    COVID-19 Experiences, Behaviors, Beliefs, and Well-Being Among Students and Employees at a University In Rural Appalachia: A Cross-Sectional Study

    Introduction: In response to the coronavirus disease (COVID-19) pandemic, most universities experienced drastic operational changes with shifts to online learning, work-from-home policies, and social distancing measures. These changes have caused concern for social isolation and mental health. Purpose: This cross-sectional study explores differences in COVID-19 experiences, behaviors, beliefs, and well-being among students and employees (faculty and staff) at a rural Appalachian university. Methods: Data were collected with an online anonymous survey in September-October 2020 using convenience sampling. The survey measured multiple domains including COVID-19-related 1) beliefs, 2) symptoms and diagnoses, 3) exposure and preventive behavior, and 4) social, mental, and financial health. Chi-square tests and linear regression models were used to determine differences in survey responses between students and employees. Results: The final sample used for analysis included 416 respondents. The majority of respondents believed COVID-19 was a serious disease and followed mask and social distancing guidelines, although employees were more likely to adhere to mask and social distancing guidelines compared to students. Most of the respondents (\u3e50%) reported feeling more stressed, anxious, and sad since the pandemic began. Students were more impacted by the pandemic compared to employees as measured by the mental, social, and financial impact scale. A limitation of this study was that convenience sampling was used instead of a probability sampling technique, which limits the inference that can be made from the results. Implications: There may be a need for greater mental health support among university employees and students. However, future studies should confirm these findings

    Uncovering Spiral Structure in Flocculent Galaxies

    We present K'(2.1 micron) observations of four nearby flocculent spirals, which clearly show low-level spiral structure and suggest that kiloparsec-scale spiral structure is more prevalent in flocculent spirals than previously supposed. In particular, the prototypical flocculent spiral NGC 5055 is shown to have regular, two-arm spiral structure to a radius of 4 kpc in the near infrared, with an arm-interarm contrast of 1.3. The spiral structure in all four galaxies is weaker than that in grand design galaxies. Taken in unbarred galaxies with no large, nearby companions, these data are consistent with the modal theory of spiral density waves, which maintains that density waves are intrinsic to the disk. As an alternative, mechanisms for driving spiral structure with non-axisymmetric perturbers are also discussed. These observations highlight the importance of near infrared imaging for exploring the range of physical environments in which large-scale dynamical processes, such as density waves, are important.Comment: 12 pages AASTeX; 3 compressed PS figures can be retrieved from ftp://ftp.astro.umd.edu/pub/michele as file thornley.tar (1.6Mbytes).     Resource Intensity for Children and Youth: The Development of an Algorithm to Identify High Service Users in Children’s Mental Health

    Children’s mental health care plays a vital role in many social, health care, and education systems, but there is evidence that appropriate targeting strategies are needed to allocate limited mental health care resources effectively. The aim of this study was to develop and validate a methodology for identifying children who require access to more intense facility-based or community resources. Ontario data based on the interRAI Child and Youth Mental Health instruments were analysed to identify predictors of service complexity in children’s mental health. The Resource Intensity for Children and Youth (RIChY) algorithm was a good predictor of service complexity in the derivation sample. The algorithm was validated with additional data from 61 agencies. The RIChY algorithm provides a psychometrically sound decision-support tool that may be used to inform the choices related to allocation of children’s mental health resources and prioritisation of clients needing community- and facility-based resources

    The pharmacy care plan service: Service evaluation and estimate of cost-effectiveness

