    Prospective study of management of unstable intertrochantric femur fractures in elderly: hemiarthroplasty vs. osteosynthesis by proximal femoral nail

    Background: A prospective study was done to compare the outcomes of management of unstable intertrochantric femur fractures in elderly by hemiarthroplasty and osteosynthesis by proximal femoral nail.Methods: This was an interventional prospective study. The study was carried out over 3 years from 2019 to 2022 at PCMC’s PGI YCMH, Pimpri, Pune, Maharashtra, India. The study was carried out over 3 years from 2019 to 2022;50 Patients were randomly divided into two groups; Group A (unstable intertrochanteric femur fracture patients treated by bipolar hemi-arthroplasty) and Group B (unstable intertrochanteric femur fracture patients treated by proximal femoral nailing) with 25 patients in each group. Outcomes were evaluated based on functional outcomes, mean surgical time, mean blood loss during surgery, post-operative length of stay, post-operative complications. Harris hip Score was used to assess clinical functional outcomes.Results: The PFN group showed a better Harris Hip score at 1, 3, and 6 months follow ups. Mean surgical time was more in bipolar hemiarthroplasty group. Mean blood loss during surgery was more in bipolar hemiarthroplasty group. Post-operative length of stay was more in bipolar hemiarthroplasty group. Post-operative complications like infections, limb length discrepencies were more in bipolar hemiarthroplasty group, whereas 2 implant failures occurred in PFN group compared to a single one in bipolar hemiarthroplasty group.Conclusions: Both PFN and bipolar-hemiarthroplasty appear to produce satisfactory outcomes in surgically treated unstable IT fractures, bipolar-hemiarthroplasty has the advantages of early mobilization and early weight bearing, no risk of non-union but PFN group is superior when it comes to functional outcomes, mean surgical time, Mean blood loss during surgery, post-operative length of stay, post-operative complications

    Medical disease prediction using Grey Wolf optimization and auto encoder based recurrent neural network

    Big data development in biomedical and medical service networks provides a research on medical data benefits, early ailment detection, patient care and network administrations.e-Health applications are particularly important for the patients who are unfit to see a specialist or any health expert. The objective is to encourage clinicians and families to predict disease using Machine Learning (ML) procedures. In addition, diverse regions show important qualities of certain provincial ailments, which may hinder the forecast of disease outbreaks. The objective of this work is to predict the different kinds of diseases using Grey Wolf optimization and auto encoder based Recurrent Neural Network (GWO+RNN). The features are selected using GWO and the diseases are predicted by using RNN method. Initially the GWO algorithm avoids the irrelevant and redundant attributes significantly, after the features are forwarded to the RNN classifier. The experimental result proved that the performance of GWO+RNN algorithm achieved better than existing method like Group Search Optimizer and Fuzzy Min-Max Neural Network (GFMMNN) approach. The GWO-RNN method used the medical UCI database based on various datasets such as Hungarian, Cleveland, PID, mammographic masses, Switzerland and performance was measured with the help of efficient metrics like accuracy, sensitivity and specificity. The proposed GWO+RNN method achieved 16.82% of improved prediction accuracy for Cleveland dataset

The common use of Online Social Networks (OSN)[2] for networking communication which authorizes real-time multimedia capturing and sharing, have led to enormous amounts of user-generated content in online, and made publicly available for analysis and mining. The efforts have been made for more privacy awareness to protect personal data against privacy threats. The principal idea in designing different marketing strategies is to identify the influencers in the network communication. The individuals influential induce “word-of-mouth” that effects in the network are responsible for causing particular action of influence that convinces their peers (followers) to perform a similar action in buying a product. Targeting these influencers usually leads to a vast spread of the information across the network. Hence it is important to identify such individuals in a network, we use centrality measures to identify assign an influence score to each user.     Solar energy–driven carbon dioxide (CO2) reduction to valuable solar fuels/chemicals (e.g., methane, ethanol, and carbon monoxide) using particulate photocatalysts is regarded as one of the promising and effective approaches to deal with energy scarcity and global warming. The growth of nanotechnology plays an eminent role in improving CO2 reduction (CO2R) efficiencies by means of offering opportunities to tailor the morphology of photocatalysts at a nanoscale regime to achieve enhanced surface reactivity, solar light absorption, and charge separation, which are decisive factors for high CO2R efficiency. Notably, quantum dots (QDs), tiny pieces of semiconductors with sizes below 20 nm, offering a myriad of advantages including maximum surface atoms, very short charge migration lengths, size-dependent energy band positions, multiple exciton generation effect, and unique optical properties, have recently become a rising star in the CO2R application. In this review, we briefly summarized the progress so far achieved in QD-assisted CO2 photoreduction, highlighting the advantages of QDs prepared with diverse chemical compositions such as metal oxides, metal chalcogenides, carbon, metal halide perovskites, and MXenes. © Copyright © 2021 Park, Murali, Modigunta, In and In.1

    Interplay between superconductivity and magnetism in K-doped EuFe2As2

    Superconductivity is found in 50% K-doped EuFe2As2 sample below 33 K. Our results of electrical resistivity, magnetic susceptibility and 57Fe and 151Eu Mossbauer spectroscopy provide clear evidence that the ordering of the Fe moments observed at 190 K in undoped EuFe2As2 is completely suppressed in our 50% K doped sample, thus there is no coexistence between the Fe magnetic order and the superconducting state. However, short range ordering of the Eu moments is coexisting with the superconducting state below 15 K. A bump in the susceptibility well below Tc as well as a slight broadening of the Fe Mossbauer line below 15 K evidence an interplay between the Eu magnetism and the superconducting state.Comment: 7 pages, 6 figure

    Flagellin-Induced Corneal Antimicrobial Peptide Production and Wound Repair Involve a Novel NF-κB–Independent and EGFR-Dependent Pathway

    The bacterial protein flagellin plays a major role in stimulating mucosal surface innate immune response to bacterial infection and uniquely induces profound cytoprotection against pathogens, chemicals, and radiation. This study sought to determine signaling pathways responsible for the flagellin-induced inflammatory and cytoprotective effects on human corneal epithelial cells (HCECs).Flagellin purified from Pseudomonas aeruginosa (strain PAK) or live bacteria were used to challenge cultured HCECs. The activation of signaling pathways was assessed with Western blot, and the secretion of cytokine/chemokine and production of antimicrobial peptides (AMPs) were measured with ELISA and dot blot, respectively. Effects of flagellin on wound healing were assessed in cultured porcine corneas. L94A (a site mutation in TLR5 binding region) flagellin and PAK expressing L94A flagellin were unable to stimulate NF-kappaB activation, but were potent in eliciting EGFR signaling in a TGF-alpha-related pathway in HCECs. Concomitant with the lack of NF-kappaB activation, L94A flagellin was ineffective in inducing IL-6 and IL-8 production in HCECs. Surprisingly, the secretion of two inducible AMPs, LL-37 and hBD2, was not affected by L94A mutation. Similar to wild-type flagellin, L94A induced epithelial wound closure in cultured porcine cornea through maintaining EGFR-mediated signaling.Our data suggest that inflammatory response mediated by NF-kappaB can be uncoupled from epithelial innate defense machinery (i.e., AMP expression) and major epithelial proliferation/repair pathways mediated by EGFR, and that flagellin and its derivatives may have broad therapeutic applications in cytoprotection and in controlling infection in the cornea and other mucosal tissues
