    Hip Arthroscopic Surgery for Femoroacetabular Impingement: A Prospective Analysis of the Relationship Between Surgeon Experience and Patient Outcomes.

    Background:Hip arthroscopic surgery is a rapidly growing procedure, but it may be associated with a steep learning curve. Few studies have used patient-reported outcome (PRO) surveys to investigate the relationship between surgeon experience and patient outcomes after the arthroscopic treatment of femoroacetabular impingement (FAI). Hypothesis:Patients undergoing hip arthroscopic surgery for the treatment of FAI in the early stages of a surgeon's career will have significantly worse outcomes and longer procedure times compared with patients treated after the surgeon has gained experience. Study Design:Cohort study; Level of evidence, 2. Methods:Patients undergoing hip arthroscopic surgery for FAI and labral injuries were prospectively enrolled during a sports medicine fellowship-trained surgeon's first 15 months of practice. Patients were stratified into an early group, consisting of the first 30 consecutive cases performed by the surgeon, and a late group, consisting of the second 30 consecutive cases. Radiographic and physical examinations were performed preoperatively and postoperatively. PRO surveys, including the 12-item Short Form Health Survey (SF-12), the modified Harris Hip Score (mHHS), and the Hip disability and Osteoarthritis Outcome Score (HOOS), were administered preoperatively and at a minimum of 1 year postoperatively. Results:There was no difference between the early and late groups for patient age (37.2 ± 11.5 vs 35.3 ± 10.8 years, respectively; P = .489), body mass index (25.6 ± 4.0 vs 25.1 ± 4.5 kg/m2, respectively; P = .615), or sex (P = .465). There was a significantly increased procedure time (119.3 ± 21.0 vs 99.0 ± 28.6 minutes, respectively; P = .002) and traction time (72.7 ± 21.4 vs 59.0 ± 16.7 minutes, respectively; P = .007) in the early group compared with the late group. Mean postoperative PRO scores significantly improved in both groups compared with preoperative values for all surveys except for the SF-12 mental component summary. No differences were found in PRO score improvements or complication rates between the early and late groups. Conclusion:The total procedure time and traction time decrease after a surgeon's first 30 hip arthroscopic surgery cases for FAI and labral tears, but patient outcomes can similarly improve regardless of surgeon experience in the early part of his or her career

    Can smartwatches replace smartphones for posture tracking?

    This paper introduces a human posture tracking platform to identify the human postures of sitting, standing or lying down, based on a smartwatch. This work develops such a system as a proof-of-concept study to investigate a smartwatch's ability to be used in future remote health monitoring systems and applications. This work validates the smartwatches' ability to track the posture of users accurately in a laboratory setting while reducing the sampling rate to potentially improve battery life, the first steps in verifying that such a system would work in future clinical settings. The algorithm developed classifies the transitions between three posture states of sitting, standing and lying down, by identifying these transition movements, as well as other movements that might be mistaken for these transitions. The system is trained and developed on a Samsung Galaxy Gear smartwatch, and the algorithm was validated through a leave-one-subject-out cross-validation of 20 subjects. The system can identify the appropriate transitions at only 10 Hz with an F-score of 0.930, indicating its ability to effectively replace smart phones, if needed

    Gestión por procesos y su influencia en la administración documentaria de una constructora de Trujillo

    El presente proyecto de investigación tiene como objetivo determinar la influencia de la Gestión por procesos en la administración documentaria en la empresa ASOLFED Contratistas Generales S.A.C. la metodología a seguir para la implementación fue a través de la muestra por conveniencia, aplicada a 5 trabajadores del área Administrativa y al Gerente General. Entre los principales resultados asociados a la variable dependiente, la conservación de los documentos ha aumentado en un 121.52% con respecto al resultado inicial, asimismo, la reproducción de información para las consultas aumentó en un 226.47%, el tiempo de acceso a la información se redujo en un 98.07%, la optimización en el uso de papel se redujo en 50.96% y en la satisfacción de los colaboradores frente al proceso de la administración documentaria aumentó en 100%. En la variable independiente, el nivel de satisfacción de los colaboradores aumentó en 100%, la eficacia en relación al cumplimiento aumentó en 75%, el porcentaje de cumplimiento de las capacitaciones se cumple en un 100% y el porcentaje de procesos administrativos caracterizados aumento en un 100%. Finalmente, se concluye que, al analizar económica y financieramente, el proyecto genera un VAN de S/ 1,531.33, un TIR de 13.88% y un Beneficio costo de 2.14%

