Surgical treatment in our hydatid cyst patients and efficiency of sphincterotomy with ERCP
Giriş: Karaciğerin Kist hidatik hastalığı, ülkemizin
de aralarında bulunduğu özellikle koyun yetiştiriciliğin
yaygın olarak yapıldığı ülkelerde endemik olarak görülen
bir hastalıktır. Kistler, %50-70 oranında karaciğere ve sıklıkla
sağ loba yerleşir ve genellikle tekdir. Cerrahi olarak
tedavi edilen olgularda safra fistülü gelişme olasılığı
%2.6-28.6’dır. Kistin safra yolları ile ilişkisini belirlemede
ERCP altın standart olarak kabul edilmekte ve aynı
zamanda tedavi amacıyla kullanılmaktadır. Çalışmamızda,
kist hidatik olgularımızda ERCP ile sfinkterotominin etkinliğinin
irdelenmesi amaçlanmıştır.
Gereç ve Yöntem: Afyon Kocatepe Üniversitesi Tıp Fakültesi
Genel Cerrahi Anabilim Dalında 2001-2005 yılları
arasında ameliyat edilen 30 kist hidatik olgusu retrospektif
olarak incelendi. Yaş, cinsiyet, ultrasonografi ve tomografi
bulguları, cerrahi yöntem ve postoperatif gelişen komplikasyonlar
veri olarak alınmış, safra fistülü gelişen ve
ERCP ile tedavi edilen olgular ayrıca incelenmiştir.
Bulgular: Olguların 8’i erkek 22’si bayandı. Hastaların
yaş ortalaması 45.8 yıl idi. Başvuru sırasında hastaların birinde
sepsis, birinde akut batın, birinde mekanik ikter,
20’sinde künt sağ üst kadran ağrısı, 7 hastada ise semptom
ve bulgu saptanmadı. Kistlerin 16’sı karaciğer sağ lobda,
7’si sol lobda, 4’ü her iki lobda yerleşim göstermekteydi.
Karaciğerdeki kiste ek olarak hastaların 3’ünde akciğer,
1’inde miyokardial, ve birinde dalak yerleşim saptandı.
Kolona ve safra kesesine fistülize iki olgumuz mevcuttu.
Hastaların tamamı açık cerrahi yöntemle tedavi edildi.
Ameliyat sonrası gözlemlerimizde hastaların 6’sında safra
fistülü tespit edilmesi üzerine ERCP ile sfinkterotomi iş-
lemi yapıldı.
Sonuç: Klinik serimizde kist hidatik hastalığında, kistin
safra yollarına açılma oranını %20 olarak tespit ettik.
Postoperatif dönemde safra fistülü tanısı konan hastalarda
ERCP ile sfinkterotomi bir tedavi modalitesi olarak göz
önünde bulundurulmalıdır.Background: Liver hydatid cyst disease is
an endemic disease in countries where sheep breeding is
common, including our country. Cysts locate to the liver,
particularly the right lobe, with a rate of 50-70%, and
usually are solitary. The frequency of bile fistule
formation in patients treated surgically is 2.6-28.6%.
Endoscopic retrograde cholangiopancreatography (ERCP)
is the gold standard in evaluating the relation between the
cyst and the bile ducts. It is also used for treatment. In this
study, we evaluated the efficiency of sphincterotomy with
ERCP in hydatid cyst patients.
Methods: Thirty hydatid cyst patients who were operated
in Afyon Kocatepe University School of Medicine
Department of General Surgery between 2001-2005 were
retrospectively evaluated. Data included age, gender,
ultrasound and tomography scan findings, surgical
procedure and postoperative complications. Patients who
developed bile fistula and treated with ERCP were
additionally evaluated.
Results: Of the patients 8 were male and 22 were female.
Mean age was 45.8. One patient presented with sepsis, 1
patient with acute abdomen, 1 patient with mechanical
icterus, 20 patients with right upper quadrant pain. Seven
patients were asymptomatic at initial presentation. Of the
cysts, 16 were located at the right lobe, 7 at the left lobe
and 4 located in both. In addition to liver cyst, 1 patient
had myocardial, 3 patients had pulmonary and 1 patient
had splenic cysts. There were 2 patients who developed
fistula to colon and bile duct. All the patients underwent
open surgery. Postoperatively, a bile fistula was developed
in 6 patients and sphincterotomy was performed with
ERCP.
