Das Privatkundengeschäft der deutschen Banken sieht sich auf dem Feld des
Vertriebskanalmanagements derzeit großen
Herausforderungen gegenüber. Die vorliegende Untersuchung stellt diese Herausforderungen und mögliche Lösungsansätze auf Basis bestehender Studien und einer eigenen Expertenbefragung dar.
Zunächst strukturiert die Untersuchung die externen Rahmenbedingungen des
Vertriebskanalmanagements, die in den
Bereichen Wettbewerbsumfeld, technologisches Umfeld und Kundenumfeld liegen. Dann zeigt sie Entwicklungstendenzen in den verschiedenen Vertriebskanälen auf.
Unterschieden werden dabei stationärer, mobiler und technischer Vertrieb. Anschließend wird ein Ansatz zur systematischen Optimierung des Vertriebskanalmanagements dargestellt. Dieser gliedert sich in die Aspekte Kanalarchitektur, Informationsmanagement, Steuerung der Kunden sowie Steuerung der Mitarbeiter

von Innovationen erkannt haben und jährlich hohe Summen in ihre Entwicklung und Vermarktung investieren, zeigen hohe
„Flopraten“, dass insbesondere hinsichtlich
der Markteinführung von Neuprodukten
noch Optimierungspotential besteht.
Vor diesem Hintergrund beschäftigt sich
der Market-Launch-Excellence-Ansatz mit
wesentlichen Ansatzpunkten zur Professionalisierung des Markteinführungsmanagements. Anhand der vier Dimensionen Timing, Markt und Zielgruppe,
Marketing-Mix und interne Rahmenbedingungen
werden zahlreiche konkrete Maßnahmen beleuch-
tet. Darüber hinaus beschäftigt sich
der vorliegende Beitrag mit dem neuen Produkt,
das Gegenstand der Markteinführung ist. Der Market-Launch-Excellence-Ansatz bietet zum einen die Möglichkeit, die Qualität des eigenen Markteinführungsmanagements zu bewerten, zum anderen kann er als Orientierungsrahmen zur systematischen Optimierung von Markteinführungsprozessen dienen
Familien mit einem schizophren erkrankten Elternteil: Ergebnisse einer fallrekonstruktiven Familienstudie
"Fragestellung: In diesem Beitrag werden Ergebnisse einer fallrekonstruktiven Studie vorgestellt, bei der Familien mit einem schizophren erkrankten Elternteil befragt wurden. Dabei sollte untersucht wurden, wie sich die Schizophrenie auf die Familienmitglieder, ihren Alltag und ihre Beziehungen auswirkt. Methodik: Die Auswertung erfolgte sowohl fall- als auch themenbezogen, wobei inhaltsanalytische und fallrekonstruktive Verfahren eingesetzt wurden. Ergebnisse: Trotz der Vielfalt der familiären Konstellationen und Belastungslagen zeigte sich eine Reihe charakteristischer Muster. Kinder zu haben bedeutet für erkrankte Eltern, sowohl Ressourcen als auch Belastungen zu haben. Paar- und Familienbeziehungen sind oft stark beeinträchtigt und weisen ein hohes Risiko für Beziehungsabbrüche auf. Zugleich wird im Familienalltag eine Normalität jenseits der Erkrankung angestrebt und erlebt. Normalisierungs- und Vermeidungsstrategien können dazu beitragen, dass die Erkrankung zu einem Tabuthema wird. Viele Kinder sind daher unzureichend über die elterliche Schizophrenie informiert. Sie sind in dieser Situation oft überfordert und entwickeln ihrerseits Verhaltensauffälligkeiten, Ängste und Depressionen. Diskussion: Die Schizophrenie kann insofern als 'Familienerkrankung' gedeutet werden, als sie das gesamte Familiensystem beeinflusst, belastet und gefährdet. Aus diesem Grund sollten verstärkt familienorientierte Präventionsangebote bereitgestellt werden, wobei Gesundheitswesen und Jugendhilfe eng miteinander kooperieren sollten." (Autorenreferat)"Objective: This study aims at investigating the impact of a parental schizophrenia on the family members, their everyday life and their relations. For this purpose, we conduct qualitative interviews with mothers and fathers suffering from schizophrenia, their spouses and children. Methods: Interview data is analyzed using case-reconstructive as well as content analysis methods. Results: Although results illustrate a great variety of family constellations and burdening circumstances, there are a number of typical patterns: Having children is perceived by affected parents in an ambiguous manner, i.e. as a resource as well as a distress. Relationships of couples and families are often impaired, resulting in a high risk of abandonment of relationships. At the same time, family members strive for normality in everyday life. Normalisation and avoidance strategies can bring about that the schizophrenia becomes a taboo issue within the family. Thus, with regard to their parent's illness, many of the children are insufficiently informed. Often, the children are overstrained by this situation and, in turn, may develop behaviour disorders, anxiety, or depression. Discussion: In sum, schizophrenia can be considered as a 'family disease' as it strongly affects the whole family system. Hence, it is necessary to provide preventive help offers for affected parents, their spouses and children. For delivering support, youth welfare and public health services should cooperate closely." Cytoplasmic versus nuclear THR alpha expression determines survival of ovarian cancer patients
PURPOSE Thyroid hormone receptors (THR) have manifold functions and are involved in the carcinogenesis of several tumor types. Within this study, we aimed to investigate the expression pattern (nuclear versus cytoplasmic) of the THR alpha and its impact on patients survival in ovarian cancer (OvCa).
