA-PKcs expression may have prognostic and predictive significance in breast cancers

    The reconstructed Indonesian warm pool sea surface temperatures from tree rings and corals: Linkages to Asian monsoon drought and El Niño–Southern Oscillation

    [ 1] The west Pacific warm pool is the heat engine for the globe's climate system. Its vast moisture and heat exchange profoundly impact conditions in the tropics and higher latitudes. Here, September - November sea surface temperature (SST) variability is reconstructed for the warm pool region (15 degrees S - 5 degrees N, 110 - 160 degrees E) surrounding Indonesia using annually resolved teak ring width and coral delta O-18 records. The reconstruction dates from A. D. 1782 - 1992 and accounts for 52% of the SST variance over the most replicated period. Significant correlations are found with El Nino - Southern Oscillation (ENSO) and monsoon indices at interannual to decadal frequency bands. Negative reconstructed SST anomalies coincide with major volcanic eruptions, while other noteworthy extremes are at times synchronous with Indian and Indonesian monsoon drought, particularly during major warm ENSO episodes. While the reconstruction adds to the sparse network of proxy reconstructions available for the tropical Indo-Pacific, additional proxies are needed to clarify how warm pool dynamics have interacted with global climate in past centuries to millennia.</p
