    Training need assessment of nursing personnel as super specialty tertiary care hospitals in Northern India

    Background - Super specialty tertiary care hospital equips nurses with the requisite knowledge and skills to deliver high quality care in their practice areas. This has necessitated adopting a structured approach to the training needs assessment (TNA) to identify the development needs of the nursing workforce at a tertiary care centre. This study involves Micro Level – Task/Job analysis i.e. identifies and describes all the skills performed by employees in a particular job and the knowledge, skills, attitudes and other behaviours needed for successful job performance. Aim and objectives: The study explores the training needs of nursing personnel at super specialty tertiary care hospital in northern India by analyzing the areas of knowledge and skills deficit as perceived by the nurses and assessing the expectations of doctors, about the role of nurses and deficiencies in their current knowledge, skills Design: Structured questionnaire based descriptive cross sectional study Methods: A set of questionnaire were framed using core competencies of nurses listed by Massachusetts department of higher education and nursing council of Hongkong and A questionnaire based descriptive cross sectional study among nurses, faculty, senior residents and patients were done. Results: The total sample collected for the study was 274 which included 19 faculty, 44 senior résidents, 211 nurses. Training needs identified to be further developed for Grade II and grade I nurses both by nurses and doctors are maximum in patient care and research domain followed by managerial/administrative and communication domain. Training needs agreed to be promoted for ANS’s are maximum in managerial/ administrative and communication domain. Training needs recognized for DNS’s /NS/CNO are maximum in managerial/ administrative and research domain followed by communication domain Conclusion – This study reflects the training need assessment of nurses in a super specialty tertiary care hospita

    GASTRIC ANTISECRETORY AND ANTIULCER PROPERTIES OF ETHANOLIC EXTRACTS OF LEAVES OF ANDROGRAPHYS PANICULATA

    Objective: To study the gastric anti-secretory and cytoprotective action of ethanolic extract of leaves of Andrographys paniculata. Methods: Effect of ethanolic extract of leaves of A. paniculata gastro-antisecretory and cytoprotective activity was studied in vivo by using gastric wall mucus determination, hypothermic restraint stress-induced ulcers, idomethacin-induced gastric ulcers, reserpine-induced gastric ulcers and necrotizing agent-induced gastric ulcers (cytoprotection studies) methods. Results: Ethanolic extract of leaves of A. paniculata produced a significant decrease in gastric secretion in pylorous ligated rats and a highly significant cytoprotective effect against 80% ethanol-0.6 M HCl, 0.2 M NaOH, and 25% NaCl-induced cytodestruction. Conclusion: Pretreatment with the extract significantly prevented hypothermic stress-induced gastric wall mucus depletion. These findings suggest that a significant antisecretory and cytoprotective action A. paniculata can be responsible for its antiulcer activity

    SYNTHESIS AND EVALUATION OF ANTIMICROBIAL ACTIVITY OF PYRIMIDINE DERIVATIVES

    Objectives: Synthesis, characterization, and evaluation of antimicrobial activity of novel pyrimidine derivatives containing O, N, and S in the ring. Methods: Pyrimidine derivatives were prepared in three steps. In the first step, chalcones containing -NO2 functional group were synthesized using Claisen-Schmidt condensation of aromatic aldehydes with 2-acetyl pyridine/3-acetylpyridine in methanol in the presence of aqueous NaOH. In the second step, -NO2 group was reduced to -NH2 group. Resulting compounds containing NH2 functional group were reacted with different dichlorothienopyrimidines and dichlorofuropyrimidines in the presence of N,N-diisopropylethylamine to obtain pyrimidine derivatives.Antibacterial and antifungal activity of pyrimidine derivatives were studied in vitro. Results: Pyrimidine derivatives were synthesized and purified using flash column chromatography. Purity of synthesized pyrimidines was determined by high-performance liquid chromatography. Pyrimidines were characterized by elemental analysis, infrared, nuclear magnetic resonance, and mass spectral analysis. Analytical data of synthesized pyrimidines supported the proposed structures. Significant antibacterial and antifungal activity were observed in the synthesized pyrimidine derivatives. Conclusion: Antibacterial and antifungal activity of the newly synthesized pyrimidine derivatives will definitely inspire future researchers for the preparation of new analogs

    FPGA Based Hardware Co-Simulation of an Area and Power Efficient FIR Filter for Wireless Communication Systems

    “In this paper FPGA based hardware co-simulation of an area and power efficient FIR filter for wireless communication systems is presented. The implementation is based on distributed arithmetic (DA) which substitutes multiply-and-accumulate operations with look up table (LUT) accesses. Parallel Distributed arithmetic (PDA) look up table approach is used to implement an FIR Filter taking optimal advantage of the look up table structure of FPGA using VHDL. The proposed design is hardware co-simulated using System Generator10.1, synthesized with Xilinx ISE 10.1 software, and implemented on Virtex-4 based xc4vlx25-10ff668 target device. Results show that the proposed design operates at 17.5 MHz throughput and consumes 0.468W power with considerable reduction in required resources to implement the design as compared to Coregen and add/shift based design styles. Due to this reduction in required resources the proposed design can also be implemented on Spartan-3 FPGA device to provide cost effective solution for DSP and wireless communication applications.

