    "Are you accepting new patients?" A pilot field experiment on telephone-based gatekeeping and Black patients' access to pediatric care.

    STUDY OBJECTIVES: To determine whether name and accent cues that the caller is Black shape physician offices' responses to telephone-based requests for well-child visits. METHOD AND DATA: In this pilot study, we employed a quasi-experimental audit design and examined a stratified national sample of pediatric and family practice offices. Our final data include information from 205 audits (410 completed phone calls). Qualitative data were blind-coded into binary variables. Our case-control comparisons using McNemar's tests focused on acceptance of patients, withholding information, shaping conversations, and misattributions. FINDINGS: Compared to the control group, "Black" auditors were less likely to be told an office was accepting new patients and were more likely to experience both withholding behaviors and misattributions about public insurance. The strength of associations varied according to whether the cue was based on name or accent. Additionally, the likelihood and ways office personnel communicated that they were not accepting patients varied by region. CONCLUSIONS: Linguistic profiling over the telephone is an aspect of structural racism that should be further studied and perhaps integrated into efforts to promote equitable access to care. Future research should look reactions to both name and accent, taking practice characteristics and regional differences into consideration

    poster abstractThis poster will describe the different ways in which the Survey Research Center at IUPUI (SRC) can help serve IUPUI faculty, staff and students as well as organizations not affiliated with the campus with research design, data collection, data management and data analyses. The poster will use recent surveys for which SRC has been a leader in designing, collecting and analyzing data to show the possibilities for collaboration to show the possibilities of collaboration within and outside the IUPUI community. SRC, a research center within the Institute for Research on Social Issues (IRSI) within the School of Liberal Arts, is an interdisciplinary survey research center that provides services to a wide variety of private, nonprofit, and governmental organizations. The center also serves the entire IUPUI campus by conducting research for faculty members, students, and various university departments. The SRC is equipped with call stations (for telephone interviewing) and is equipped to conduct face-toface, web and mail surveys depending on research design needs

    Our experiments on viscous (Saffman-Taylor) fingering in Hele-Shaw channels reveal several phenomena that were not observed in previous experiments. At low flow rates, growing fingers undergo width fluctuations that intermittently narrow the finger as they evolve. The magnitude of these fluctuations is proportional to Ca^{-0.64}, where Ca is the capillary number, which is proportional to the finger velocity. This relation holds for all aspect ratios studied up to the onset of tip instabilities. At higher flow rates, finger pinch-off and reconnection events are observed. These events appear to be caused by an interaction between the actively growing finger and suppressed fingers at the back of the channel.     Chain‐Growth Polymerization of Aryl Grignards Initiated by a Stabilized NHC‐Pd Precatalyst

    An N‐heterocyclic carbene‐ligated palladium catalyst was discovered to mediate living, chain‐growth polymerizations of both phenylene‐ and thiophene‐based monomers. Polymerization of a fluorene‐based monomer, on the other hand, did not proceed through a living, chain‐growth pathway. Excitingly, block copolymerizations of phenylene and thiophene proceeded via a chain‐growth pathway, regardless of the order of monomer addition. Although some chain termination was observed during these copolymerizations, this pathway could be minimized when the second monomer was added shortly after consumption of the first monomer. These results suggest that the catalyst resting‐state at the end of polymerization is unstable. As a result, modifications to the NHC‐scaffold or the 3‐chloropyridine ligand will be necessary to generate an improved catalyst. A Pd catalyst that can mediate a living, chain‐growth polymerization of π‐conjugated monomers is reported. Using (IPr)Pd(3‐chloropyridine)Cl 2 as a precatalyst, both homopolymers and block copolymers of phenylene‐ and thiophene‐based monomers were prepared.     Calipso Observations of the Dependence of Homo- and Heterogeneous Ice Nucleation in Cirrus Clouds on Latitude, Season and Surface Condition

    There are two fundamental mechanisms through which cirrus clouds form; homo- and heterogeneous ice nucleation (henceforth hom and het). The relative contribution of each mechanism to ice crystal production often determines the microphysical and radiative properties of a cirrus cloud. A new satellite remote sensing method is described in this study to estimate cirrus cloud ice particle number concentration and the relative contribution of hom and het to cirrus cloud formation as a function of altitude, latitude, season and surface type (e.g. land vs. ocean). This method uses co-located observations from the Infrared Imaging Radiometer (IIR) and from the CALIOP (Cloud and Aerosol Lidar with Orthogonal Polarization) lidar aboard the CALIPSO (Cloud-Aerosol Lidar and Infrared Pathfinder Satellite Observation) polar orbiting satellite, employing IIR channels at 10.6 m and 12.05 m. The method is applied here to single-layered clouds of visible optical depth between about 0.3 and 3. Two years of Version 3 data have been analyzed for the years 2008 and 2013, with each season characterized in terms of 532 nm cirrus cloud centroid altitude and temperature, the cirrus cloud ice particle number concentration, effective diameter, layer-average ice water content and visible optical depth. Using a conservative criterion for hom cirrus, on average, the sampled cirrus clouds formed through hom occur about 43% of the time in the Arctic and 50% of the time in the Antarctic, and during winter at mid-latitudes in the Northern Hemisphere, hom cirrus occur 37% of the time. Elsewhere (and during other seasons in the Northern Hemisphere mid-latitudes), this hom cirrus fraction is lower. Processes that could potentially explain these observations are discussed, as well as the potential relevancy of these results to ice nucleation studies, climate modeling and jet-stream dynamics

