    Tornadoes in a Microchannel

    In non-dilute colloidal suspensions, gradients in particle volume fraction result in gradients in electrical conductivity and permittivity. An externally applied electric field couples with gradients in electrical conductivity and permittivity and, under some conditions, can result in electric body forces that drive the flow unstable forming vortices. The experiments are conducted in square 200 micron PDMS microfluidic channels. Colloidal suspensions consisted of 0.01 volume fraction of 2 or 3 micron diameter polystyrene particles in 0.1 mM Phosphate buffer and 409 mM sucrose to match particle-solution density. AC electric fields at 20 Hz and strength of 430 to 600 V/cm were used. We present a fluid dynamics video that shows the evolution of the particle aggregation and formation of vortical flow. Upon application of the field particles aggregate forming particle chains and three dimensional structures. These particles form rotating bands where the axis of rotation varies with time and can collide with other rotating bands forming increasingly larger bands. Some groups become vortices with a stable axis of rotation. Other phenomena showed include counter rotating vortices, colliding vortices, and non-rotating particle bands with internal waves

    It is well known that for the Gaussian broadcast channel, timeshared coding is not as efficient as more sophisticated broadcast coding strategies. However, the relative advantage of broadcast coding over timeshared coding is shown to be small if the signal-to-noise ratios of both receivers are small. One surprising consequence of this is that for the wideband Gaussian broadcast channel, which is defined, broadcast coding offers no advantage over timeshared coding at all, and so timeshared coding is optimal

    Daunting fiscal policy challenges face democratic systems throughout the world. Fiscal austerity in the wake of the Great Recession prompted nations in the Organisation for Economic Co-operation and Development (OECD) to institute major spending cuts and tax increases, increases that caused political and social fallout for years to come. While economies and budgets have improved in the past several years, significant fiscal adjustments lie ahead due to aging populations and the seemingly inexorable growth of health care costs. Faced with larger cohorts of retirees and fewer workers, nations will have to come to grips with a fiscal reality of higher spending and lower revenues for the foreseeable future. This article examines whether and how fiscal austerity measures are politically stabilizing or destabilizing for elected leaders responsible for their imposition. Many would predict that democracies are ill suited to make these hard choices, and certainly regimes have gone down to defeat following the formulation of austerity programs. However, the record is mixed-many regimes have found strategies to mitigate the political fallout and guarantee reelection in the wake of austerity. While traditional incremental and pluralist politics continue to characterize the budgetary strategies of many systems, major policy reforms have risen to the center of policy agendas elsewhere. While the magnitude of the fiscal crisis prompted the adoption of major policy shifts in hard-pressed nations, such reforms were undergirded by a more volatile policy process featuring the emergence of new pathways to power with greater roles for experts and symbolically compelling ideas

    Induction chemotherapy in locally advanced squamous cell carcinoma of the head and neck: role, controversy, and future directions.

    BackgroundThe value of induction chemotherapy (ICT) remains under investigation despite decades of research. New advancements in the field, specifically regarding the induction regimen of choice, have reignited interest in this approach for patients with locally advanced squamous cell carcinoma of the head and neck (LA SCCHN). Sufficient evidence has accumulated regarding the benefits and superiority of TPF (docetaxel, cisplatin, and fluorouracil) over the chemotherapy doublet cisplatin and fluorouracil. We therefore sought to collate and interpret the available data and further discuss the considerations for delivering ICT safely and optimally selecting suitable post-ICT regimens.DesignWe nonsystematically reviewed published phase III clinical trials on TPF ICT in a variety of LA SCCHN patient populations conducted between 1990 and 2017.ResultsTPF may confer survival and organ preservation benefits in a subgroup of patients with functionally inoperable or poor-prognosis LA SCCHN. Additionally, patients with operable disease or good prognosis (who are not candidates for organ preservation) may benefit from TPF induction in terms of reducing local and distant failure rates and facilitating treatment deintensification in selected populations. The safe administration of TPF requires treatment by a multidisciplinary team at an experienced institution. The management of adverse events associated with TPF and post-ICT radiotherapy-based treatment is crucial. Finally, post-ICT chemotherapy alternatives to cisplatin concurrent with radiotherapy (i.e. cetuximab or carboplatin plus radiotherapy) appear promising and must be investigated further.ConclusionsTPF is an evidence-based ICT regimen of choice in LA SCCHN and confers benefits in suitable patients when it is administered safely by an experienced multidisciplinary team and paired with the optimal post-ICT regimen, for which, however, no consensus currently exists
