    Child Care Characteristics and Quality in Nebraska

    The Midwest Child Care Research Consortium conducted a study of child care quality and characteristics of the child care work force in Nebraska, Iowa, Kansas and Missouri to help states establish a baseline for tracking quality over time, following initiatives, policy and other changes. The measures are not based on Nebraska child care licensing standards. Rather, using research-based measures of quality, they assess the extent to which quality indicators are present among the child care settings and in the work force. The current study included a random telephone survey of 2022 Midwestern child care providers (508 from Nebraska), conducted during late spring and summer of 2001 by the Gallup Organization, and follow-up in-depth observations of 365 providers (85 from Nebraska), conducted by four Midwestern state universities. Key findings from the study are as follows: 1. In Nebraska, as is true across the Midwestern states, a majority of providers regard child care as their profession, have been providing child care for over five years and intend to stay in the field. This is despite low, fulltime earnings (averaging $14,700 a year in Nebraska), which, for many providers, is below poverty level. 2. Using well-respected measures of quality, the researchers found that child care quality in Nebraska is comparable to that of Midwestern neighbors Missouri and Kansas and to child care nationwide; 34% of care observed was “good” quality; 48% was rated as minimal or mediocre quality and 18% was rated poor quality. Center-based infant/toddler, center-based preschool and licensed family child care were comparable to one another in quality and to similar care of Midwestern neighbors, while license exempt (approved) care averaged lower quality than other types of care in Nebraska. Other studies have shown that good quality on the measure used in the Midwest study predicts positive school readiness outcomes for children, and poor quality predicts poorer outcomes for children, especially for children in poverty. 3. A number of training, education, accreditation and workplace efforts were associated with higher quality including: Heads Up! Reading (in Nebraska preschool center-based settings); employee benefits such as health care (in center-based settings); the Child Development Associate Credential; participating in the USDA Food Program; first aid training; higher levels of education; entering into partnership with a Head Start or Early Head Start program; completing a nationally recognized accreditation in early childhood education; following a curriculum; and completing more than 24 hours of training in the previous year. Nebraska led the Midwest in the percentage of providers who had completed CPR and first aid training. 4. The study identified ways that Nebraska can improve child care quality. Two of these are to improve pre-literacy environments and to provide incentives to improve quality to providers who serve children receiving child care subsidies. First, Nebraska child care was deficient in preliteracy environments. Following the current national emphasis on preliteracy skills (reading to children, helping them understand and appreciate print media, and encouraging expression), pre-literacy environments are likely to be emphasized in upcoming child care block grant and Head Start reauthorizations in 2003. Few infant/toddler providers were observed reading to children; many family home providers lacked materials to encourage verbal expression. While preschool center-based providers had more books available, many scored only at a minimal level in pre-literacy activities. Second, among providers caring for children receiving subsidies, in some sectors the quality was lower when providers cared for larger portions of children receiving child care subsidies. Incentives for quality among family providers caring for children receiving subsidies are recommended to ensure that low-income children receive quality care. Nebraska and its neighbors in the Region VII of U.S. DHHS are among the first states in the nation to assess child care quality on a statewide and region wide basis. These baseline data will permit examination of changes over time in quality; for example, as a result of the new Nebraska TEACH program and other new and continuing initiatives

    Novel Mouse Model for Analysis of Macrophage Function in Neuroblastoma

    Background: Neuroblastoma is the third most common childhood cancer and accounts for 12% of cancer-associated deaths in children under the age of 15. Patients with high risk neuroblastoma have a poor 5-year survival rate of less than 50%. Neuroblastoma tumors treated with the histone deacetylase inhibitor (HDACi) vorinostat have increased infiltration of macrophages with upregulated immune cell-surface receptors. Neuroblastoma cells release VEGF and M-CSF, which may alter intratumoral macrophage populations. VEGF has also been implicated in alteration of amyloid precursor protein family processing. Our lab demonstrated that amyloid precursor protein 2 (APLP2), a member of the amyloid precursor protein family, plays an important role in the migration of tumor cells. APLP2 is known to be expressed by macrophages, but no studies have previously examined macrophage functions that are impacted by APLP2 in the context of neuroblastoma disease and its treatment by HDACi drugs. Significance of Problem: Because of the high morbidity and mortality associated with neuroblastoma, studies such as this one that are designed to comprehend the interaction of immunity and treatment in neuroblastoma are clinically significant. The results from this study are also expected to expand our comprehension of macrophage function and regulation, and thus will be of broad value in the immunology and oncology fields. Experimental Design and Results: We have treated neuroblastoma tumor cells in vitro with M344, an HDACi with structural similarity to vorinostat, and showed that M344 decreases neuroblastoma cell growth. In addition, we have generated mice that lack APLP2 expression in cells expressing the Csf-1 receptor (a protein characteristically expressed by macrophages and dendritic cells). We discovered that following polarization, macrophages collected from the bone marrow of these mice have an altered distribution of M1 and M2 sub-populations, which are macrophage sub-populations known to differ in their migratory capabilities. Furthermore, we have shown that M1 and M2 subpopulations of bone marrow-derived macrophages from normal mice differ in their expression of APLP2. Thus, APLP2 is influential in macrophage biology, and we have created a novel mouse model for defining its specific contributions in mice treated with HDACi that influence macrophage biology. Conclusions: Based on the data that we have acquired, we are well positioned to fully explore both the impact of HDACi drugs on macrophage/dendritic cell populations in a syngeneic neuroblastoma mouse model, and to define the role of APLP2 in the function of these cell populations in the context of neuroblastoma.https://digitalcommons.unmc.edu/chri_forum/1000/thumbnail.jp

