    Addition of granulocyte colony-stimulating factor to chemotherapy in patients with Aids-related lymphoma: effects on neutrophil Fcgamma receptor expression and soluble FcgammaRIII plasma levels

    AIDS-related neutropenia and neutrophil dysfunction can (partly) be reversed by granulocyte-colony stimulating factor (G-CSF). We studied the effect of G-CSF on neutrophil increment and levels of soluble Fc gamma receptor type III in 15 patients with AIDS-related lymphoma (ARL) undergoing chemotherapy. In six of these patients we performed a detailed kinetic analysis of the membrane expression of the functionally important Fc gamma-receptors type I, II and III. In all these patients G-CSF induced Fc gammaRI positive neutrophils with a decreased expression of the Fc gammaRIII receptor. These changes were similar to those seen both in healthy volunteers and in non-HIV-infected individuals treated with chemotherapy. Interestingly, the mean neutrophil and sFc gammaRIII increment were significantly lower and more patients had a nadir granulocyte count <0.5 x 10(9)/l after the first cycle than after the second cycle of chemotherapy. This may be related to a therapy-associated decrease in HIV-1 viral load. The conclusion is that patients treated with chemotherapy for ARL have a qualitatively normal response to G-CS

    Ruxolitinib in Myelofibrosis and Baseline Thrombocytopenia in Real Life: Results in Dutch Patients and Review of the Literature

    Item does not contain fulltextBACKGROUND: Ruxolitinib is an approved treatment for myelofibrosis patients, but data regarding patients with baseline thrombocytopenia are limited. The EXPAND study recently suggested tolerability of ruxolitinib, with a maximum starting dose of 10 mg 2 times a day (BID). However, the small sample size and vigorous follow-up in this trial hamper direct translation of these results to routine practice. PATIENTS AND METHODS: We report retrospective data on Dutch ruxolitinib-treated myelofibrosis patients, focusing on those with baseline thrombocytopenia. Additionally, we reviewed current literature regarding ruxolitinib treatment in this subgroup. RESULTS: In our cohort, 12 of 119 patients had a baseline platelet count of < 100 x 10(9)/L. Spleen responses at a mean treatment duration of 25 weeks were documented in 1 of 6 and 15 of 47 patients with and without baseline thrombocytopenia, respectively. Despite a high rate of grade 3 or higher thrombocytopenia in thrombocytopenic versus nonthrombocytopenic patients (42% vs. 15%), no grade 3 or higher hemorrhage was reported. Median doses in thrombocytopenic patients were 15 and 10 mg BID at the start and after 12 weeks of treatment, respectively. Additionally, 238 thrombocytopenic patients were identified in the available literature, of whom 59 were treated in routine practice. Incidences of severe thrombocytopenia reported separately for patients with baseline thrombocytopenia were 30% to 59% (grade 3 or higher) and 4% to 60% (grade 4). Severe bleeding, pooled across our data and evaluable studies, occurred in 2.4%. CONCLUSION: Ruxolitinib treatment appears to be safe for patients with platelet counts of 50 to 100 x 10(9)/L in real-life practice. We did not find any reason to discourage a starting dose of 10 mg BID in this subgroup

    This article treats with Lily Braun, known as an ardent feminist and a maverick Social Democrat, by concentrating on an analysis of her Feminism. Although she was a noble by birth and predestined to inherit a fortune, her decision to live as a feminist changed her whole life. At first, she began her activity in the bourgeois feminist organization, but soon disappointed at its passivity and became a member of the Social Democratic party. However, Braun had trouble in the Social democratic party, especially because of her notorious competition with Clara Zetkin, the leader of the women&quot;s organization within SPD, and resigned from it. Irrespective of her failure as an activist, her feminism deserves comprehensive assessment. Her feminism can be characterized by emphasizing not only equal rights in public sphere, but also domestic and sexual liberation. Although she appears to have held a stereotyped image of femininity in elevating motherhood to the most fundamental human relationship, it is also undeniable that she is a significant precursor of the present-day feminism.clos
