    Relationship between influx of yellow dust and bronchial asthma mortality using satellite data

    The amount of yellow dust that reaches Japan has recently increased. This increase could worsen asthma attacks. However, it is difficult to conduct quantitative research because yellow dust is widely dispersed. Therefore, very few epidemiological studies regarding yellow dust and asthma have been conducted. Given that, we applied a monitoring method that can investigate the amount of incoming yellow dust by using satellite data to this epidemiological study. This study attempts to shed light on such an important public health issue in Asia where cross boundary air pollution problems are increasing in recent years, by using remotely sensed satellite data and to examine the influx of yellow dust and its association with bronchial asthma mortality in Western Japan. We evaluated the relationship between the annual average amount of incoming yellow dust obtained from satellite data and the annual average mortality rate from asthma. Spearman\u27s rank correlation result revealed no significant correlation (r = 0.268, n = 8, P > 0.05). However, we were able to conduct a quantitative analysis on the influx of yellow dust conditions using satellite data collected over a period of 10 years. This confirms the applicability of use of satellite data in assessing future epidemiological research regarding yellow dust and air polluted related diseases

    Causes of In-Hospital Death and Pharmaceutical Associations with Age of Death during a 10-Year Period (2011–2020) in Individuals with and without Diabetes at a Japanese Community General Hospital

    Since diabetes and its complications have been thought to exaggerate cardiorenal disease, resulting in a short lifespan, we investigated causes of death and lifespans in individuals with and without diabetes at a Japanese community general hospital during the period from 2011 to 2020. Causes of death and age of death in individuals with and those without diabetes were compared, and associations between medications used and age of death were statistically analyzed. A total of 2326 deaths were recorded during the 10-year period. There was no significant difference between the mean ages of death in individuals with and those without diabetes. Diabetic individuals had higher rates of hepato-pancreatic cancer and cardio-renal failure as causes of death. The prescription rates of antihypertensives, antiplatelets, and statins in diabetic individuals were larger than those in non-diabetic individuals. Furthermore, the use of sulfonyl urea or glinides and insulin was independently and inversely associated with the age of death. In conclusion, individuals with diabetes were treated with comprehensive pharmaceutical interventions and had life spans comparable to those of individuals without diabetes. This study’s discovery of an inverse relationship between the use of insulin secretagogues or insulin and the age of death suggests that the prevention of life-threatening hypoglycemia is crucial for individuals with diabetes

    Insulinoma with symptoms of suspected transient ischemic attack : A case report

    We report the case of a67-year-old woman who had symptoms suggestive of a transient ischemic attack(TIA), such as lightheadedness and transient visual changes before meals for 4 months. She experienced altered consciousness before lunch and was taken to the emergency room2weeks ago. She had repeated hypoglycemia with a blood glucose level of 31 mg/dL. Insulin secretion was not suppressed, with an immunoreactive insulin level of 14.0 μU/mL and connecting peptide immunoreactivity of 1.83 ng/mL for occasional blood glucose levels of 49 mg/dL. Dynamic CT revealed a 17‐mm mass enhanced during the arterial phase in the pancreatic uncinate process, suggestive of a pancreatic neuroendocrine tumor. A selective arterial secretagogue(calcium)injection test revealed the localization of insulinoma in the head of the pancreas. Therefore, pancreatoduodenectomy was performed. Hyperglycemia occurred after the surgery, and it was judged that the insulinoma was resected. This case showed TIA-like symptoms without signs of sympathetic overdrive associated with hypoglycemia. Thus, the diagnosis was delayed. Insulinoma may present with symptoms of neuroglycopenia but not autonomic activity due to hypoglycemia. Insulinoma should be distinguished in patients with unknown neurological symptoms since neuroglycopenia caused by insulinoma is diverse

    The Japanese Clinical Practice Guidelines for Management of Sepsis and Septic Shock 2020 (J-SSCG 2020)

    The Japanese Clinical Practice Guidelines for Management of Sepsis and Septic Shock 2020 (J-SSCG 2020), a Japanese-specific set of clinical practice guidelines for sepsis and septic shock created as revised from J-SSCG 2016 jointly by the Japanese Society of Intensive Care Medicine and the Japanese Association for Acute Medicine, was first released in September 2020 and published in February 2021. An English-language version of these guidelines was created based on the contents of the original Japanese-language version. The purpose of this guideline is to assist medical staff in making appropriate decisions to improve the prognosis of patients undergoing treatment for sepsis and septic shock. We aimed to provide high-quality guidelines that are easy to use and understand for specialists, general clinicians, and multidisciplinary medical professionals. J-SSCG 2016 took up new subjects that were not present in SSCG 2016 (e.g., ICU-acquired weakness [ICU-AW], post-intensive care syndrome [PICS], and body temperature management). The J-SSCG 2020 covered a total of 22 areas with four additional new areas (patient- and family-centered care, sepsis treatment system, neuro-intensive treatment, and stress ulcers). A total of 118 important clinical issues (clinical questions, CQs) were extracted regardless of the presence or absence of evidence. These CQs also include those that have been given particular focus within Japan. This is a large-scale guideline covering multiple fields; thus, in addition to the 25 committee members, we had the participation and support of a total of 226 members who are professionals (physicians, nurses, physiotherapists, clinical engineers, and pharmacists) and medical workers with a history of sepsis or critical illness. The GRADE method was adopted for making recommendations, and the modified Delphi method was used to determine recommendations by voting from all committee members.As a result, 79 GRADE-based recommendations, 5 Good Practice Statements (GPS), 18 expert consensuses, 27 answers to background questions (BQs), and summaries of definitions and diagnosis of sepsis were created as responses to 118 CQs.     Oxygen-Atom Transfer from Iodosylarene Adducts of a Manganese(IV) Salen Complex: Effect of Arenes and Anions on I(III) of the Coordinated Iodosylarene

    This paper reports preparation, characterization, and reactivity of iodosylarene adducts of a manganese­(IV) salen complex. In order to systematically investigate steric and electronic factors that control reactivity and selectivity, we prepared iodosylarene adducts from iodosylbenzene, iodosylmesitylene, 2,4,6-triethyliodosylbenzene, and pentafluoroiodosylbenzene. We also investigated the effect of anions on I­(III) by using chloride, benzoate, and <i>p</i>-toluenesulfonate. Spectroscopic studies using <sup>1</sup>H NMR, electron paramagnetic resonance, infrared spectroscopy, and electrospray ionization mass spectrometry show that these iodosylarene adducts are manganese­(IV) complexes bearing two iodosylarenes as external axial ligands. Reactions with thioanisole under the pseudo-first-order conditions show that the electron-withdrawing pentafluorophenyl group and the <i>p</i>-toluenesulfonate anion on I­(III) significantly accelerate the oxygen-atom transfer. The high reactivity is correlated with a weakened I–OMn bond, as indicated by IR spectroscopy and mass spectrometry. Stoichiometric reactions with styrenes show that both enantioselectivity and diastereoselectivity are dependent on the arenes and anions on I­(III) of the coordinate iodosylarenes. Notably, the pentafluorophenyl group and the <i>p</i>-toluenesulfonate anion suppress the cis-to-trans isomerization in the epoxidation of <i>cis</i>-β-methylstyrene. The present results show that iodosylarene adducts of manganese­(IV) salen complexes are indeed active oxygen-atom-transfer reagents and that their reactivity and selectivity are regulated by steric and electronic properties of the arenes and anions on I­(III) of the coordinated iodosylarenes