    This article reports the findings of a study modeled after Saracevic, Shaw, and Kantor\u27s efforts to identify and quantify the causes of users\u27 failures to identify and locate library materials. The researchers analyzed patron-reported and librarian-observed subject and known-item searches and found an overall success rate of only 54 percent. The problems that led to the 46 percent failure rate were analyzed by source and type of failure, and subjective observations concerning problems encountered by patrons were recorded. Recommendations are made for reducing library malfunctions and circulation, patron, and acquisition errors

    Does diabetes mellitus influence pathologic complete response and tumor downstaging after neoadjuvant chemoradiation for esophageal and gastroesophageal cancer? A two-institution report.

    BACKGROUND: Esophageal carcinoma is an aggressive disease that is often treated with neoadjuvant therapy followed by surgical resection. Diabetes mellitus (DM) has been associated with reduced efficacy of chemoradiation (CRT) in other gastrointestinal cancers. The goal of this study was to determine if DM affects response to neoadjuvant CRT in the management of gastroesophageal carcinoma. METHODS: We retrospectively reviewed the esophageal cancer patient databases and subsequently analyzed those patients who received neoadjuvant CRT followed by surgical resection at two institutions, Thomas Jefferson University (TJUH) and Fox Chase Cancer Center (FCCC). Comparative analyses of rates of pathologic complete response rate (pCR) and pathologic downstaging in DM patients versus non-DM patients was performed. RESULTS: Two hundred sixty patients were included in the study; 36 patients had DM and 224 were non-diabetics. The average age of the patients was 61 years (range 24-84 years). The overall pCR was 26%. The pCR rate was 19% and 27% for patients with DM and without DM, respectively (P = 0.31). Pathologic downstaging occurred in 39% of study patients, including of 33% of DM patients and 40% of non-DM patients (P = 0.42). CONCLUSIONS: Although the current analysis does not demonstrate a significant reduction in pCR rates or pathologic downstaging in patients with DM, the observed trend suggests that a potential difference may be observed with a larger patient population. Further studies are warranted to evaluate the influence of DM on the effectiveness of neoadjuvant CRT in esophageal cancer

    Performing public credit at the eighteenth-century Bank of England

    Much is known about the negotiation of personal credit relationships during the eighteenth century. It has been noted how direct contact and observation allowed individuals to assess the creditworthiness of those with whom they had financial connections and to whom they might lend money. Much less is known about one of the most important credit relationships of the long eighteenth century: that between the state and its creditors. This article shows that investors could experience the performance of public credit at the Bank of England. By 1760 the Bank was the manager of nearly three-quarters of the state's debt and housed the main secondary market in that debt. Thus, it provided a place for public creditors, both current and potential, to attend and scrutinize the performance of the state's promises. The article demonstrates how the Bank acted to embody public credit through its architecture, internal structures, and imagery and through the very visible actions of its clerks and the technologies that they used to record ownership and transfer of the national debt. The Bank of England, by those means, allowed creditors to interrogate the financial stability and reputation of the state in the same ways that they could interrogate the integrity of a private debtor.Peer reviewedFinal Accepted Versio

    Salivary melatonin onset in youth at familial risk for bipolar disorder

    Melatonin secretion and polysomnography (PSG) were compared among a group of healthy adolescents who were at high familial risk for bipolar disorder (HR) and a second group at low familial risk (LR). Adolescent participants (n = 12) were a mean age 14 ± 2.3 years and included 8 females and 4 males. Saliva samples were collected under standardized condition light (red light) and following a 200 lux light exposure over two consecutive nights in a sleep laboratory. Red Light Melatonin onset (RLMO) was defined as saliva melatonin level exceeding the mean of the first 3 readings plus 2 standard deviations. Polysomnography was also completed during each night. HR youth, relative to LR, experienced a significantly earlier melatonin onset following 200 lux light exposure. Polysomnography revealed that LR youth, relative to HR, spent significantly more time in combined stages 3 and 4 (deep sleep) following red light exposure. Additionally, regardless of the group status (HR or LR), there was no significant difference in Red Light Melatonin Onset recorded at home or in the laboratory, implying its feasibility and reliability
