    Thermal Fluctuations of the Electric Field in the Presence of Carrier Drift

    We consider a semiconductor in a non-equilibrium steady state, with a dc current. On top of the stationary carrier motion there are fluctuations. It is shown that the stationary motion of the carriers (i.e., their drift) can have a profound effect on the electromagnetic field fluctuations in the bulk of the sample as well as outside it, close to the surface (evanescent waves in the near field). The effect is particularly pronounced near the plasma frequency. This is because drift leads to a significant modification of the dispersion relation for the bulk and surface plasmons.Comment: Comments are welcom

    Management of Hypertension in the Elderly Patient at Abidjan Cardiology Institute (Ivory Coast)

    Background. Since the treatment of hypertension is beneficial for the elderly, we have undertaken this study that aims to evaluate the management of hypertension in elderly patient in Côte d'Ivoire. Methods. A retrospective study was conducted among 854 hypertensive elderly patients of Abidjan Cardiology Institute who were followed for a minimum of one year, between January 2000 and December 2009. Results. The patients mean age was 73.1 ± 5.3 years, and 59% were women. At the first presentation, it was mostly systolic-diastolic hypertension (51.8%) and isolated systolic hypertension (38.5%). Mean blood pressure was 169.4 ± 28.4 mmHg for systolic, 95.3 ± 15.7 mmHg for diastolic, and 74.1 ± 22.8 mmHg for pulse pressure. Pulse pressure was ≥60 mmHg in 80.4%. According to the European Guidelines stratification of the cardiovascular risk-excess attributable to high blood pressure, 82.1% of the sample had a very high added risk. The pharmacological therapy was prescribed in 93.5%. More than 66% of patients were receiving ≥2 antihypertensive drugs including fixed-dose combination drugs. The most common agents used were diuretics (63.5%) followed by angiotensin-converting enzyme inhibitors or angiotensin receptor blockers in 61.3%. The most common agents used for monotherapy were calcium antagonists. When ≥2 drugs were used, diuretics and angiotensin-converting enzyme inhibitors or angiotensin receptor blockers were the most common. Blood pressure control was achieved in 42.6%. Conclusion. The control of elderly hypertension can be effective in Sub-Saharan Africa. He required at least two antihypertensive drugs to meet the recommended blood pressure target

    Search for the Radiative Capture d+d->^4He+\gamma Reaction from the dd\mu Muonic Molecule State

    A search for the muon catalyzed fusion reaction dd --> ^4He +\gamma in the dd\mu muonic molecule was performed using the experimental \mu CF installation TRITON and NaI(Tl) detectors for \gamma-quanta. The high pressure target filled with deuterium at temperatures from 85 K to 800 K was exposed to the negative muon beam of the JINR phasotron to detect \gamma-quanta with energy 23.8 MeV. The first experimental estimation for the yield of the radiative deuteron capture from the dd\mu state J=1 was obtained at the level n_{\gamma}\leq 2\times 10^{-5} per one fusion.Comment: 9 pages, 3 Postscript figures, submitted to Phys.     External beam radiation therapy (EBRT) is currently an important component of the treatment of gastric cancer (GC). In accordance with international and domestic recommendations, it can be used in the postoperative period, before the operation and as a sole option, with a curative and palliative purpose, usually in combination with chemotherapy. With the transition of RT to a new technological level, which involves the use of conformal, intensively modulated regimens, and image guidance the revision of indications for and doses of EBRT is required. The article discusses modern approaches to the determination of irradiated volumes in various clinical scenarios of current radiation therapy for GC and strategies aimed at reducing the dose in critical organs at risk and increasing the overall effectiveness of treatment.Дистанционная лучевая терапия (ЛТ) в настоящее время является важным компонентом лечения рака желудка (РЖ). В соответствии с международными и отечественными рекомендациями она может быть использована в послеоперационном периоде, перед операцией и самостоятельно, с лечебной и паллиативной целью, как правило, в сочетании с химиотерапией. В связи с переходом ЛТ на новый технологический уровень, предусматривающий использование конформных, интенсивно-модулированных режимов, требуется пересмотр показаний, доз и режимов дистанционного облучения. В статье рассмотрены современные подходы к определению облучаемых объемов при различных клинических сценариях современной ЛТ по поводу РЖ и обсуждены стратегии, направленные на снижение дозы в критических органах и повышение эффективности лечения

