    Electro-Mechanical Fredericks Effects in Nematic Gels

    The solid nematic equivalent of the Fredericks transition is found to depend on a critical field rather than a critical voltage as in the classical case. This arises because director anchoring is principally to the solid rubbery matrix of the nematic gel rather than to the sample surfaces. Moreover, above the threshold field, we find a competition between quartic (soft) and conventional harmonic elasticity which dictates the director response. By including a small degree of initial director misorientation, the calculated field variation of optical anisotropy agrees well with the conoscopy measurements of Chang et al (Phys.Rev.E56, 595, 1997) of the electro-optical response of nematic gels.Comment: Latex (revtex style), 5 EPS figures, submitted to PRE, corrections to discussion of fig.3, cosmetic change

    Hot subdwarfs in close binaries observed from space : II. Analyses of the light variations

    Context. Hot subdwarfs in close binaries with either M dwarf, brown dwarf, or white dwarf companions show unique light variations. In hot subdwarf binaries with M dwarf or brown dwarf companions, we can observe the so-called reflection effect, while in hot subdwarfs with close white dwarf companions, we find ellipsoidal modulation and/or Doppler beaming. Aims. Analyses of these light variations can be used to derive the mass and radius of the companion and determine its nature. Thereby, we can assume the most probable sdB mass and the radius of the sdB derived by the fit of the spectral energy distribution and the Gaia parallax. Methods. In the high signal-to-noise space-based light curves from the Transiting Exoplanet Survey Satellite and the K2 space mission, several reflection effect binaries and ellipsoidal modulation binaries have been observed with much better quality than with ground-based observations. The high quality of the light curves allowed us to analyze a large sample of sdB binaries with M dwarf or white dwarf companions using LCURVE. Results. For the first time, we can constrain the absolute parameters of 19 companions of reflection effect systems, covering periods from 2.5 to 19 h and with companion masses from the hydrogen-burning limit to early M dwarfs. Moreover, we were able to determine the mass of eight white dwarf companion to hot subdwarf binaries showing ellipsoidal modulations, covering the as-yet unexplored period range of 7 to 19 h. The derived masses of the white dwarf companions show that all but two of the white dwarf companions are most likely helium-core white dwarfs. Combining our results with previously measured rotation velocities allowed us to derive the rotation period of seven sdBs in short-period binaries. In four of those systems, the rotation period of the sdB agrees with a tidally locked orbit, whereas in the other three systems, the sdB rotates significantly more slowly

    Spectral Analysis of Binary Pre-white Dwarf Systems

    Short period double degenerate white dwarf (WD) binaries with periodsof less than ∼1 day are considered to be one of the likely progenitors of type Ia super-novae. These binaries have undergone a period of common envelope evolution. If thecore ignites helium before the envelope is ejected, then a hot subdwarf remains priorto contracting into a WD. Here we present a comparison of two very rare systems thatcontain two hot subdwarfs in short period orbits. We provide a quantitative spectro-scopic analysis of the systems using synthetic spectra from state-of-the-art non-LTEmodels to constrain the atmospheric parameters of the stars.     Relationship of glycated haemoglobin and reported hypoglycaemia to cardiovascular outcomes in patients with type 2 diabetes and recent acute coronary syndrome events: The EXAMINE trial.

    AIMS: To investigate relationships between glycated haemoglobin (HbA1c) and reported hypoglycaemia and risk of major adverse cardiovascular events (MACE). METHODS: The EXAMINE trial randomized 5380 patients with type 2 diabetes (T2DM) and a recent acute coronary syndrome (ACS) event, in 49 countries, to double-blind treatment with alogliptin or placebo in addition to standard of care. We used Cox proportional hazards models to analyse relationships among MACE, HbA1c levels and hypoglycaemic events. RESULTS: Patients randomized to alogliptin achieved lower HbA1c levels than the placebo group in all baseline HbA1c categories without differences in hypoglycaemia rates. No systematic change was found in MACE rates according to baseline HbA1c (Pinteraction  = 0.971) or HbA1c category at 1 month. Patients in the combined treatment groups (n = 5380) who experienced serious hypoglycaemia (n = 34) had higher MACE rates than those who did not (35.3% vs 11.4%, adjusted hazard ratio [HR] 2.42, 95% confidence interval [CI] 1.27-4.60; P = .007), although the association was less strong when analysing only events after the hypoglycaemic event (adjusted HR 1.60, 95% CI 0.80, 3.20). CONCLUSIONS: There were no relationships between baseline HbA1c levels or HbA1c levels after 1 month of treatment and the risk of MACE. Alogliptin improved glycaemic control without increasing hypoglycaemia. Reported events of hypoglycaemia and serious hypoglycaemia were associated with MACE. These data underscore the safety of alogliptin in improving glycaemic control in T2DM post-ACS. Further study of hypoglycaemia as an independent risk factor for MACE in patients with T2DM and coronary disease is needed

