Skin microcirculation and echocardiographic and biochemical indices of left ventricular dysfunction in non-diabetic patients with heart failure
Background: We sought to noninvasively investigate skin microcirculation and to assess the
relation between parameters of microcirculation and echocardiographic and biochemical parameters
of left ventricular (LV) function in non-diabetic patients with heart failure (HF).
Methods and Results: We measured skin microcirculation with laser Doppler flowmetry
(LDF) at basal conditions (MFb), after warming to 44 degrees Centigrade (MF44) and after
occlusion (AUC, PF%). Blood was sampled for NT-proBNP. We obtained information on
patients’ medical history and medication status. The mean (SD) age of 100 patients (43 women)
was 68.2 ± 11.5 years. LV ejection fraction (LVEF) averaged 34.9 ± 13.3%, LV end-diastolic
diameter (LVEDD) 6.0 ± 0.9 cm, NT-proBNP 4,582.6 ± 6,339.7 pg/mL. The parameters of
microcirculation averaged: MFb 6.2 ± 4.7 perfusion units (PU), PF% 716.0 ± 437.8%, AUC
794 ± 706.1 PU/s, and MF44 77.9 ± 40.2 PU. NT-proBNP correlated negatively with LVEF
(p £ 0.0001) and positively with LVEDD (p = 0.003). MFb was positively correlated with
LVEF (r = 0.24, p = 0.03), and MF44 was negatively correlated with LVEDD (r = 0.22,
p = 0.02). The relations remained significant after adjustments for sex, age, and use of
medication. We observed no relation between NT-proBNP and microcirculatory derangement.
Conclusions: LDF-derived parameters of skin microcirculation are related to echocardiographic,
but not biochemical, indices of HF. (Cardiol J 2011; 18, 3: 270–276
Blood pressure, arterial stiffness and endogenous lithium clearance in relation to AGTR1 A1166C and AGTR2 G1675A gene polymorphisms
Introduction: Although recently a matter of epidemiologic controversy, sodium overload and its interaction with
genetic factors predispose to hypertension and related target organ complications.
Methods: In 131 (66 male) treated hypertensives, we measured peripheral and central arterial pressures and pulse wave
augmentation indexes (, , ), pulse wave velocity (PWV), daily urinary sodium excretion and did genetic
studies of AGTR1 A1166C and AGTR2 G1675A polymorphisms. Proximal () and distal () sodium reabsorption
measurements were performed using endogenous lithium clearance.
Results: In men, we found interaction between and AGTR2 G1675A polymorphism with respect to
(=0.01), (=0.05) and (=0.006). Arterial stiffness increased with higher sodium reabsorption in the
distal tubule, in the presence of AGTR2 G allele with the opposite tendency in A allele carriers. In the subgroup with
below median, as compared to those with above median, the (139.6±3.8 vs 159.1±5.7%; =0.009),
(26.3±1.8 vs 33.3±1.7%; =0.016) and (83.4±2.5 vs 96.5±2.6%; <0.0001) were lower, in the G allele carrying
men and GG homozygous women.
Conclusions: The relation between sodium reabsorption in the distal tubule and the development of arterial stiffness
depends on the AGTR2 G1675A polymorphism in blood pressure independent fashion
Coping with stress and hypertension-mediated organ damage
Aim of the study: Chronic exposure to high blood pressure may lead to the development of hypertensionmediated organ damage (HMOD). This study compares styles and strategies of coping with stress in hypertensive patients with arterial stiffness or left ventricular hypertrophy (LVH) and in individuals with hypertension, but without HMOD.
Material and methods: Each study participant (n=93) underwent the following procedures: clinical assessment, echocardiography, pulse wave velocity measurement and psychological testing. Blood pressure in the study group was measured using ABPM method. Carotid-femoral pulse wave velocity (PWV) was assessed to identify patients with arterial stiffness. Left ventricular mass index was measured to diagnose LVH. Each patient was also assessed using three psychometric tools: PSS-10, CISS and Brief COPE. Results: Subjects with arterial stiffness (increased PWV) scored significantly lower than patients with normal PWV in three scales: CISS Avoidance-oriented coping (median values: 39 vs. 41.5; p=0.042), Brief COPE Self-distraction (median values: 1.5 vs. 2; p=0.013) and Brief COPE Venting (median values: 1 vs. 1.5; p=0.037). Individuals with LVH had significantly lower results in Brief COPE Use of emotional support scale than hypertensive subjects with normal left ventricular mass index (median values: 1.5 vs. 2; p=0.041). Discussion: In our study group, hypertensive patients with HMOD preferred different coping styles and strategies than individuals with hypertension, but without vascular and cardiac damage. The mechanism underlying these differences is probably complex. Conclusions: HMOD may be associated with coping styles and strategies, but further research is necessary to fully understand the results of this study
Nitrendipine and nitroglicerine efficacy and safety in reducing unexpected high blood pressure in hypertensive outpatients
Wstęp Celem pracy było porównanie efektów działania
nitrendipiny (NTD) i nitrogliceryny (NTG) lub
połączenia obu tych leków u pacjentów nadciśnieniem
tętniczym z nagłym wzrostem wartości ciśnienia. Oceniano
skuteczność hipotensyjną oraz bezpieczeństwo.