    Where not too long ago we serviced athletes; nowadays, we have the responsibility to lead a multidisciplinary team that is mandated to protect the athletes' health, ensure competition availability and ability to compete at peak performance. In essence, our roles have shifted from being a service provider to a health and performance manager who steers a multidisciplinary team of specialists. In this viewpoint, we question whether we really have the skills to lead a 'team' like this effectively. We provide a potential definition of leadership and discuss the importance of leadership for health and performance teams with recent literature. Finally, we share our baseline 'leadership' pearls that allow you to elevate collaboration through leadership

    Objectifs : Préciser la prévalence, les facteurs de risque, les aspects électrocardiographiques, thérapeutiques et évolutifs des troubles de la conduction cardiaque chez les patients atteints d’un syndrome coronarien aigu avec sus-décalage du segment ST.Patients et méthode : Il s’agissait d’une étude rétrospective descriptive portant sur une période de 8 ans (Octobre 2010 à Octobre 2018) et  concernant 53 cas de troubles de la conduction cardiaque observés parmi 320 cas de syndrome coronariens avec susdécalage du segment ST.Résultats : La prévalence des troubles de la conduction cardiaque était de 16,5%. La moyenne d’âge de nos patients concernés était de 58 ans (extrêmes : 26 et 81 ans). L’hypertension artérielle était le facteur de risque prédominant (60,4 %). La localisation inférieure était la plus fréquente (53%) et les troubles de cardiaques étaient dominés par le bloc de branche droit (35,8%). La coronarographie a été réalisée chez tous nos patients ; les lésions retrouvées concernaient davantage l’artère circonflexe (32,1%) et l’artère inter ventriculaire antérieure (28,3%). L’angioplastie a été réalisée dans 66% des cas et la thrombolyse dans 9,4%. L’implantation d’un pace maker provisoire a été faite dans 32% des cas L’évolution était favorable chez tous nos patients.Conclusion : Les troubles de la conduction cardiaque au cours du syndrome coronarien avec sus-décalage du segment ST doivent être  diagnostiqués précocement et prises en charge immédiatement en raison du risque de mort subite. Mots clés : Hypertension artérielle, Bloc de branche droit, Syndrome coronarien aigu avec sus-décalage de ST.   English Title: Cardiac conduction disorders in acute coronary syndrome with st elevation: Study of 53 cases Introduction: Cardiac conduction disorders are quite common in the clinical evolution of acute coronary syndrome with ST segment elevation and have a significant influence on the patients prognosis.Objective: The determine the epidemiological clinical and therapeutic aspects of rhythm disorders with elevation in the ST segment.Methods: We have performed a descriptive retrospective study of 53 cases of rhythm disorder among 320 cases of coronaries syndromes with ST segment elevation.Results: The prevalence of cardiac conduction disorders was 16,5% over a period of 8 years. The average age of our patients was 58 years old (extremes: 26 to 81 years old). Hypertension was the most prevalent risk factor (60.4%). Low location was the most found (53%) and conduction disorders were dominated by right bundle branch block (35, 8%).Coronary angiography was performed in all our patients. The found lesions incoronary angiography concerned more the circumflex (32,1%) and anterior inter ventricular artery (28,3%). Coronary angioplasty has been  performed in 66 % of cases, and thrombolysis in 9, 4%. The implantation of temporary pace maker was done in 32% of cases. The evolution was favorable for all our patients.Conclusion: Arrhythmias in ST-segment elevation coronary syndrome should be diagnosed early and immediately be taken care of because of the risk of sudden death.
