    A dinâmica dos sistemas de produção praticados na reserva extrativista do Rio Cajari no Estado do Amapá.

    Este trabalho aborda um tipo específico de extrativismo: o extrativismo da castanha-do-Brasil (Bertholletia excelsa H.B.K) realizado na Reserva Extrativista do rio Cajari no Estado do Amapá (RESEX/CA). O trabalho é uma continuidade da pesquisa participativa realizada no ano de 2000, e tem como foco principal compreender a dinâmica dos sistemas de produção praticados nesta reserva extrativista no período 2000/2005. A partir da metodologia Typologie à dire d´experts (Perrot, 1991), percebemos que as mudanças ocorridas nos sistemas de produção no período estudado, evidenciam que além de fatores de ordem econômica, a permanência das práticas agrícolas com o extrativismo da castanha se rege por outros fatores. A relação histórica de uso da floresta com o extrativismo e as agriculturas é bem mais complexa, não podendo ser avaliada apenas do prisma econômico. Os dados demonstram que as unidades de produção extrativistas são aquelas de menor desempenho econômico, e, apesar disso, não há um movimento das famílias em acabar com esse sistema. Ao contrário, o tipo extrativista não apenas se manteve, mas aumentou de representatividade de 2000 para 2005, indicando atratividade deste tipo para as famílias da RESEX/CA. Entretanto, os sistemas agroextrativistas também cresceram, indicando que algumas famílias procuram alcançar melhores níveis de qualidade de vida, através da agricultura. Em se tratando de uma unidade de conservação de uso direto, para esta reserva extrativista deveria ser direcionadas ações de apoio aos castanheiros da RESEX/CA, para que eles pudessem ter melhores condições de vida, sem ter que investir em outras atividades, que não o extrativismo

    Encephalopathy Associated with Autoimmune Thyroid Disease: A Potentially Reversible Condition

    Autoimmune thyroid disease may occasionally associate with unspecific neurological symptoms, which are more commonly insidious, include cognitive or behavioural symptoms, and may associate with tremor, myoclonus, or ataxia. We report a 61-year-old female patient who presented with chronic headache, insidious mood, and cognitive disturbance which evolved in a few months to dementia associated with exuberant limb myoclonus. Diagnostic workup revealed high anti-thyroid peroxidase antibody titers and an inflammatory CSF profile, and it was negative for other possible etiologies. Treatment with steroids induced significant improvement. The diagnosis of encephalopathy associated with autoimmune thyroid disease is still controversial given the fact that the clinical presentation and diagnostic workup are unspecific, the pathophysiology is still undetermined, and the diagnosis is mostly of exclusion. No direct correlation is found between anti-thyroid antibody titers and clinical presentation, and it is currently speculated that other still unrecognized antibodies may be responsible for this clinical entity. It is extremely important to recognize this entity because it is potentially treatable with immunotherapies. It is also increasingly recognized that clinical improvement with first-line treatment with steroids may be absent or incomplete, and other immunotherapies as immunosuppressants, intravenous immunoglobulin, or plasma exchange must be attempted in the clinical suspicion of EEAT.info:eu-repo/semantics/publishedVersio

    The new automated daily mortality surveillance system

    The experience reported in an earlier Eurosurveillance issue on a fast method to evaluate the impact of the 2003 heatwave on mortality in Portugal, generated a daily mortality surveillance system (VDM) that has been operating ever since jointly with the Portuguese Heat Health Watch Warning System. This work describes the VDM system and how it evolved to become an automated system operating year-round, and shows briefly its potential using mortality data from January 2006 to June 2009 collected by the system itself. The new system has important advantages such as: rapid information acquisition, completeness (the entire population is included), lightness (very little information is exchanged, date of death, age, sex, place of death registration). It allows rapid detection of impacts (within five days) and allows a quick preliminary quantification of impacts that usually took several years to be done. These characteristics make this system a powerful tool for public health action. The VDM system also represents an example of inter-institutional cooperation, bringing together organisations from two different ministries, Health and Justice, aiming at improving knowledge about the mortality in the population

    Spontaneous Intracranial Hypotension Treated with a Targeted CT-Guided Epidural Blood Patch

