    Impact of helium neutral gas puff on plasma turbulence in linear magnetized argon plasmas

    In order to explore the impact of the neutrals on the plasma turbulence, a new gas puff system is developed for a linear magnetized plasma column PANTA. Helium gas is injected into the argon plasma without changing the electron density profile. After the helium gas puff, the dominant fluctuation mode changes from the broadband m = 2 drift wave to the coherent m = 1 mode, where m is the azimuthal mode number. Accordingly, the property of the nonlinear coupling with other fluctuating modes changes from broadband couplings to coherent couplings

    Hemorrhage from varices in hepaticojejunostomy in the fifth and tenth year after surgery for hepatic hilar bile duct cancer: a case report

    We report a case of a 64-year-old female patient who underwent a right lobectomy of the liver (including total resection of the caudate lobe), dissection of the group 2 lymph nodes, left hepaticojejunostomy (Roux-en-Y fashion), and reconstruction of the portal vein (end-to-end anastomosis between the main portal vein and the left portal branch) for treatment of hepatic hilar bile duct cancer in 1996. In 2001, the anastomotic site of the hepaticojejunostomy was dissected and re-anastomosed due to gastrointestinal bleeding caused by variceal rupture in the jejunal loop. In 2006, splenectomy was performed for recurrence of gastrointestinal bleeding due to another variceal rupture in the jejunal loop. Portal venography performed perioperatively showed a decrease in portal blood flow into the liver via the jejunal varices and an increase in portal blood flow into the liver via the left gastric vein. She had two jejunal variceal ruptures at five-year intervals after extrahepatic portal obstruction and underwent successful treatments

    How turbulence fronts induce plasma spin-up

    A calculation which describes the spin-up of toroidal plasmas by the radial propagation of turbulence fronts with broken parallel symmetry is presented. The associated flux of parallel momentum is calculated by using a two-scale direct-interaction approximation in the weak turbulence limit. We show that fluctuation momentum spreads faster than mean flow momentum. Specifically, the turbulent flux of wave momentum is stronger than the momentum pinch. The scattering of fluctuation momentum can induce edge-core coupling of toroidal flows, as observed in experiments

    Conversion of poloidal flows into toroidal flows by phase space structures in trapped ion resonance driven turbulence

    A theory to describe the conversion of poloidal momentum into toroidal momentum by phase space structures in trapped ion resonance driven turbulence is presented. In trapped ion resonance driven turbulence, phase space structures are expected to form and can contribute to transport by exerting dynamical friction. Toroidal momentum flux by dynamical friction is calculated. It is shown that dynamical friction exerted on trapped ion granulations can mediate momentum transfer between poloidal and toroidal flows. The conversion coefficient is calculated as measurable, which can be validated in current devices. © 2013 IOP Publishing Ltd

    Port site herniation of the small bowel following laparoscopy-assisted distal gastrectomy: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Port-site herniation is a rare but potentially dangerous complication after laparoscopic surgery. Closure of port sites, especially those measuring 10 mm or more, has been recommended to avoid such an event.</p> <p>Case presentation</p> <p>We herein report the only case of a port site hernia among a series 52 consecutive cases of laparoscopy-assisted distal gastrectomy (LADG) carried out by our unit between July 2002 and March 2007. In this case the small bowel herniated and incarcerated through the port site on day 4 after LADG despite closure of the fascia. Initial manifestations experienced by the patient, possibly due to obstruction, and including mild abdominal pain and nausea, occurred on the third day postoperatively. The definitive diagnosis was made on day 4 based on symptoms related to leakage from the duodenal stump, which was considered to have developed after severe obstruction of the bowel. Re-operation for reduction of the incarcerated bowel and tube duodenostomy with peritoneal drainage were required to manage this complication.</p> <p>Conclusion</p> <p>We present this case report and review of literature to discuss further regarding methods of fascial closure after laparoscopic surgery.</p

    Contemporary percutaneous treatment of unprotected left main coronary stenoses: initial results from a multicenter registry analysis 1994-1996.

    BACKGROUND: Coronary artery bypass surgery (CABG) has been considered the therapy of choice for patients with unprotected left main (ULMT) coronary stenoses. Selected single-center reports suggest that the results of percutaneous intervention may now approach those of CABG. METHODS AND RESULTS: To assess the results of percutaneous ULMT treatment from a wide variety of experienced interventional centers, we requested data on consecutive patients treated after January 1, 1994, from 25 centers. One hundred seven patients were identified who were treated either electively (n=91) or for acute myocardial infarction (n=16). Of patients treated electively, 25% were considered inoperable, and 27% were considered high risk for bypass surgery. Primary treatment included stents (50%), directional atherectomy (24%), and balloon angioplasty (20%). Follow-up was 98.8% complete at 15+/-8 months.