Materiał i metody Do badania kwalifikowano dorosłe
osoby z nagłym wzrostem ciśnienia skurczowego
(SBP) do wartości 180-240 mm Hg i/lub rozkurczowego
(DBP) 110-130 mm Hg, którzy zgłosili się do
izby przyjęć. W doraźnym leczeniu nadciśnienia stosowano
preparat nitrendipiny oraz nitroglicerynę
w aerozolu lub ich kombinację. Pomiary ciśnienia
i tętna wykonywano początkowo po 2 i 5 minutach,
a następnie co 15 minut przez 3 godziny. W analizie
statystycznej wykorzystywano test χ2, analizę wariancji
z testem F Snedecora oraz test post-hoc Duncana.
Wyniki Objęto badaniem 83 chorych. Stwierdzono,
że oba badane leki NTG i NTD i ich kombinacja
powodowały istotny spadek zarówno SBP, jak i DBP.
W czasie leczenia NTG obserwowano nieco większy
wzrost częstości tętna na początku obserwacji.
Przy skojarzeniu NTG i NTD spadek ciśnienia był
wolniejszy i nie obserwowano wzrostu tętna. Nie zanotowano
poważnych działań niepożądanych w trakcie
obserwacji.
Wnioski Leczenie NTG, NTD lub kombinacją leków
w stanach pilnych jest skuteczne, bezpieczne
i dobrze tolerowane. Najsłabszy efekt występował po
NTG. Leczenie skojarzone NTG i NTD wiąże się z korzystniejszym działaniem hemodynamicznym, co
może być przydatne w leczeniu pacjentów z chorobą
wieńcową.Background The purpose of this work was to assess the hypotensive
effect of nitrendipine (NTD), nitroglicerine (NTG)
or combination of those two drugs in hypertensive patients
with unexpected increase of blood pressure. This work evaluated
hypotensive efficacy and safety of these drugs.
Material and methods Enrolled were adults admitted to Emergency
Department with unexpected increase of systolic blood
pressure (SBP) ranging 180-240 mm Hg and/or diastolic blood
pressure (DBP) ranging 110-130 mm Hg. Urgent treatment of
the hypertension with NTD, NTG or their combination were
started. At the beginning measurement of the blood pressure
and pulse rate were taken after 2 and 5 minutes and later after
each 15 minutes in 3 hour observation period. In statistical
analysis χ2 test, variation analysis with F Snedecor test and posthoc
Duncan test were used.
Results Eighty three patients were enrolled in the study. The
analysis confirmed that both drugs (NTG and NTD) and their
combination caused relevant decrease in SBP and DBP. An
increase in pulse rate at the beginning of the NTG treatment
was observed. In combined treatment with NTG and NTD
blood pressure decreased slowly and pulse rate did not increase.
There was not serious side-effects during the observation. Conclusions Treatment by NTG, NTD or combination of
these drugs in urgent hypertensive cases was effective, save
and well-tolerated. The weakest effect was after using NTG.
Combined treatment with NTG and NTD was more advantageous
with regard to hemodynamic effects what could be
helpful in treatment of the coronary ischemic disease patients

Przestrzeganie zaleceń terapeutycznych i współpraca z pacjentem: przegląd pojęć, narzędzi i czynników wpływających na skuteczność leczenia hipotensyjnego
W ostatnich dziesięcioleciach osiągnięto ogromne postępy w zrozumieniu epidemiologii, patofizjologii i ryzyka oraz związanego z nadciśnieniem tętniczym. Mimo to wyniki badań wskazują, że u mniej niż 50% pacjentów leczonych z powodu nadciśnienia terapia jest skuteczna. Jednocześnie zwiększa się liczba dowodów wskazujących, że za tę sytuacje w istotnym stopniu (obok inercji terapeutycznej lekarzy w stosowaniu najnowszych zaleceń i intensyfikacji leczenia) odpowiada nieprzestrzeganie przez pacjenta zaleceń. W artykule omówiono pojęcia najlepiej opisujące różne warianty współpracy terapeutycznej (w tym sugestię odejścia od wciąż od popularnego w Polsce terminu compliance), skale umożliwiające pomiar skuteczności tej współpracy oraz czynniki i strategie, które mogą zwiększyć efektywność wprowadzania uzgodnionych wspólnie z pacjentem interwencji terapeutycznych