    Isolation and identification of candida species from various clinical samples in a tertiary care hospital

    Background: Candida spp is a member of the normal flora of the skin, mucous membrane and gastrointestinal tract. They are endogenous opportunists which cause secondary infection in individuals with underlying immunocompromised conditions. Candidiasis is a common fungal disease in humans. An increase in the prevalence of non-albicans species has been noted during the last decades because of increasing use of azoles. This study aims to Spectate Candida using chromogenic medium.Methods: A total of 50 Candida isolates from various clinical samples were included in the study. These isolates were subjected to gram's stain, germ tube test and inoculation on commercially available CHROM agar (HiMedia India).Results: In current study majority of isolates were from high vaginal swab (34%) followed by sputum (28%), urine (18%), pus from surgical sites and others constituted to 20%. Candida albicans (51%) was the most common candida species, followed by C. tropicalis (25%), C. krusei (16%), C. glabrata (6%) and C. dubliniensis (1%).Conclusions: Along with Candida albicans, non-albicans candida spp like C. tropicalis, C. krusei, C. glabrata, and C. dubliniensis are increasingly being isolated from clinical samples. CHROM agar is a simple, rapid and inexpensive method for identification of such species. Characterization to species level helps to identify species which might be intrinsically resistant to commonly used antifungal agents

    Ovarian fibromas are the most common benign solid tumor of the ovary and usually are large and asymptomatic. It can mimic several conditions hence it can often be misdiagnosed pre-operatively. In this case the pre-operative diagnosis was bilateral ovarian tumor with endometrial hyperplasia. The final diagnosis was a torsion left ovarian fibroma with an area of hemorrhagic infarction and a benign endometrial polyp. The benefit of a multidisciplinary team approach and the challenges faced during the peri-operative period are discussed

    In vitro effect of Withania somnifera, AYUSH-64, and remdesivir on the activity of CYP-450 enzymes: Implications for possible herb−drug interactions in the management of COVID-19

    Ayurvedic medicines Withania somnifera Dunal (ashwagandha) and AYUSH-64 have been used for the prevention and management of COVID-19 in India. The present study explores the effect of Ashwagandha and AYUSH-64 on important human CYP enzymes (CYP3A4, CYP2C8, and CYP2D6) to assess their interaction with remdesivir, a drug used for COVID-19 management during the second wave. The study also implies possible herb−drug interactions as ashwagandha and AYUSH-64 are being used for managing various pathological conditions. Aqueous extracts of ashwagandha and AYUSH-64 were characterized using LC-MS/MS. A total of 11 and 24 phytoconstituents were identified putatively from ashwagandha and AYUSH-64 extracts, respectively. In addition, in silico studies revealed good ADME properties of most of the phytoconstituents of these herbal drugs and suggested that some of these might possess CYP-450 inhibitory activity. In vitro CYP-450 studies with human liver microsomes showed moderate inhibition of CYP3A4, 2C8, and 2D6 by remdesivir, while ashwagandha had no inhibitory effect alone or in combination with remdesivir. AYUSH-64 also exhibited a similar trend; however, a moderate inhibitory effect on CYP2C8 was noticed. Thus, ashwagandha seems to be safe to co-administer with the substrates of CYP3A4, CYP2C8, and CYP2D6. However, caution is warranted in prescribing AYUSH-64 along with CYP2C8 substrate drugs. Furthermore, preclinical and clinical PK studies would be helpful for their effective and safer use in the management of various ailments along with other drugs

    Examining Higher Education Instructor Perceptions of Roles and Competencies in Online Teaching

    Online instructors adopt various roles and perform various competencies in the design and delivery of online courses. In this study, online instructor roles are categorized into eight types including Subject Matter Expert, Course Designer and Developer, Course Facilitator, Course Manager, Advisor/Mentor, Assessor/Evaluator, Technology Expert, and Lifelong Learner. Through survey-based research with 141 online instructors, this study examines competencies that online instructors perform based on various roles. When rating competencies, overall categorical means for all the roles were rated above 4.00, which showed that they used all these roles. The highest rated items and lowest rated items are discussed in addition to the connection between research and practice in online teaching. Online instructors who participate in training and who collaborate with instructional designers rated the frequency with which they perform the competencies to be higher. This study has implications for online instructors, instructional designers, and administrators who design and deliver online learning and offer professional development for online instructors
