    Inserción laboral de la formación dual técnica y tecnológica en el Ecuador

    The research had as a general objective to analyze the labor insertion of the technical and technological dual training in Ecuador from the case of the Superior Technological Institute “Luis Arboleda Martínez”; located in the city of Manta - Ecuador, it was a non-experimental descriptive type, the population was made up of 19 representatives of companies that welcome students in their organizations as part of the dual training process, knowing their opinions, through the application of a questionnaire type checklist. It is also necessary to deepen scientific research from a perspective that allows concatenating the academic - business - social, in order that the institution can fulfill the role of teaching, research and social bonding with relevance to optimize the resources used , together that entrepreneurs are formed from the generation of research projects from an application approach.La investigación tuvo como objetivo general analizar la inserción laboral de la formación dual técnica y tecnológica en el Ecuador desde el caso del Instituto Tecnológico Superior “Luis Arboleda Martínez”; ubicado en la ciudad de Manta – Ecuador, fue de tipo descriptiva no experimental, la población estuvo conformada 19 representantes de empresas que acogen a los estudiantes en sus organizaciones como parte del proceso de formación dual, conociéndose sus apreciaciones, mediante la aplicación de un cuestionario tipo lista de cotejo. Se hace necesario además profundizar en la investigación científica desde una perspectiva que permita concatenar lo académico – empresarial – social, con la finalidad de que en la institución se pueda cumplir con el rol de docencia, investigación y vinculación social con pertinencia a optimizar los recursos empleados, aunado que se forman emprendedores desde la generación de proyectos investigativos desde un enfoque aplicativo.

    Frecuencia de M. hyopneumoniae, M. hyorhinis y M. hyosynoviae en muestras nasales y de pulmón de cerdos con síntomas de neumonía enzoótica porcina

    M. hyopneumoniae, M. hyorhinis and M. hyosynoviae are genetically related species of the genus Mycoplasma that affect pig production. The objective of this work was the isolation and identification by PCR of M. hyopneumoniae, M. hyorhinis and M. hyosynoviae from nasal swabs and lung samples of pigs from different regions of Mexico in order to determine the frequency of these species and to evaluate PCR as a diagnostic tool for PEP. Pigs aged 4 to 8 weeks with clinical diagnosis of PEP were included. Lung samples and nasal swabs were obtained for the isolation of the Mycoplasma in liquid Friis medium and identified by species-specific PCR based on the 16S rRNA subunit. Isolation was achieved in 37.11 % (36/97) of the samples. The three Mycoplasma species were identified in lung and nasal swab samples. Mycoplasma co-infection was identified in 27.77 % (10/36). The bacterial genera associated with Mycoplasma infections were E. coli, Bordetella, Enterobacter, SCN, Corynebacterium, Pasteurella, Streptococcus, Shigella and Klebsiella. Mixed infection was present in 26 nasal swabs (45.61 %) and absent in the lungs. It was concluded that the frequency of  Mycoplasma on  production  farms  was higher  than  expected (40.27 %). It was also identified other Mycoplasma species involved in the development of PEP. Therefore, surveillance through isolation and molecular techniques can be of great help to breeding stock providers, as well as for removing Mycoplasma from pig farms.M. hyopneumoniae, M. hyorhinis y M. hyopsynoviae son especies genéticamente relacionadas del género Mycoplasma que afectan la producción porcina. El objetivo de este trabajo fue el aislamiento e identificación por PCR de M. hyopneumoniae, M. hyorhinis y M. hyosynoviae a partir de hisopos nasales y muestras de pulmón de cerdos de diferentes regiones mexicanas para determinar la frecuencia de estas especies y evaluar la PCR como herramienta diagnóstica para NEP. Se incluyeron cerdos de 4 a 8 semanas con diagnóstico clínico de NEP. Se obtuvieron muestras de pulmón e hisopos nasales para el aislamiento de Mycoplasma en medio Friis líquido y se identificaron mediante la PCR especie-específica basada en la subunidad 16S rRNA. El aislamiento se logró en 37.11 % (36/97) muestras. Las tres especies de Mycoplasma se identificaron en muestras de pulmón e hisopos nasales. La co-infección por Mycoplasma se identificó en el 27.77 % (10/36). Los géneros bacterianos asociados a las infecciones por Mycoplasma fueron E. coli, Bordetella, Enterobacter, SCN, Corynebacterium, Pasteurella, Streptococcus, Shigella y Kebsiella. La infección mixta estuvo presente en 26 hisopos nasales (45.61 %) y ausente en pulmones. Se concluyó que la frecuencia de Mycoplasma en las fincas de producción fue mayor a la esperada (40.27 %). También se identificaron otras especies de Mycoplasma involucradas en el desarrollo de la NEP. Por lo tanto, la vigilancia asistida por el aislamiento y las técnicas moleculares pueden ser de gran ayuda para la eliminación de Mycoplasma de las explotaciones porcinas y para los proveedores de pie de cría