METHODS The presence of the thyroid hormone receptors THR\textgreeka, THR\textgreeka1 and -~2 was investigated in 156 ovarian cancer samples using immunohistochemistry (IHC) using semi-quantitative immunoreactivity (IR) scores and correlated with clinical, pathological data, subtype of ovarian cancer, clinical data, staining of 20 already described OvCa marker proteins and overall survival (OS).
RESULTS Among all subtypes of OvCa, clear cell carcinomas showed the highest THR\textgreeka expression. Furthermore, nuclear THR\textgreeka was associated with a reduced survival in this subtype. However, nuclear expressed THR\textgreeka1 turned out to be a positive prognosticator for all subtypes of OvCa patients. Nuclear THR\textgreeka2 is a positive prognosticator for OvCa patients of the serous subtype. In contrast, cytoplasmic expression THR\textgreeka2 was associated with a reduced OS in all subtypes of OvCa patients; while, cytoplasmic expression of THR\textgreeka1 is associated with reduced OS in mucinous OvCa patients only. In addition, THR\textgreeka expression correlates with gonadotropin receptors, steroid hormone receptors, TA-MUC1 and glycodelin.
CONCLUSION Depending on nuclear or cytoplasmic expression, our study shows that THR\textgreeka and its isoforms 1 and 2 provide different prognostic information for ovarian cancer patients. Further investigations should analyze if THRs may represent new endocrine targets for the treatment of ovarian cancer
The immunologic tumor microenvironment in endometrioid endometrial cancer in the morphomolecular context: mutual correlations and prognostic impact depending on molecular alterations
OBJECTIVE POLE-mutant, microsatellite-instable (MSI), p53-mutant and non-specific molecular profile (NSMP) are TCGA-defined molecular subgroups of endometrial cancer (EC). Hypothesizing that morphology and tumor immunology might differ depending on molecular background concerning composition and prognostic impact, we aimed to comprehensively interconnect morphologic, immunologic and molecular data. METHODS TCGA-defined molecular groups were determined by immunohistochemistry and sequencing in n = 142 endometrioid EC. WHO-defined histopathological grading was performed. The immunologic microenvironment (iTME) was characterised by the quantification of intraepithelial and stromal populations of tumor-infiltrating lymphocytes (TIL: overall T-cells; T-Killer cells; regulatory T-cells (Treg)). Immunologic parameters were correlated with WHO-grading, TCGA-subgroups and prognosis. RESULTS High density TIL were significantly more frequent in high-grade (G3) compared to low-grade (G1/2) EC in the whole cohort and in the subgroup of POLE-wildtype-/microsatellite-stable-EC. MSI was associated with high-level TIL-infiltration when taking into account the type of mismatch repair defect (MLH1/PMS2; MSH2/MSH6). Prognostic impact of biomarkers depended on molecular subgroups: In p53-mutant EC, Treg were independently prognostic, in NSMP, the unique independently prognostic biomarker was WHO-grading. CONCLUSIONS EC morphology and immunology differ depending on genetics. Our study delineated two molecularly distinct subgroups of immunogenic EC characterized by high-density TIL-infiltration: MSI EC and high-grade POLE-wildtype/microsatellite-stable-EC. Prognostic impact of TIL-populations relied on TCGA-subgroups indicating specific roles for TIL depending on molecular background. In NSMP, histopathological grading was the only prognostic biomarker demonstrating the relevance of WHO-grading in an era of molecular subtyping
SARS-CoV-2-Antikörperprävalenz bei medizinischem Fachpersonal -- Eine prospektive, serologische Kohortenstudie mit vier Testzeitpunkten in Thüringen 2020 bis 2021 (COVID-PraxImmun)
Im Verlauf der COVID-19-Pandemie zeigte sich schnell, dass für medizinisches Fachpersonal im Vergleich zur Allgemeinbevölkerung ein erhöhtes Risiko für SARS-CoV-2-Infektionen besteht. Um den SARS-CoV-2-Immunstatus bei medizinischem Personal in Thüringer Vertragsarztpraxen systematisch und flächendeckend zu untersuchen, führte die Kassenärztliche Vereinigung Thüringen eine Kohortenstudie durch, an der 3.658 ärztlich, psychologisch und zahnärztlich tätige Personen bzw. nicht ärztlich Tätige mit Patientenkontakt teilnahmen. Die SARS-CoV-2-Antikörperprävalenzen und deren Verlauf zwischen August 2020 und September 2021 werden im Epidemiologischen Bulletin 45/2022 dargestellt
Establishing a Mental Health Surveillance in Germany: Development of a framework concept and indicator set