Conclusion: We found that 20% of the liver hydatid cysts
develop a fistula to bile ducts. Sphincterotomy with ERCP
can be used as a treatment modality in patients with
postoperative bile fistula

Correlation of ultrasonography with the pathological diagnosis in gallbladder polyps
Ultrasonografı (USG), safra kesesi ve safra yollan patolojilerini değerlendirmede yaygın olarak kullanılan non-invazif bir görüntüleme yöntemidir. USG görüntülemenin klinik kullanıma girmesi ile safra kesesi poliplerine daha sık rastlanmaktadır. Küçük kolesterol polipleri parlak ve ekojenik görülebildiğinden, kese duvarına yapışık, impakte ve gölge bırakmayan taşlar kolesterol polipi olarak yorumlanabilmektedir. Çalışmamızda, polipoid oluşumların tespitinde USG görüntülemenin etkinliğini ve tedavi seçeneklerini tartışmayı amaçladık.
Gereç ve Yöntem: Ocak 2001- Şubat 2006 tarihleri arasında kiniğimizde kolesistektomi yapılan hastalar çalışmaya alındı. Çalışmamızda preoperatif USG veya postoperatif histopatoloji ile polip tanısı konan 26 hastanın dosyaları retrospektif olarak tarandı. Hastaların; yaş, cins, klinik bulguları, polip çapları ve sayıları, ameliyat türü, USG ve patoloji inceleme bulguları çalışma kapsamına alındı. USG incelemede ameliyat endikasyonu olarak; semptomatik polipler, > 10 mm polipler, < 10 mm + risk faktörleri varlığı kriter olarak alındı. Çalışmamızda; kolesterol polipleri, inflamatuar polipleri, hiperplastik polipleri, lenfoid polipleri, fıbröz polipleri, kolesterolozis (çilek kese) ve adenomlar polipoid lezyon olarak değerlendirildi. Bulgular: Hastaların ortalama yaşı Al fi yıl (en küçük 22, en büyük 76) ve kadın erkek oranı 14/12 olarak belirlendi. Kliniğimizde 5 yıllık sürede çeşitli nedenlerle toplam 554 hastaya kolesistektomi yapıldı. Piyeslerin histopatolojik incelemesinde hastaların 26 (%4,6)’smda polipoid lezyon saptandı ve polip çaplan 3-14 mm arasında değişmekte idi. Hastalann 22 (%3,9)’si, USG incelemede polipoid oluşum tespit edilmesi üzerine ameliyat edildi. USG ile polipoid lezyon tanısı konup ameliyat edilen 6 (%27) hastanın patolojisinde polip görülmedi. USG ile tespit edilmeyen 4 (%15,3) hastada patolojik incelemede polip tespit edildi. Çalışmamızda vaka sayısı az olmakla beraber, USG ile polip tanısı koymadaki duyarlılık %80 ve yalancı pozitiflik oranı %27 olarak tespit edildi. USG inceleme her ne kadar non-invaziv bir yöntem olsa da, kesin olmayan polipoid lezyonlann ayırıcı tanısında diğer radyolojik görüntüleme yöntemlerine zaman zaman gerek duyulabilir.Objectives: Ultrasonography (USG) is a widely used non-invasive method for the evaluation of gallbladder and biliary tree pathology. The frequency of gallbladder polyps increased after the clinical use of USG. Because of the brightness and echogenic image of small cholesterol polyps, non-shadow stones that are impact and stuck to the gallblader wall can be considered as polyps. In this study, we aimed to evaluate the postoperative pathological diagnosis of gallbladder polyps which were diagnosed radiologically at preoperative stage.
Materials and Methods: We evaluated the
cholecystectomy patients between January 2001 and February 2006, retrospectively. There were 26 patients in our study who were diagnosed pathologically or with USG as having polyps. The age, gender, and clinical findings of patients, number and diameter of polyps, operation types, USG and pathological investigation findings were evaluated. Symptomatic polyps, polyps greater than 10 mm, and polyps smaller than 10 mm and has risk factors were considered as the criterion for operation indication. In our study, cholesterol polyps, inflammatory polyps, hyperplastic polyps, lymphoid polyps, fibrous polyps, cholesterolosis (strawberry gallbladder) and adenomas were evaluated as polypoid lesions.
Results: The mean age of the patients was 47,7 (between 22 and 76) and the women/men ratio was 14/12. Cholecystectomy was performed to 554 patients in 5 years due to different diagnoses. Histopathological studies showed polypoid lesions in 26 patients (4.6%). The diameter of polyps were between 3 and 14 mm. Twenty two patients (3.9%) were operated after the diagosis of polypoid lesion in USG. The histopathological studies of 6 patients (27%) did not show polypoid lesion which was diagnosed as polypoid lesion with USG. The histopathological studies of 4 patients (15.3%) showed polypoid lesion which were not diagnosed with USG. Conclusion: This study showed that the sensitivity of USG for the diagnosis of polypoid lesion was 80% and false positivity ratio was 27%. Although USG is a non-invasive method, further radiological methods are necessary for the diagnosis of polypoid lesions