At the international level, there is a longer tradition in the Critical Political Economy of Media/Communication, especially in the United Kingdom and North America. Since the start of the new crisis of capitalism in 2008, the interest in Marx’s works has generally increased. At the same time communicative and ideological features of societal changes’ unpredictable turbulences have become evident. This contribution introduces some specific approaches. It also discusses 14 aspects of why the complex, multidimensional, open and dynamic research approach of the critique of capitalism and society that goes back Marx’s theory remains relevant today

    Effectiveness of postoperative radiotherapy after radical cystectomy for locally advanced bladder cancer

    BACKGROUND: Local-regional failure (LF) for locally advanced bladder cancer (LABC) after radical cystectomy (RC) is common even with chemotherapy and is associated with high morbidity/mortality. Postoperative radiotherapy (PORT) can reduce LF and may enhance overall survival (OS) but has no defined role. We hypothesized that the addition of PORT would improve OS in LABC in a large nationwide oncology database. METHODS: We identified ≥ pT3pN0-3M0 LABC patients in the National Cancer Database diagnosed 2004-2014 who underwent RC ± PORT. OS was calculated using Kaplan-Meier and Cox proportional hazards regression modeling was used to identify predictors of OS. Propensity matching was performed to match RC patients who received PORT vs those who did not. RESULTS: 15,124 RC patients were identified with 512 (3.3%) receiving PORT. Median OS was 20.0 months (95% CI, 18.2-21.8) for PORT vs 20.8 months (95% CI, 20.3-21.3) for no PORT (P = 0.178). In multivariable analysis, PORT was independently associated with improved OS: hazard ratio 0.87 (95% CI, 0.78-0.97); P = 0.008. A one-to-three propensity match yielded 1,858 patients (24.9% receiving PORT and 75.1% without). In the propensity-matched cohort, median OS was 19.8 months (95% CI, 18.0-21.6) for PORT vs 16.9 months (95% CI, 15.6-18.1) for no PORT (P = 0.030). In the propensity-matched cohort of urothelial carcinoma patients (N = 1,460), PORT was associated with improved OS for pT4, pN+, and positive margins (P \u3c 0.01 all). CONCLUSION: In this observational cohort, PORT was associated with improved OS in LABC. While the data should be interpreted cautiously, these results lend support to the use of PORT in selected patients with LABC, regardless of histology. Prospective trials of PORT are warranted

    Radiogenic and Muon-Induced Backgrounds in the LUX Dark Matter Detector

    The Large Underground Xenon (LUX) dark matter experiment aims to detect rare low-energy interactions from Weakly Interacting Massive Particles (WIMPs). The radiogenic backgrounds in the LUX detector have been measured and compared with Monte Carlo simulation. Measurements of LUX high-energy data have provided direct constraints on all background sources contributing to the background model. The expected background rate from the background model for the 85.3 day WIMP search run is (2.6±0.2stat±0.4sys)×103(2.6\pm0.2_{\textrm{stat}}\pm0.4_{\textrm{sys}})\times10^{-3}~events~keVee1_{ee}^{-1}~kg1^{-1}~day1^{-1} in a 118~kg fiducial volume. The observed background rate is (3.6±0.4stat)×103(3.6\pm0.4_{\textrm{stat}})\times10^{-3}~events~keVee1_{ee}^{-1}~kg1^{-1}~day1^{-1}, consistent with model projections.     Amyloid Precursor-like Protein 2 Expression Increases during Pancreatic Cancer Development and Shortens the Survival of a Spontaneous Mouse Model of Pancreatic Cancer.

    In the United States, pancreatic cancer is a major cause of cancer-related deaths. Although substantial efforts have been made to understand pancreatic cancer biology and improve therapeutic efficacy, patients still face a bleak chance of survival. A greater understanding of pancreatic cancer development and the identification of novel treatment targets are desperately needed. Our analysis of gene expression data from patient samples showed an increase in amyloid precursor-like protein 2 (APLP2) expression within primary tumor epithelium relative to pancreatic intraepithelial neoplasia (PanIN) epithelial cells. Augmented expression of APLP2 in primary tumors compared to adjacent stroma was also observed. Genetically engineered mouse models of spontaneous pancreatic ductal adenocarcinoma were used to investigate APLP2\u27s role in cancer development. We found that APLP2 expression intensifies significantly during pancreatic cancer initiation and progression in the LSL-KrasG12D/+; LSL-Trp53R172H/+; Pdx-1-Cre (KPC) mouse model, as shown by immunohistochemistry analysis. In studies utilizing pancreas-specific heterozygous and homozygous knockout of APLP2 in the KPC mouse model background, we observed significantly prolonged survival and reduced metastatic progression of pancreatic cancer. These results demonstrate the importance of APLP2 in pancreatic cancer initiation and metastasis and indicate that APLP2 should be considered a potential therapeutic target for this disease

    On Observability and Monitoring of Distributed Systems: An Industry Interview Study

    Business success of companies heavily depends on the availability and performance of their client applications. Due to modern development paradigms such as DevOps and microservice architectural styles, applications are decoupled into services with complex interactions and dependencies. Although these paradigms enable individual development cycles with reduced delivery times, they cause several challenges to manage the services in distributed systems. One major challenge is to observe and monitor such distributed systems. This paper provides a qualitative study to understand the challenges and good practices in the field of observability and monitoring of distributed systems. In 28 semi-structured interviews with software professionals we discovered increasing complexity and dynamics in that field. Especially observability becomes an essential prerequisite to ensure stable services and further development of client applications. However, the participants mentioned a discrepancy in the awareness regarding the importance of the topic, both from the management as well as from the developer perspective. Besides technical challenges, we identified a strong need for an organizational concept including strategy, roles and responsibilities. Our results support practitioners in developing and implementing systematic observability and monitoring for distributed systems