In the course of the recognition of mental health as an essential component of population health, the Robert Koch Institute has begun developing a Mental Health Surveillance (MHS) system for Germany. MHS aims to continuously report data for relevant mental health indicators, thus creating a basis for evidence-based planning and evaluation of public health measures. In order to develop a set of indicators for the adult population, potential indicators were identified through a systematic literature review and selected in a consensus process by international and national experts and stakeholders. The final set comprises 60 indicators which, together, represent a multidimensional public health framework for mental health across four fields of action. For the fifth field of action ‘Mental health promotion and prevention’ indicators still need to be developed. The methodology piloted proved to be practicable. Strengths and limitations will be discussed regarding the search and definition of indicators, the scope of the indicator set as well as the participatory decision-making process. Next steps in setting up the MHS will be the operationalisation of the single indicators and their extension to also cover children and adolescents. Given assured data availability, the MHS will contribute to broadening our knowledge on population mental health, supporting a targeted promotion of mental health and reducing the disease burden in persons with mental disorders

Tumoral PD-L1 expression defines a subgroup of poor-prognosis vulvar carcinomas with non-viral etiology
Vulvar cancer is rare but incidence rates are increasing due to an aging population and higher frequencies of young women being affected. In locally advanced, metastatic or recurrent disease prognosis is poor and new treatment modalities are needed. Immune checkpoint blockade of the PD-1/PD-L1 pathway is one of the most important advancements in cancer therapy in the last years. The clinical relevance of PD-L1 expression in vulvar cancer, however, has not been studied so far. We determined PD-L1 expression, numbers of CD3+ T cells, CD20+ B cells, CD68+ monocytes/macrophages, Foxp3+ regulatory T cells and CD163+ tumor-associated macrophages by immunohistochemistry in 103 patients. Correlation analysis with clinicopathological parameters was undertaken; the cause-specific outcome was modeled with competing risk analysis; multivariate Cox regression was used to determine independent predictors of survival. Membranous PD-L1 was expressed in a minority of tumors, defined by HPV-negativity. Its presence geographically correlated with immunocyte-rich regions of cancer islets and was an independent prognostic factor for poor outcome. Our data support the notion that vulvar cancer is an immunomodulatory tumor that harnesses the PD-1/PD-L1 pathway to induce tolerance. Accordingly, immunotherapeutic approaches might have the potential to improve outcome in patients with vulvar cancer and could complement conventional cancer treatment
Aufbau einer Mental Health Surveillance in Deutschland: Entwicklung von Rahmenkonzept und Indikatorenset
Im Zuge der Anerkennung psychischer Gesundheit als wesentlicher Bestandteil der Bevölkerungsgesundheit wird am Robert Koch-Institut eine Mental Health Surveillance (MHS) für Deutschland aufgebaut. Ziel der MHS ist die kontinuierliche Berichterstattung relevanter Kennwerte zum Zweck einer evidenzbasierten Planung und Evaluation von Public-Health- Maßnahmen. Zur Entwicklung eines Indikatorensets für die erwachsene Bevölkerung wurden potenzielle Indikatoren durch eine systematische Literaturrecherche identifiziert und in einem strukturierten Konsentierungsprozess durch internationale und nationale Expertinnen und Experten beziehungsweise Stakeholder ausgewählt. Das finale Set enthält 60 Indikatoren, die ein mehrdimensionales Public-Health-Rahmenkonzept psychischer Gesundheit in vier Handlungsfeldern abbilden. Im fünften Handlungsfeld „Gesundheitsförderung und Prävention“ besteht Bedarf der Indikatorenentwicklung. Die pilotierte Methodik erwies sich als praktikabel. Diskutiert werden Stärken und Limitationen der Recherche und Definition von Indikatoren, der Umfang des Indikatorensets sowie die partizipative Entscheidungsfindung. Nächste Schritte zur Etablierung der MHS bestehen in der Operationalisierung von Indikatoren und der Erweiterung unter anderem auf das Kindes- und Jugendalter. Bei gesicherter Datenverfügbarkeit wird die MHS zum Wissen über die Gesundheit der Bevölkerung beitragen und die gezielte Förderung psychischer Gesundheit sowie die Verringerung der Krankheitslast psychischer Störungen unterstützen
Synaptotagmin IV determines the linear Ca2+ dependence of vesicle fusion at auditory ribbon synapses
Mammalian cochlear inner hair cells (IHCs) are specialized for the dynamic coding of continuous and finely graded sound signals. This ability is largely conferred by the linear Ca2+ dependence of neurotransmitter release at their synapses, which is also a feature of visual and olfactory systems. The prevailing hypothesis is that linearity in IHCs occurs through a developmental change in the Ca2+ sensitivity of synaptic vesicle fusion from the nonlinear (high order) Ca2+ dependence of immature spiking cells. However, the nature of the Ca2+ sensor(s) of vesicle fusion at hair cell synapses is unknown. We found that synaptotagmin IV was essential for establishing the linear exocytotic Ca2+ dependence in adult rodent IHCs and immature outer hair cells. Moreover, the expression of the hitherto undetected synaptotagmins I and II correlated with a high-order Ca2+ dependence in IHCs. We propose that the differential expression of synaptotagmins determines the characteristic Ca2+ sensitivity of vesicle fusion at hair cell synapses
