    Toward a Northern Irish Pastoral: Reading the Rural in Seamus Heaney and Paul Muldoon

    The goal of this dissertation is three-fold: to mount a comparison of Seamus Heaney and Paul Muldoon, arguing that the two poets actually share much in common, particularly in their use of the pastoral mode; to argue that the pastoral mode offers a provocative, even radical platform for postcolonial writing and thinking; and to argue that reading Heaney and Muldoon, and Ireland in general, as postcolonial offers much for critics and scholars. This project looks particularly at Heaney’s use of gender in landscape to argue that Heaney relies on an abject pastoral mode, one which is dominated by excess fertility and dangerous maternity, to portray an Irish landscape that is both colonially scarred and potentially radically anti-colonial. These readings stretch from Heaney’s early pastoral poems to his famous sequence of bog poems and through to his much more recent eclogues, and in each instance this project seeks to bring a discussion of gender to the fore—thus making an intervention into the existing scholarship, which often overlooks or elides these concerns. The latter chapters focus on Paul Muldoon’s poems, arguing that Muldoon’s pastoral poems use the pastoral as a platform for writing about Irish cultural tropes and stereotypes. This project divides Muldoon’s pastoral poems into two groups: the first, his pastoral performances, are marked by his use of the pastoral as a setting from which to play with stock characters and commodified Irish culture in a way that both mocks and celebrates such tropes; the second, his “water poems,” use bodies of water as a form of landscape writing that enables Muldoon both to ground his identity to Northern Ireland and to take flight, embarking on transatlantic crossings from Northern Ireland to America to the Amazon and back again. In these readings of Muldoon, this project seeks to establish a reading of Muldoon that explores and celebrates his roots in the rural and the natural—a perspective that is often lost in Muldoon scholarship, which traditionally has been much more concerned with global, urban, and cosmopolitan concerns

    Intraductal approaches encompass procedures and technologies that are designed to access and interrogate the ductal–alveolar systems of the human breast, and include nipple aspiration, ductal lavage, random periareolar fine needle aspiration, and ductoscopy. These approaches are being used to collect and analyze fluids and cells to develop methods for breast cancer detection and risk assessment; to introduce imaging technologies to explore the mammary tree for abnormalities; to administer therapeutic and/or preventive agents directly to the breast tissue; and to explore the biology of the normal mammary gland. The latest research findings in these areas, presented at The 4th International Symposium on the Intraductal Approach to Breast Cancer in 2005, are summarized in this report

    Status of Intraductal Therapy for Ductal Carcinoma in Situ

    The intraductal approach is particularly appealing in the setting of ductal carcinoma in situ (DCIS), a preinvasive breast neoplasm that is thought to be entirely intraductal in its extent. Based on an emerging understanding of the anatomy of the ductal system as well as novel techniques to leverage the access accorded by the intraductal approach, researchers are actively exploring how ductal lavage, ductoscopy, and intraductal infusion of therapeutic agents may enhance breast cancer treatment. Both cytologic and molecular diagnostics continue to improve, and work is ongoing to identify the most effective diagnostic biomarkers for DCIS and cancer, although optimal targeting of the diseased duct remains an important consideration. Ductoscopy holds potential in detection of occult intraductal lesions, and ductoscopically guided lumpectomy could increase the likelihood of a more comprehensive surgical excision. Exciting pilot studies are in progress to determine the safety and feasibility of intraductal chemotherapy infusion. These studies are an important starting point for future investigations of intraductal ablative therapy for DCIS, because as our knowledge and techniques evolve, it is likely that DCIS may be the target most amenable to treatment by intraductal therapy. If such studies are successful, these approaches will allow an important and meaningful transformation in treatment options for women diagnosed with DCIS

    Gene promoter hypermethylation in ductal lavage fluid from healthy BRCA gene mutation carriers and mutation-negative controls

    INTRODUCTION: Female germline BRCA gene mutation carriers are at increased risk for developing breast cancer. The purpose of our study was to establish whether healthy BRCA mutation carriers demonstrate an increased frequency of aberrant gene promoter hypermethylation in ductal lavage (DL) fluid, compared with predictive genetic test negative controls, that might serve as a surrogate marker of BRCA1/2 mutation status and/or breast cancer risk. METHODS: The pattern of CpG island hypermethylation within the promoter region of a panel of four genes (RAR-β, HIN-1, Twist and Cyclin D2) was assessed by methylation-specific polymerase chain reaction using free DNA extracted from DL fluid. RESULTS: Fifty-one DL samples from 24 healthy women of known BRCA mutation status (7 BRCA1 mutation carriers, 12 BRCA2 mutation carriers and 5 controls) were available for methylation analysis. Eight of 19 (42.1%) BRCA mutation carriers were found to have at least one hypermethylated gene in the four-gene panel. Two BRCA mutation carriers, in whom aberrant methylation was found, also had duct epithelial cell atypia identified. No hypermethylation was found in DL samples from 5 negative controls(p = 0.13). CONCLUSION: We found substantial levels of aberrant methylation, with the use of a four-gene panel, in the fluid from the breasts of healthy BRCA mutation carriers compared with controls. Methylation analysis of free DNA in DL fluid may offer a useful surrogate marker for BRCA1/2 mutation status and/or breast cancer risk. Further studies are required for the evaluation of the specificity and predictive value of aberrant methylation in DL fluid for future breast cancer development in BRCA1/2 mutation carriers

    The intraductal approach to the breast: raison d'être

    Opportunities for the detection, prediction, and treatment of breast cancer exist at three biological levels: systemically via the blood, at the whole organ level, and within the individual ductal lobular structures of the breast. This review covers the evaluation of approaches targeted to the ductal lobular units, where breast cancer begins. Studies to date suggest the presence of 5 to 12 independent ductal lobular systems per breast, each harboring complex cellular fluids contributed by local and systemic processes. New techniques for accessing and interrogating these systems offer the potential to gauge the microenvironment of the breast and distill biological risk profiles
