    Spatiotemporal Dynamics of Reward and Punishment Effects Induced By Associative Learning

    While reward associative learning has been studied extensively across different species, punishment avoidance learning has received far less attention. Of particular interest is how the two types of learning change perceptual processing of the learned stimuli. We designed a task that required participants to learn the association of emotionally neutral images with reward, punishment, and no incentive value outcomes through trial-and-error. During learning, participants received monetary reward, neutral outcomes or avoided punishment by correctly identifying corresponding images. Results showed an early bias in favor of learning reward associations, in the form of higher accuracy and fewer trials needed to reach learning criterion. We subsequently assessed electrophysiological learning effects with a task in which participants viewed the stimuli with no feedback or reinforcement. Critically, we found modulation of two early event-related potential components for reward images: the frontocentral P2 (170–230 ms) and the anterior N2/Early Anterior Positivity (N2/EAP; 210–310 ms). We suggest that reward associations may change stimuli detection and incentive salience as indexed by P2 and N2/EAP.     Biological vulnerability to depression: Linked structural and functional brain network findings

    Background: Patients in recovery following episodes of major depressive disorder (MDD) remain highly vulnerable to future recurrence. Although psychological determinants of this risk are well established, little is known about associated biological mechanisms. Recent work has implicated the default mode network (DMN) in this vulnerability but specific hypotheses remain untested within the high risk, recovered state of MDD. Aims: To test the hypothesis that there is excessive DMN functional connectivity during task performance within recovered-state MDD and to test for connected DMN cortical gyrification abnormalities. Method: A multimodal structural and functional magnetic resonance imaging (fMRI) study, including task-based functional connectivity and cortical folding analysis, comparing 20 recoveredstate patients with MDD with 20 matched healthy controls. Results: The MDD group showed significant task-based DMN hyperconnectivity, associated with hypogyrification of key DMN regions (bilateral precuneus). Conclusions: This is the first evidence of connected structural and functional DMN abnormalities in recovered-state MDD, supporting recent hypotheses on biological-level vulnerability

    Covid-19 vaccines in children with cow’s milk and food allergies

    The COVID-19 pandemic is the most challenging global health crisis of our times. Vaccination against COVID-19 plays a key role to control the current pandemic situation. The risk of allergic reactions to new COVID-19 vaccines is low. However, there is a debate on the safety in allergic patients following post marketing findings by different agencies. Our aim is to understand from current experiences whether children with cow’s milk or food allergy are at higher risk than a general population for allergic reactions to COVID-19 vaccines. Current data indicate that patients with a history of allergy to cow’s milk or other foods, even if severe, should receive COVID-19 vaccine in a setting with availability of treatments for anaphylactic reactions and under medical supervision. Recipients should be discharged after a protracted observation period of 30 min if no reaction developed

    Effectiveness of expressive writing protocol in palliative care healthworkers: A quantitative study

    Background and aim of the work: Palliative Care professionals are exposed to intense emotional envi-ronment. This puts them at risk for Compassion Fatigue and Burnout. The protective factors that can counter their onset are Compassion Satisfaction, Organizational Commitment and Resilience. Expressive Writing is a valid tool for adapting to traumatic events and enhancing psychological well-being. Aim of this study is to evaluate the effect of the Expressive Writing in Palliative Care professionals on Compassion Satisfaction, Organizational Commitment, Resilience, Compassion Fatigue and perceived distress. Methods: Prospective experimental study with experimental/control groups and pre/post measurements. 50 Palliative Care professionals were recruited in Northern and Central Italy. Participants filled: Organizational Commitment Questionnaire; ProQol-revision III; Resilience Scale for Adults; Impact of Event-Scale Revised; Emotion Thermometer; ad hoc questionnaire for the evaluation of protocol usefulness. Results: Wilcoxon test demon-strated change in Continuative Commitment (Z =-3.357, p = .001), anger (Z =-2.214, p = .027), sleep (Z =-2.268, p = .023), help (Z =-2.184, p = .029), intrusiveness (Z =-2.469, p = .014), hyperarousal (Z =-2.717, p = .007), and total IES (Z =-2.456, p =, 014). Mann Whitney test showed a significantly lower score on post-test Intrusiveness in the experimental group (U = 202, p = .038). Conclusions: The Expressive Writing intervention was effective in improving organizational and emotional variables. Expressive Writing supports healthcare professionals in relieving the burden of traumatic episodes, ordering associated thoughts and emo-tions, and implementing a process of deep comprehension

    Prostate health index and multiparametric mri: Partners in crime fighting overdiagnosis and overtreatment in prostate cancer

    Widespread use of PSA as the standard tool for prostate cancer (PCa) diagnosis led to a high rate of overdiagnosis and overtreatment. In this study, we evaluated the performance of the prostate health index (PHI) and multiparametric magnetic resonance imaging (mpMRI) for the prediction of positive biopsy and of high‐grade PCa at radical prostatectomy (RP). To this end, we prospectively enrolled 196 biopsy‐naïve patients who underwent mpMRI. A subgroup of 116 subjects with biopsy‐proven PCa underwent surgery. We found that PHI significantly outperformed both PI‐RADS score (difference in AUC: 0.14; p < 0.001) and PHI density (difference in AUC: 0.08; p = 0.002) in the ability to predict positive biopsy with a cut‐off value of 42.7 as the best threshold. Conversely, comparing the performance in the identification of clinically significant prostate cancer (csPCa) at RP, we found that PHI ≥61.68 and PI‐RADS score ≥4 were able to identify csPCa (Gleason score ≥7 (3 + 4)) both alone and added to a base model including age, PSA, fPSA‐to-tPSA ratio and prostate volume. In conclusion, PHI had a better ability than PI‐RADS score to predict positive biopsy, whereas it had a comparable performance in the identification of pathological csPCa

    We propose the gravity’s rainbow scenario as a possible alternative of the inflation paradigm to account for the flatness and horizon problems. We focus on studying the cosmological scalar perturbations which are seeded by the quantum fluctuations in the very early universe. The scalar power spectrum is expected to be nearly scale-invariant. We estimate the rainbow index λ\lambda and energy scale M in the gravity’s rainbow scenario by analyzing the Planck temperature and WMAP polarization datasets. The constraints on them are given by λ=2.933±0.012\lambda =2.933\pm 0.012 and ln(105M/Mp)=0.4010.451+0.457\ln (10^5M/M_p)= -0.401^{+0.457}_{-0.451} at the 68%68\,\% confidence level
