    LES FRACTURES DE L’OS ZYGOMATIQUE AU CHU SYLVANUS OLYMPIO DE LOME : A PROPOS DE 152 CAS.

    Background: fractures of the zygomatic complex are a fairly common phenomenon in the practice of maxillo-facial traumatology and are a major problem or therapeutic management due to possible functional and aesthetic complications. The purpose of this study was to report the different epidemiological, clinical and therapeutic aspects of fractured zygoma in Lomé. Methods: A 10-year retrospective audit was undertaken of all hospitalized patients, at the department of ENT and Cervico-maxillofacial surgery in Sylvanus Olympio teaching Hospital of Lomé (Togo), who had sustained a fractured zygoma from 2004 to 2013. Results: A total of 152 fractures were sustained and representing 5.4% of all the patients of the department. The average age of the patient was 34 years. The sex distribution is markedly higher for males than for females (6.6/1). Road traffic accidents (88.8%) were the main etiology and motorcycle accident (42.76%) were predominant. Tetrapod fractures (61.18%) were the most frequent type of fractures. In 16,44% of cases fracture of zygoma were not associated with another fracture. Cases were managed by either closed (50%) or open (48.90%) reduction. Percutaneous reduction described by Ginestet (60.43%) was the commonest technique employed for closed treatment and transosseous wiring (66.29%) was the most frequently employed fixation for open reduction Conclusion: fractures of the zygomatic bone are relatively frequent interesting young and male people, due to traffic accidents.     Introduction: Les pathologies chirurgicales de la cavité buccale sont multiples et variées tout comme le sont les techniques opératoires qui leur sont opposables pour leur prise en charge thérapeutique.Objectifs: La présente étude avait pour objectifs: - de déterminer le profil épidémiologique des pathologies chirurgicales de la cavité buccale et - d’étudier les différents aspects de la prise en charge thérapeutique des pathologies buccales chirurgicales au CHU-Campus de Lomé.Méthode: Il s’est agi d’une étude rétrospective descriptive des dossiers des patients ayant bénéficié d’un traitement chirurgical bucco-dentaire, durant l’année 2013, au CHUCampus de Lomé.Résultats: L’infection dentaire et ses complications avec 62,66 % des cas, étaient les pathologies les plus en causes d’un traitement chirurgical. Les extractions dentaires (86,43% des actes) étaient les plus fréquents traitements.Discussion. Les activités de chirurgie buccale étaient dominées par les avulsions dentaires, de pratique courante. Comme pour toute autre chirurgie, des complications hémorragiques, infectieuses ou inflammatoires peuvent accompagner ces actes opératoires, d’où la nécessité de renforcement des précautions visant à mieux sécuriser ces thérapeutiques chirurgicaux de la cavité buccaleConclusion: Le relèvement du niveau des équipements, le renforcement en personnel qualifié, ainsi que l’organisation d’un service des urgences chirurgicales, devraient permettre d’améliorer, les performances annuelles de prise en charge des pathologies chirurgicales de la cavité buccale au CHU-Campus.Mots clés: Chirurgie buccale, Pathologie buccale, Extraction dentaire, Bilan d’activités annuelleEnglish Title: One-year survey of oral surgery at the Lome CHU campusEnglish AbstractIntroduction: The surgical pathologies of the oral cavity are multiple and varied as are the operative techniques that are opposable to them for their therapeutic management.Objectives: The objectives of this study were: - to determine the epidemiological profile of surgical pathologies of the oral cavity and  - to study the various aspects of the therapeutic management of surgical oral pathologies at the CHU-Campus of Lomé.Method: This was a descriptive retrospective study of patients who had received oral surgical treatment during the year 2013, at the CHU-Campus Lomé.Results: The dental infection and its complications with 62.66% of the cases, were the most important causes of a surgical treatment. Dental extractions (86.43% of the acts) were the most frequent treatments.Discussion. Oral surgery activities were dominated by dental avulsions, a common practice. As with any other surgery, hemorrhagic, infectious or inflammatory complications may accompany these surgical procedures, hence the need to reinforce precautions to better secure these surgical treatments of the oral cavity.Conclusion: Upgrading the level of equipment, strengthening qualified personnel, as well as the organization of a surgical emergency department, should improve the annual performance of management of surgical diseases of the oral cavity at the CHU-Campus.Keywords: Oral surgery, Oral pathology, Dental extraction, Activity repor