    The EREBOS project -- Investigating the effect of substellar and low-mass stellar companions on late stellar evolution

    Eclipsing post-common envelope binaries are highly important for resolving the poorly understood, very short-lived common envelope phase. Most hot subdwarfs (sdO/Bs) are the bare He-burning cores of red giants which have lost almost all of their hydrogen envelopes. This mass loss is often triggered by common envelope interactions with close stellar or even sub-stellar companions. In the recently published catalog of eclipsing binaries in the Galactic Bulge and in the ATLAS survey, we discovered 161 new eclipsing systems showing a reflection effect by visual inspection of the light curves and using a machine-learning algorithm. The EREBOS (Eclipsing Reflection Effect Binaries from Optical Surveys) project aims at analyzing all newly discovered eclipsing binaries with reflection effect based on a spectroscopic and photometric follow up. To constrain the nature of the primary we derived the absolute magnitude and the reduced proper motion of all our targets with the help of the parallaxes and proper motions measured by the Gaia mission and compared those to the Gaia white dwarf catalogue. For a sub-set of our targets with observed spectra the nature could be derived by measuring the atmospheric parameter of the primary confirming that less than 10\% of our systems are not sdO/Bs with cool companions but white dwarfs or central stars of planetary nebula. This large sample of eclipsing hot subdwarfs with cool companions allowed us to derive a significant period distribution for hot subdwarfs with cool companions for the first time showing that the period distribution is much broader than previously thought and ideally suited to find the lowest mass companions to hot subdwarf stars. In the future several new photometric surveys will be carried out, which will increase the sample of this project even more giving the potential to test many aspects of common envelope theory and binary evolution.Comment: accepted in A&A, 29 pages, 18 figure

    Cutaneous lupus erythematosus after treatment with paclitaxel and bevacizumab for metastatic breast cancer: a case report

    Abstract Introduction The monoclonal anti-vascular endothelial growth factor antibody bevacizumab is increasingly used in the treatment of several malignant tumors. The usual side effects of this drug are hypertension and proteinuria. Paclitaxel is widely used in the treatment of breast cancer and head and neck carcinomas. Neither of these two drugs typically causes skin disorders. Paclitaxel-related cutaneous lupus erythematosus has been described before, but in earlier cases patients had a history of autoimmune disease. Case presentation We report a case of a 65-year-old Caucasian woman who presented with cutaneous lupus erythematosus after receiving paclitaxel-bevacizumab combination treatment as first-line therapy for metastatic breast cancer. Her cutaneous symptoms and increased serum anti-SSA and anti-SSB antibodies disappeared shortly after the discontinuation of therapy. Conclusion We conclude that cutaneous lupus erythematosus can also be seen in patients without earlier anamnesis of autoimmune disorders and that, furthermore, bevacizumab might cause atypical cutaneous side effects.</p

    Considering the case for an antidepressant drug trial involving temporary deception: a qualitative enquiry of potential participants

    <p>Abstract</p> <p>Background</p> <p>Systematic reviews of randomised placebo controlled trials of antidepressant medication show small and decreasing differences between pharmacological and placebo arms. In part this finding may relate to methodological problems with conventional trial designs, including their assumption of additivity between drug and placebo trial arms. Balanced placebo designs, which include elements of deception, may address the additivity question, but pose substantial ethical and pragmatic problems. This study aimed to ascertain views of potential study participants of the ethics and pragmatics of various balanced placebo designs, in order to inform the design of future antidepressant drug trials.</p> <p>Methods</p> <p>A qualitative approach was employed to explore the perspectives of general practitioners, psychiatrists, and patients with experience of depression. The doctors were chosen via purposive sampling, while patients were recruited through participating general practitioners. Three focus groups and 12 in-depth interviews were conducted. A vignette-based topic guide invited views on three deceptive strategies: post hoc, authorised and minimised deception. The focus groups and interviews were tape-recorded and transcribed. Transcripts were analysed thematically using Framework.</p> <p>Results</p> <p>Deception in non-research situations was typically perceived as acceptable within specific parameters. All participants could see the potential utility of introducing deception into trial designs, however views on the acceptability of deception within antidepressant drug trials varied substantially. Authorized deception was the most commonly accepted strategy, though some thought this would reduce the effectiveness of the design because participants would correctly guess the deceptive element. The major issues that affected views about the acceptability of deception studies were the welfare and capacity of patients, practicalities of trial design, and the question of trust.</p> <p>Conclusion</p> <p>There is a trade-off between pragmatic and ethical responses to the question of whether, and under what circumstances, elements of deception could be introduced into antidepressant drug trials. Ensuring adequate ethical safeguards within balanced placebo designs is likely to diminish their ability to address the crucial issue of additivity. The balanced placebo designs considered in this study are unlikely to be feasible in future trials of antidepressant medication. However there remains an urgent need to improve the quality of antidepressant drug trials.</p
