    A Left Temporal Arachnoid Cyst in a Patient with Early Onset Schizophrenia

    Arachnoid cysts are mostly asymptomatic and diagnosed incidentally. In the literature, some case studies report a probable etiological association between arachnoid cysts and psychiatric disorders. We report an early onset schizophrenia case who had a left temporal arachnoid cyst and who had an inadequate response to antipsychotic treatment. We discuss the potential association between arachnoid cysts and psychiatric disorders in the light of the literature

    Dynamic Interchanging Native States of Lymphotactin Examined by SNAPP-MS

    The human chemokine lymphotactin (Ltn) is a remarkable protein that interconverts between two unrelated native state structures in the condensed phase. It is possible to shift the equilibrium toward either conformation with selected sequence substitutions. Previous results have shown that a disulfide-stabilized variant preferentially adopts the canonical chemokine fold (Ltn10), while a single amino acid change (W55D) favors the novel Ltn40 dimeric structure. Selective noncovalent adduct protein probing (SNAPP) is a recently developed method for examining solution phase protein structure. Herein, it is demonstrated that SNAPP can easily recognize and distinguish between the Ltn10 and Ltn40 states of lymphotactin in aqueous solution. The effects of organic denaturants, acid, and disulfide bond reduction and blocking were also examined using SNAPP for the CC3, W55D, and wild type proteins. Only disulfide reduction was shown to significantly perturb the protein, and resulted in considerably decreased adduct formation consistent with loss of tertiary/secondary structure. Cold denaturation experiments demonstrated that wild-type Ltn is the most temperature sensitive of the three proteins. Examination of the higher charge states in all experiments, which are presumed to represent transition state structures between Ltn-10 and Ltn-40, reveals increased 18C6 attachment relative to the more folded structures. This observation is consistent with increased competitive intramolecular hydrogen bonding, which may guide the transition. Experiments examining the gas phase structures revealed that all three proteins can be structurally distinguished in the gas phase.     Objectives: Evidence suggests that lysosomal acid lipase deficiency (LAL-D) is often underdiagnosed because symptoms may be nonspecific. We aimed to investigate the prevalence of LAL-D in children with unexplained liver disease and to identify demographic and clinical features with a prospective, multicenter, cross-sectional study. Methods: Patients (aged 3 months-18 years) who had unexplained transaminase elevation, unexplained hepatomegaly or hepatosplenomegaly, obesity-unrelated liver steatosis, biopsy-proven cryptogenic fibrosis and cirrhosis, or liver transplantation for cryptogenic cirrhosis were enrolled. A Web-based electronic data collection system was used. LAL activity (nmol/punch/h) was measured using the dried blood spot method and classified as LAL-D(0.37). Asecond dried blood spot sample was obtained from patients with intermediate LAL activity for confirmation of the result. Results: A total of 810 children (median age 5.6 years) from 795 families were enrolled. The reasons for enrollment were unexplained transaminase elevation (62%), unexplained organomegaly (45%), obesity-unrelated liver steatosis (26%), cryptogenic fibrosis and cirrhosis (6%), and liver transplantation for cryptogenic cirrhosis (<1%). LAL activity was normal in 634 (78%) and intermediate in 174 (21%) patients. LAL-D was identified in 2 siblings aged 15 and 6 years born to unrelated parents. Dyslipidemia, liver steatosis, and mild increase in aminotransferases were common features in these patients. Moreover, the 15-year-old patient showed growth failure and microvesicular steatosis, portal inflammation, and bridging fibrosis in the liver biopsy. Based on 795 families, 2 siblings in the same family were identified as LAL-D cases, making the prevalence of LAL-D in this study population, 0.1% (0.125%-0.606%). In the repeated measurement (76/174), LAL activity remained at the intermediate level in 38 patients. Conclusions: Overall, the frequency of LAL-D patients in this study (0.1%) suggests that LAL-D seems to be rare even in the selected high-risk population

    Can partially hydrolyzed guar gum be an alternative to lactulose in treatment of childhood constipation?

    Background/aims: In the present study, we aimed to investigate if partially hydrolyzed guar gum (PHGG) can be used safely as a fiber source for treatment of constipation in children and to compare its success with the most commonly used osmotic laxative, lactulose. Methods: A randomized prospective controlled study on 61 patients (partially hydrolyzed guar gum group, n: 31; lactulose group, n: 30) was performed. Patients were given lactulose or partially hydrolyzed guar gum for four weeks. Using a standardized bowel diary, defecation frequency, stool consistency, and presence of flatulence and abdominal pain were recorded. Family questionnaires about the success, safety and side effect profile of both treatment arms were also obtained. Results: No significant differences were found in the baseline daily fiber (fruits and vegetables) intake between the two groups. Bowel movement frequency per week and stool consistency improved significantly in both treatment groups (p<0.05). The percent of children with abdominal pain and stool withholding also decreased eminently in both groups (p<0.05). Weekly defecation frequency increased from 4±0.7 to 6±1.06 and from 4±0.7 to 5±1.7 in the lactulose and partially hydrolyzed guar gum treated groups, respectively (p<0.05). According to the family questionnaire, the parents complained of bad taste, flatulence and necessity to ingest a high amount of drug in the lactulose treatment group. In the partially hydrolyzed guar gum treatment group, parents were satisfied with the defecation frequency of their children. Conclusions: Treatment with partially hydrolyzed guar gum is as effective as lactulose treatment in relieving stool withholding and constipation-associated abdominal pain, and its use improves stool consistency. Lactulose seemed to have more side effects, including flatulence and sensation of bad taste

    Impact of Shortened Winter Road Access on Costs of Forest Operations

    A significant portion of the forest harvesting in the cooler regions of North America occurs in the winter when the ground is frozen and can support machine traffic. Climate change may influence the cost of forestry operations by reducing the period of winter access in those cold regions. In this study, we examined the impact of a shortened period of frozen ground conditions on logging operation and costs. To adapt to shorter period of frozen soil conditions, logging contractors might need to provide more machines and labor to complete logging in a shorter period of frozen conditions. The objectives were to calculate the costs of logging operations of a hypothetical forestry company in Alberta, Canada under two conditions: first, when the wood was hauled to the mill directly; and second, when part of the wood was hauled to satellite yards close to the logging area, thereby minimizing the annual number of idle hauling trucks. General Circulation Models were used to predict future winter weather conditions. Using the current type of harvesting machines and hauling directly to the mill, the unit cost of logging operations ($/m3) was projected to increase by an average of 1.6% to 2.5% in 2030s, 2.8% to 5.3% in the 2050s and 4.8% to 10.9% in the 2080s compared to the base year of 2015&#8722;2016. With use of satellite yards during the winter logging, the total logging cost will increase over direct haul, by 1.8% to 2.8% in the 2030s, 3.1% to 5.7% in the 2050s and 5.2% to 11.4% in the 2080s. Using satellite yards, however, will provide year-around employment for hauling truckers and more consistent and reliable hauling operations