    If You Build It, Will They Come? Challenges in E-learning Delivery System Choice

    This study incorporates principles derived from ‘mindful technology adoption’, instructional design, technology acceptance models, and target learner needs assessments in order to determine whether or not e-learning technologies would be used by multi-organizational health administrators in their re-accreditation processes. It was initially hypothesized that these nontraditional, geographically dispersed, computer literate learners would be likely to use the elearning technologies, due to course flexibility, their full time work status, age and other variables. Results indicate that the sample had lower than expected intentions to use online learning resources, predictions that they would to use online learning resources, and plans to use an online learning system. The design of this study and its results are a case study that lends support for the need for mindful adoption of e-learning technologies. A needs assessment addressing the likeliness of technology acceptance is a critical first step in designing e-learning delivery systems, in order to ensure that e-learning technology investments are worthwhile

    Ice-fuelled food webs in the Polar Oceans

    The Arctic Ocean is undergoing substantial warming and sea ice loss which are likely to cause changes in primary production, export fluxes and productivity of the Arctic marine ecosystems. Yet, we lack sufficient information about the ecosystem productivity, organic carbon cycling and cryo-pelago-benthic coupling processes in the Arctic Ocean. Particularly such information is scarce for deep Arctic basins and the spring to summer transition time. Our aim was to provide carbon budget estimations under different regimes and bloom development stages in the region north of Svalbard. We conducted ecological and biogeochemical early spring process studies in sea ice covered areas, from the shelf to the basins of the European Arctic margin and on the Yermak Plateau during the TRANSSIZ cruise PS 92 on the ice breaker R/V Polarstern in May/June 2015. We identified the potential characteristics of carbon production of primary producers in the sea ice and water column, and secondary production of zooplankton and benthos. We looked into the organic carbon production fate and export, including respiration and burial, as well as identified similarities and differences in ecosystem functioning along topography- , sea ice- and water mass-related gradients.     Mentors' self‐assessed competence in mentoring nursing students in clinical practice: A systematic review of quantitative studies

    Aims and objectives: To examine registered nurses' self‐evaluation of their competence in mentoring nursing students in clinical practice. Background: Clinical mentors have significant roles and responsibility for nursing students' clinical learning. Moreover, the mentors' role is becoming increasingly important internationally, as the role of nurse teachers in mentoring students in clinical practice has declined. However, in most EU countries there are no specific educational requirements for clinical mentors, although they need targeted education to increase their competence in mentoring nursing students. Design: The systematic review of quantitative studies was designed according to guidelines of the Centre for Reviews and Dissemination and PRISMA protocol. Methods: Studies published during 2000–2019 that met inclusion criteria formulated in PiCOS format were systematically reviewed by three independent reviewers. CINAHL (Ebsco), PubMed (MEDLINE), Scopus, ERIC and Medic databases were used to retrieve the studies. Three independent reviewers conducted the systematic review process. The studies were tabulated, thematically compared and narratively reported. Results: In total, 16 peer‐reviewed studies met the inclusion criteria. The studies identified various dimensions of mentors´ competence and associated environmental factors. Generally, participating mentors rated competences related to the clinical environment, mentoring, supporting students' learning processes and relevant personal characteristics fairly high. They also rated organisational practices in their workplaces, resources in the clinical environment and their mentor–student and mentor–stakeholder pedagogical practices, as respectable or satisfactory. Conclusion: The results indicate considerable scope for improving mentors' competence, particularly through enhancing organisational mentoring practices and relevant resources in clinical environments. Relevance for clinical practice: Pedagogical practices of mentors in relations with both students and stakeholders should be enhanced to improve future nurses' learning.
