    Widespread detection of highly pathogenic H5 influenza viruses in wild birds from the Pacific Flyway of the United States

    A novel highly pathogenic avian influenza virus belonging to the H5 clade 2.3.4.4 variant viruses was detected in North America in late 2014. Motivated by the identification of these viruses in domestic poultry in Canada, an intensive study was initiated to conduct highly pathogenic avian influenza surveillance in wild birds in the Pacific Flyway of the United States. A total of 4,729 hunter-harvested wild birds were sampled and highly pathogenic avian influenza virus was detected in 1.3% (n = 63). Three H5 clade 2.3.4.4 subtypes were isolated from wild birds, H5N2, H5N8, and H5N1, representing the wholly Eurasian lineage H5N8 and two novel reassortant viruses. Testing of 150 additional wild birds during avian morbidity and mortality investigations in Washington yielded 10 (6.7%) additional highly pathogenic avian influenza isolates (H5N8 = 3 and H5N2 = 7). The geographically widespread detection of these viruses in apparently healthy wild waterfowl suggest that the H5 clade 2.3.4.4 variant viruses may behave similarly in this taxonomic group whereby many waterfowl species are susceptible to infection but do not demonstrate obvious clinical disease.     Running against the clock: a qualitative study of internal medicine residents' work experience.

    While hospitals are adopting strategies designed to increase the overall efficiency of the healthcare system, physicians are facing expanding requirements. Such changes in work environment add new psychosocial and physical stressors. Building on a previous quantitative time-motion study, we conducted a qualitative study to better understand the work experience of internal medicine residents. The study used a qualitative description approach, and was based on focus group discussions with residents. Data were analysed using reflexive thematic analysis. The study was conducted among all residents of the Internal Medicine division of a tertiary university hospital in Switzerland. Time emerged as the major determinant of residents' daily experience, which residents want to waste on no account. Shifts are perceived as a constraining succession of distinct periods, with little room for adjustments. Moreover, residents feel held back and distracted in their progression toward the end of the shift. Under time pressure, some essential professional activities, such as caring for patients and families, dealing with medical complications and talking with consultants, may be experienced as unexpected undesirable bumps on the road. Residents describe "running through" a structured day, scattered with obstacles, and resorting to "tricks of the trade" in an attempt to influence the course of the shift. Time constraints are not new to medicine. However, our findings outline how time has become a constant preoccupation for internal medicine residents, permeating their daily work experience. This changing relationship with time carries the risk of undermining the foundations of clinical medicine and challenges the ability of hospitals to preserve the "sense of the profession".     The SARS-CoV-2 pandemic is putting our healthcare system under exceptional pressure, given the number of affected patients. In a context of limited human healthcare resources, senior medical students represent a valuable workforce that can quickly be mobilized for patient care. This is the approach followed in Switzerland and other countries, in several outpatient structures or inpatient services, including the Department of Internal Medicine, of the Lausanne University Hospital (CHUV). In this article, we first give the floor to students who responded to our call. We conclude with important considerations in terms of students' clinical supervision. It is reminded that the involvement of students in the care of COVID-19 patients should only occur on a vo luntary basis

    Managing innovation in non-R&D-intensive firms

    Innovations with little R&D, or without it, do not necessarily represent isolated cases; they just are different! Companies can compete innovatively on a global scale without incurring (high) R&D costs. Because the innovation processes of non-R&D-intensive companies are less formalised and more heavily customer driven, current solutions for innovation strategies based on R&D activities are not suitable. Thus, this chapter presents two case studies that focus on increasing innovativeness and competitive advantage by utilising two approaches: the organisation of internal innovation processes and the assessment of innovation cooperation. The aim of the selected case studies is to illustrate both approaches to processing the problems and issues identified in these companies as well as the individually generated solutions and organisational measures. Each case study is introduced more generally from a conceptual perspective and concludes with a reflection summarising the significant findings compiled during the course of the joint work phases. In particular, the conceptual perspective can provide guidance for the application of these approaches in different industrial contexts and can thus be adopted by other companies in the same form and sequence
