    Estimating the Yin-Yang Nature of Western Herbs: A Potential Tool Based on Antioxidation- Oxidation Theory

    Background : One of the biggest obstacles to progress in traditional Chinese medicine (TCM) development in Western countries is the difficulty of applying the traditional concepts to the Western medicinal plants, which are not traditionally described in ancient literature. During recent years, new advances in the field of understanding Yin/Yang aspects from a modern bioscientific point of view have led to the conclusion that antioxidationoxidation concepts might mirror a Yin-Yang relationship.Methods: This study was intended to integrate the Yin-Yang theory of the traditional Chinese medicine with modern antioxidation-oxidation theory, and to propose a biochemical tool based on redox parameters (e.g. antioxidant capacity, chemiluminescence-CL signal inducing capacity), usable for the classification of Western medicinal plants from Yin/Yang perspective. Trolox equivalent antioxidant capacity (TEAC) of six vegetal aqueous extracts (Symphitum officinalae (radix)- SYM, Inula helenium (radix)- INU, Calendula officinalis (flores)- CAL, Angelica arhanghelica (folium)- ANG(F), Angelica arhanghelica (radix)- ANG(R), Ecbalium Elaterium (fruits)- ECB) and luminol-enhanced chemiluminescence of PMNL on addition of these vegetal extracts were measured. Percentages from the maximal or minimal values obtained were calculated for each extract (TEAC%, PMNL stimulation%, PMNL inhibition%, relative speed of action% (RSA%%)), specific Yin-Yang significance was assigned to each relative parameter. In the end, an integration of all the relative values was done, in order to find a global ”Yin” or a ”Yang” trait of each vegetal extract.Results: TEAC decreased in the following order: SYM > INU > CAL >ANG(F) > ANG(R > ECB. Three vegetal extracts (SYM > INU > ECB) decreased the luminol-enhanced chemiluminescence of PMNL, two (ANG(R) > ANG(F)) increased it, while one (CAL) had a dual effect. After the integration of the percentages, CAL was found to have a global ”Yang” trait, while the rest of the plants had a global ”Yin” trait.Conclusions: TEAC% and PMNL inhibition% appears to correlate with the Yin properties of herbs, while PMNL stimulation% and RSA% might correlate with Yang aspects within the formal TCM classification system, and may be useful criteria in describing the Western herbs from a TCM point of view.Key words: antioxidant, TEAC, herbs, chemiluminescence, Yin, Yang, TC

    Coffee yield variations and their relations to rainfall events in Nicaragua

    In order to predict the impacts of foreseen climate change on coffee production, one first step would be to check the impacts of past climate variations on coffee yields. We developed a survey in coffee zones in Nicaragua to compile the historical registers made by farmers on yields, rainfalls and temperature daily, management and blossoming date, and analyze their relationship to specific climate events. The farmers' perceptions on climate risks and actual damages were also investigated. A simple model was then developed, that links coffee phenology, rainfall effects on flowering and soil water balance. Coffee yield data were obtained from 23 farms, over a span ranging from 6 to 78 years. The Pacific Zone, and the most ancient coffee zone, presented the longest series of yields and rainfalls. Coffee yields are much more variable in this region than in the North Zone (variation coefficient 33% vs. 18%, resp.).Farmers' perceptions agreed with this finding, with much higher risks perceived in the Pacific Zone. Drought and rainfall excess alike were identified as causing the highest risks, temperature variations were not reported, possibly because they are much less easy to perceive than rainfall variations. The blossoming period was perceived, in both regions, as the most sensitive period, to drought as well as to rainfall excess. Drought events are perceived as more frequent. Very long series on blossoming dates and intensities allowed us to build and calibrate a model, based on rainfall and temperature, to estimate the rainfall during blossoming and the resulting yield loss. Rainfall over 40 mm during the blossoming could reduce the yield from 60%. Alternative practices are discussed that could mitigate the risks identified in the risk-prone Pacific zone.     Radiofrequency catheter ablation in a child with Wolff-Parkinson-White syndrome and congenitally corrected transposition of the great arteries

    The case is presented of a five-year-old boy with Wolff-Parkinson-White syndrome, who had undergone three surgical palliations because of congenitally corrected transposition of the great arteries, ventricular septal defect and pulmonary trunk near-atresia. Successful and uncomplicated radiofrequency catheter ablation of an accessory pathway located across the systemic atrioventricular valve ring was performed. The procedure was motivated by forthcoming corrective surgery, which would preclude venous access to the heart. This case emphasizes the need for precise timing of ablative therapy in such patients and shows that surgery itself may facilitate catheter ablation. (Cardiol J 2007; 14: 500-503