    Background: The UK Community Pharmacy Future group developed the Pharmacy Care Plan (PCP) service with a focus on patient activation, goal setting and therapy management.  Objective: To estimate the effectiveness and cost-effectiveness of the PCP service from a health services perspective.  Methods: Patients over 50 years of age prescribed one or more medicines including at least one for cardiovascular disease or diabetes were eligible. Medication review and person-centred consultation resulted in agreed health goals and actions towards achieving them. Clinical, process and cost-effectiveness data were collected at baseline and 12-months between February 2015 and June 2016. Mean differences are reported for clinical and process measures. Costs (NHS) and quality-adjusted life year scores were estimated and compared for 12 months pre- and post-baseline.  Results: Seven hundred patients attended the initial consultation and 54% had a complete set of data obtained. There was a significant improvement in patient activation score (mean difference 5.39; 95% CI 3.9 – 6.9; p<0.001), systolic (mean difference -2.90 mmHg ; 95% CI -4.7 - -1; p=0.002) and diastolic blood pressure (mean difference -1.81 mmHg; 95% CI -2.8 - -0.8; p<0.001), adherence (mean difference 0.26; 95% CI 0.1 – 0.4; p<0.001) and quality of life (mean difference 0.029; 95% CI 0.015 – 0.044; p<0.001). HDL cholesterol reduced significantly and QRisk2 scores increased significantly over the course of the 12 months. The mean incremental cost associated with the intervention was estimated to be £202.91 (95% CI 58.26 to £346.41) and the incremental QALY gain was 0.024 (95% CI 0.014 to 0.034), giving an incremental cost per QALY of £8,495.  Conclusions: Enrolment in the PCP service was generally associated with an improvement over 12 months in key clinical and process metrics. Results also suggest that the service would be cost-effective to the health system even when using worst case assumptions

    Viral Infection: A Potent Barrier to Transplantation Tolerance

    Transplantation of allogeneic organs has proven to be an effective therapeutic for a large variety of disease states, but the chronic immunosuppression that is required for organ allograft survival increases the risk for infection and neoplasia and has direct organ toxicity. The establishment of transplantation tolerance, which obviates the need for chronic immunosuppression, is the ultimate goal in the field of transplantation. Many experimental approaches have been developed in animal models that permit long-term allograft survival in the absence of chronic immunosuppression. These approaches function by inducing peripheral or central tolerance to the allograft. Emerging as some of the most promising approaches for the induction of tolerance are protocols based on costimulation blockade. However, as these protocols move into the clinic, there is recognition that little is known as to their safety and efficacy when confronted with environmental perturbants such as virus infection. In animal models, it has been reported that virus infection can prevent the induction of tolerance by costimulation blockade and, in at least one experimental protocol, can lead to significant morbidity and mortality.     Gross Domestic Product (GDP) and productivity of schizophrenia trials: an ecological study

    The 5000 randomised controlled trials (RCTs) in the Cochrane Schizophrenia Group's database affords an opportunity to research for variables related to the differences between nations of their output of schizophrenia trials. Ecological study – investigating the relationship between four economic/demographic variables and number of schizophrenia RCTs per country. The variable with closest correlation was used to predict the expected number of studies. GDP closely correlated with schizophrenia trial output, with 76% of the total variation about the Y explained by the regression line (r = 0.87, 95% CI 0.79 to 0.92, r2 = 0.76). Many countries have a strong tradition of schizophrenia trials, exceeding their predicted output. All nations with no identified trial output had GDPs that predicted zero trial activity. Several nations with relatively small GDPs are, nevertheless, highly productive of trials. Some wealthy countries seem either not to have produced the expected number of randomised trials or not to have disseminated them to the English-speaking world. This hypothesis-generating study could not investigate causal relationships, but suggests, that for those seeking all relevant studies, expending effort searching the scientific literature of Germany, Italy, France, Brazil and Japan may be a good investment

    Molecular hydrogen beyond the optical edge of an isolated spiral galaxy

    We know little about the outermost portions of galaxies because there is little light coming from them. We do know that in many cases atomic hydrogen (HI) extends well beyond the optical radius \cite{Casertano91}. In the centers of galaxies, however, molecular hydrogen (H2) usually dominates by a large factor, raising the question of whether H2 is abundant also in the outer regions but hitherto unseen.Here we report the detection of emission from carbon monoxide (CO), the most abundant tracer of H2, beyond the optical radius of the nearby galaxy NGC 4414. The molecular clouds probably formed in the regions of relatively high HI column density and in the absence of spiral density waves. The relative strength of the lines from the two lowest rotational levels indicates that both the temperature and density of the H2 are quite low compared to conditions closer to the center. The inferred surface density of the molecular material continues the monotonic decrease from the inner regions. We conclude that while molecular clouds can form in the outer region of this galaxy, there is little mass associated with them.Comment: 3 Nature page