    Spontaneous intracranial hypotension (SIH) is an important cause of new daily persistent headache. It is thought to be due to spontaneous spinal cerebrospinal fluid (CSF) leaks, which probably have a multifactorial etiology. The classic manifestation of SIH is an orthostatic headache, but other neurological symptoms may be present. An epidural blood patch is thought to be the most effective treatment, but a blind infusion may be ineffective. We describe the case of a young man who developed an acute severe headache, with pain worsening when assuming an upright posture and relief gained with recumbency. No history of previous headache, recent cranial or cervical trauma, or invasive procedures was reported. Magnetic resonance imaging showed pachymeningeal enhancement and other features consistent with SIH and pointed towards a cervical CSF leak site.     Predictors of first-line treatment persistence in a Portuguese cohort of relapsing-remitting multiple sclerosis

    Treatment persistence in first-line injectable disease-modifying therapies (DMT) for relapsing-remitting multiple sclerosis (RRMS) is an important indicator of effectiveness. Identifying predictors of treatment discontinuation is important as there are other therapies currently available and a growing range of emerging drugs. We report a retrospective study of RRMS and clinically isolated syndrome patients followed in a University Hospital during a 13-year period with the objective of identifying predictors of treatment persistence. An evaluation of persistence on the first DMT, rates of DMT discontinuation, and reasons and predictors of discontinuation was performed. A total of 410 patients were included, 69% female, with mean disease duration of 37.8months, mean age of 34.2years and mean follow-up time of 6.1years. The first DMT was glatiramer acetate (GA) in 27.56% of patients, interferon (IFN) β-1a intramuscular in 26.34%, IFNβ-1b in 26.10%, IFNβ-1a22 in 13.66% and IFNβ-1a44 in 6.34%. Treatment was discontinued in 16.34% of patients after 1year of treatment and in 50.24% of patients in the total follow-up time, with a mean time for discontinuation of 39.80months. Higher baseline Expanded Disability Status Scale score was an independent predictor of treatment discontinuation (hazard ratio 1.35, p=0.002). After the first year, treatment persistence was 90.74% for IFNβ-1a-IM, 88.46% for IFNβ-1a44, 83.18% for IFNβ-1b, 83.19% for GA and 69.64% for IFNβ-1a22 (p=0.014). Lower frequency of administration was associated with higher persistence rates.     Potential bio-activity of whey fermented extract as sanitizer of organic grown lettuce

    Short CommunicationConsumption of vegetables is increasing due to demand for healthy products in peoples' diets. To reduce microbial contamination and maintain freshness, industrial processes in Portugal rely on minimally processing of vegetables with hypochlorite as sanitizer. Formation of toxic chlorine derivatives has raised concern restrictions to its use and alternatives with whey permeate as a disinfection agent has been attempted. The aim of this work was to evaluate the bio potential of fermented cheese whey, for use on disinfection of minimally processed lettuce organically grown. Assays were made with whey obtained from inoculated milk during cheese processing, fermented for 120 h at 37 C, after which, among other carbohydrates, lactic acid was measured by HPLC, giving average yields of 18 g L 1. The sanitizing effect of whey, undiluted, 75 and 50% solutions, was compared with 110 ppm sodium hypochlorite, after rinsing. Aerobic Microorganisms (AM), Psychrotrophic Microorganisms (PM) and Enterobacteriaceae (ENT), were used as indicators for hygiene quality. For a level of significance of P < 0.05, the hygiene quality standards of lettuce samples, were better using 75% whey solution (AM 6.62, PM 7.48 cfu g 1), than using sodium hypochlorite (AM 7.48, PM 8.15 cfu g 1), for the 7 days of shelf life studied. Evaluation of Enterobacteriaceae showed significant differences after 3 days, betweenwater (ENT 4.98 cfu g 1) sodium hypochlorite (ENT 4.81 cfu g 1) and 75% solution of whey (ENT 4.63 cfu g 1). Considering the actual limitations imposed to chlorine sanitation, these results point a good alternative to the food industry, especially for organic fresh vegetables, which are chemical free brandsinfo:eu-repo/semantics/publishedVersio