    Unmodifiable variables related to thyroid cancer incidence

    The incidence of thyroid cancer is significantly different between male and female patients. Thyroid cancer is also the only form of cancer where age can be considered a staging variable. Identifying biological prognostic factors such as age or sex is important as it helps select an optimal personalized therapy. The present analysis is an observational, prospective study that enrolled all patients with thyroid disease who were operated upon at a single center. The study aimed to determine the most frequent age at presentation, the predominance of one sex over the other, the incidence of malignant thyroid disease, and the relative risk for each sex to develop thyroid carcinoma. The incidence of thyroid carcinoma was higher for women than for men, with a higher relative risk in the female subgroup. Incidence was also highest in the 50-60-year-old group. Given that studies show better survival for women and for younger patients, even when presenting with advanced disease, compared with older, male patients, such prognostic indicators should be a factor in the treatment decision

    Ultra-short pulse HiPIMS: a strategy to suppress arcing during reactive deposition of SiO2 thin films with enhanced mechanical and optical properties

    In this contribution, based on the detailed understanding of the processes’ characteristics during reactive high-power impulse magnetron sputtering (HiPIMS), we demonstrated the deposition of silicon oxide (SiO2) thin films with improved optical and mechanical performances. A strategy for stabilizing the arc-free HiPIMS of Si target in the presence of oxygen was investigated. Arcing was suppressed by suitable pulse configurations, ensuring good process stability without using any feedback control system. It was found that arcing can be significantly alleviated when ultra-short HiPIMS pulses are applied on the target. The optical and mechanical properties of SiO2 coatings deposited at various pulsing configurations were analyzed. The coatings prepared by ultra-short pulse HiPIMS exhibited better optical and mechanical performance compared to the coatings prepared by long pulse HiPIMS. The optimized SiO2 coatings on quartz substrates exhibited an average transmittance of 98.5% in the 190–1100-nm wavelength range, hardness of 9.27 GPa, hardness/Young’s modulus ratio of 0.138, and critical adhesion load of 14.8 N. The optical and mechanical properties are correlated with the film morphology, which is inherently related to energetic conditions and process stability during film growth.D.     New oral anticoagulants and their reversal agents

    Atrial fibrillation is a commonly encountered pathology in medical practice, and its prevalence has shown a continuous rise over the past years. Atrial fibrillation has a significant impact on patients\u27 quality of life, not only due to the standard anticoagulant treatment with vitamin K antagonists that require close monitoring and dose adjustment, but also due to the fragile equilibrium between hemorrhagic and thrombotic risks. The introduction of new oral anticoagulants (NOACs) in the treatment guidelines for atrial fibrillation has improved the quality of life, as NOACs do not require close monitoring or dose adjustments. However, even if the safety profile of the NOACs regarding the hemorrhagic risk is superior to vitamin K antagonists, the problem raised by an unexpected hemorrhage (e.g. severe hemorrhage after an accident) and the need for efficient hemostasis in a chronic anticoagulated patient has remained unsolved. To find a solution for this problem, reversal agents for NOACs have been developed and tested, and two of them, idarucizumab and andexanet-alpha, have already been approved by the FDA, thus making NOACs increasingly appealing as a choice of anticoagulation treatment

    Does sex of the patient play a role in survival for MSI colorectal cancer?

    Microsatellite instability (MSI) is a feature of colorectal tumors that develops as a result of inactivation of the DNA mismatch repair system. It is found in about 15% of all colorectal cancers and is an important prognostic molecular marker when assessing patients with colorectal cancer. It can influence prognosis and treatment decisions in both the advanced and early stages. Although in early stages this marker suggests a favorable prognosis and presents an important argument against adjuvant treatment in stage II disease, in metastatic stages it no longer associated with such an optimistic outcome. The present trial is a prospective, single-center study which included 122 colorectal cancer patients who were tested for MSI using immunohistochemistry. The trial included patients with stage II to IV colorectal cancer, treated in the Prof. Dr. Agrippa Ionescu Emergency Hospital, Bucharest, Romania. Follow-up data were collected during a 24-month period. The study attempted to determine whether differences exist in overall survival for MSI (microsatellite instability) vs. MSS (microsatellite stable) colorectal cancer and to ascertain whether sex of the patient influences prognosis in MSI patients, irrespective of stage or treatment. Results demonstrated no significant differences in survival for MSI vs MSS colorectal patients, and patients’ gender proved not to influence the outcome in MSI patients