    Epidemiological aspects of cutaneous leishmaniasis in the Iguazú falls area of Argentina

    Over the last three decades the incidence of American cutaneous leishmaniasis (ACL) has increased sharply in Argentina and throughout the world. In the Iguazú Falls area, on the border between Brazil and Paraguay, the incidence of human ACL has risen since 2004. Most of the 36 cases of human ACL reported until 2005 have involved males over 15 years old (75%) infected during deforestation to establish individual farms. Captures carried out in primary forest, periurban areas, and deforested land sites yielded 18,438 sand flies belonging to 13 species; the most prevalent species were Lutzomyia (Nyssomyia) whitmani (87.4%) and Lutzomyia (Mygonemyia.) migonei (7.6%). Cluster analysis was used to group traps according to species and abundance of sand flies. The group of traps located in recently deforested places, in pig and chicken dwellings of houses where ACL cases had been reported in the past, and at one house with an active ACL case, had the highest abundance of Lu. whitmani and Lu. whitmani + Lu. migonei as well as the highest ratio of Lu. whitmani/Lu. migonei. Leishmania sp. infections, both in Lu. whitmani, in Lu. quinquefer, and in smears from human cases were detected by DNA kinetoplast amplification using a generic PCR protocol. The risk of ACL outbreak in the Iguazú Falls area is still associated with economic and leisure activities in primary-secondary forest, including deforestation, rural settlements, fishing, hunting, and ecotourism. In addition, the risk of periurban transmission seems likely, and this is discussed within the framework of surveillance and prevention strategies. © 2008 Elsevier B.V.     Surgical outcomes of gallbladder cancer: the OMEGA retrospective, multicentre, international cohort study

    Background Gallbladder cancer (GBC) is rare but aggressive. The extent of surgical intervention for different GBC stages is non-uniform, ranging from cholecystectomy alone to extended resections including major hepatectomy, resection of adjacent organs and routine extrahepatic bile duct resection (EBDR). Robust evidence here is lacking, however, and survival benefit poorly defined. This study assesses factors associated with recurrence-free survival (RFS), overall survival (OS) and morbidity and mortality following GBC surgery in high income countries (HIC) and low and middle income countries (LMIC).Methods The multicentre, retrospective Operative Management of Gallbladder Cancer (OMEGA) cohort study included all patients who underwent GBC resection across 133 centres between 1st January 2010 and 31st December 2020. Regression analyses assessed factors associated with OS, RFS and morbidity.Findings On multivariable analysis of all 3676 patients, wedge resection and segment IVb/V resection failed to improve RFS (HR 1.04 [0.84-1.29], p = 0.711 and HR 1.18 [0.95-1.46], p = 0.13 respectively) or OS (HR 0.96 [0.79-1.17], p = 0.67 and HR 1.48 [1.16-1.88], p = 0.49 respectively), while major hepatectomy was associated with worse RFS (HR 1.33 [1.02-1.74], p = 0.037) and OS (HR 1.26 [1.03-1.53], p = 0.022). Furthermore, EBDR (OR 2.86 [2.3-3.52], p < 0.0010), resection of additional organs (OR 2.22 [1.62-3.02], p < 0.0010) and major hepatectomy (OR 3.81 [2.55-5.73], p < 0.0010) were all associated with increased morbidity and mortality. Compared to LMIC, patients in HIC were associated with poorer RFS (HR 1.18 [1.02-1.37], p = 0.031) but not OS (HR 1.05 [0.91-1.22], p = 0.48). Adjuvant and neoadjuvant treatments were infrequently used.Interpretation In this large, multicentre analysis of GBC surgical outcomes, liver resection was not conclusively associated with improved survival, and extended resections were associated with greater morbidity and mortality without oncological benefit. Aggressive upfront resections do not benefit higher stage GBC, and international col-laborations are needed to develop evidence-based neoadjuvant and adjuvant treatment strategies to minimise surgical morbidity and prioritise prognostic benefit.Funding Cambridge Hepatopancreatobiliary Department Research Fund.Copyright & COPY; 2023 The Author(s).
