Assessment of the Validity of Reported Antibiotic Allergic Reactions in Pediatric Patients
Study Objective. To determine whether a reported antibiotic allergy was likely to have been immunologically mediated.
Design. Questionnaire-based study.
Setting. Tertiary care, freestanding children\u27s hospital.
Patients. One hundred patients aged 1 month-18 years for whom guardians reported an allergy to an antibiotic at the time of hospital admission between October 2009 and March 2010.
Intervention. Guardians of the patients were interviewed by using a standardized allergy assessment questionnaire.
Measurements and Main Results. Based on answers to the questionnaire, the reported allergic reactions were categorized to determine if they were true allergies or adverse reactions. Among the 100 patients, reported allergies were categorized as immunologically mediated reactions in 58%, non-immunologically mediated adverse drug reactions in 27%, no reaction in 3%, and unknown in 12%. Reactions to penicillins, cephalosporins, or sulfonamides were reported most frequently and were attributed to immunologically mediated reactions in 68% (26/38), 74% (17/23), and 67% (10/15) of instances, respectively.
Conclusion. Use of the allergy assessment questionnaire determined that 58% of the 100 reported antibiotic allergies fulfilled criteria for an immunologically mediated reaction. These findings underscore the utility of an allergy assessment questionnaire, versus a simple drug history, in improving the accuracy of reported antibiotic reactions
Billiard Systems in Three Dimensions: The Boundary Integral Equation and the Trace Formula
We derive semiclassical contributions of periodic orbits from a boundary
integral equation for three-dimensional billiard systems. We use an iterative
method that keeps track of the composition of the stability matrix and the
Maslov index as an orbit is traversed. Results are given for isolated periodic
orbits and rotationally invariant families of periodic orbits in axially
symmetric billiard systems. A practical method for determining the stability

One-way multigrid method in electronic structure calculations
We propose a simple and efficient one-way multigrid method for
self-consistent electronic structure calculations based on iterative
diagonalization. Total energy calculations are performed on several different
levels of grids starting from the coarsest grid, with wave functions
transferred to each finer level. The only changes compared to a single grid
calculation are interpolation and orthonormalization steps outside the original
total energy calculation and required only for transferring between grids. This
feature results in a minimal amount of code change, and enables us to employ a
sophisticated interpolation method and noninteger ratio of grid spacings.
Calculations employing a preconditioned conjugate gradient method are presented
for two examples, a quantum dot and a charged molecular system. Use of three
grid levels with grid spacings 2h, 1.5h, and h decreases the computer time by

Semiclassical Treatment of Diffraction in Billiard Systems with a Flux Line
In billiard systems with a flux line semiclassical approximations for the
density of states contain contributions from periodic orbits as well as from
diffractive orbits that are scattered on the flux line. We derive a
semiclassical approximation for diffractive orbits that are scattered once on a
flux line. This approximation is uniformly valid for all scattering angles. The
diffractive contributions are necessary in order that semiclassical

Second harmonic generation in SiC polytypes
LMTO calculations are presented for the frequency dependent second harmonic
generation (SHG) in the polytypes 2H, 4H, 6H, 15R and 3C of SiC. All
independent tensor components are calculated. The spectral features and the
ratios of the 333 to 311 tensorial components are studied as a function of the
degree of hexagonality. The relationship to the linear optical response and the
underlying band structure are investigated. SHG is suggested to be a sensitive
tool for investigating the near band edge interband excitations.Comment: 12 pages, 10 figure
A Lyman-alpha-only AGN from the Sloan Digital Sky Survey
The Sloan Digital Sky Survey has discovered a z=2.4917 radio-loud active
galactic nucleus (AGN) with a luminous, variable, low-polarization UV
continuum, H I two-photon emission, and a moderately broad Lyman-alpha line
(FWHM = 1430 km/s) but without obvious metal-line emission. SDSS
J113658.36+024220.1 does have associated metal-line absorption in three
distinct, narrow systems spanning a velocity range of 2710 km/s. Despite
certain spectral similarities, SDSS J1136+0242 is not a Lyman-break galaxy.
Instead, the Ly-alpha and two-photon emission can be attributed to an extended,
low-metallicity narrow-line region. The unpolarized continuum argues that we
see SDSS J1136+0242 very close to the axis of any ionization cone present. We
can conceive of two plausible explanations for why we see a strong UV continuum
but no broad-line emission in this `face-on radio galaxy' model for SDSS
J1136+0242: the continuum could be relativistically beamed synchrotron emission
which swamps the broad-line emission; or, more likely, SDSS J1136+0242 could be
similar to PG 1407+265, a quasar in which for some unknown reason the
high-ionization emission lines are very broad, very weak, and highly

Intermittent control models of human standing: similarities and differences
Two architectures of intermittent control are compared and contrasted in the context of the single inverted pendulum model often used for describing standing in humans. The architectures are similar insofar as they use periods of open-loop control punctuated by switching events when crossing a switching surface to keep the system state trajectories close to trajectories leading to equilibrium. The architectures differ in two significant ways. Firstly, in one case, the open-loop control trajectory is generated by a system-matched hold, and in the other case, the open-loop control signal is zero. Secondly, prediction is used in one case but not the other. The former difference is examined in this paper. The zero control alternative leads to periodic oscillations associated with limit cycles; whereas the system-matched control alternative gives trajectories (including homoclinic orbits) which contain the equilibrium point and do not have oscillatory behaviour. Despite this difference in behaviour, it is further shown that behaviour can appear similar when either the system is perturbed by additive noise or the system-matched trajectory generation is perturbed. The purpose of the research is to come to a common approach for understanding the theoretical properties of the two alternatives with the twin aims of choosing which provides the best explanation of current experimental data (which may not, by itself, distinguish beween the two alternatives) and suggesting future experiments to distinguish between the two alternatives
Meaning in hoarding: perspectives of people who hoard on clutter, culture, and agency
Hoarding has become increasingly prominent in clinical practice and popular culture in recent years, giving rise to extensive research and commentary. Critical responses in the social sciences have criticised the cultural assumptions built in to the construct of ‘hoarding disorder’ and expressed fears that it may generate stigma outweighing its benefits; however, few of these studies have engaged directly with ‘hoarders’ themselves. This paper reports on in-depth, semi-structured interviews with ten individuals living in England, who received assessment and intervention for hoarding from Social Services. Their narratives drew on the cultural repertoire of values and discourses around waste and worth, the mediation of sociality and relationships through material objects, physical constraints on keeping order, and the role played by mental health. Analysing these perspectives anthropologically shows how dominant models of hoarding, such as the DSM-5 paradigm, potentially lend themselves to reductionist understandings that efface the meaning ‘hoarding’ may have and thereby deny agency to the person labelled as ‘hoarder’. More culturally informed analysis, by contrast, affords insights into the complex landscape of value, waste, social critique, emotion, interpersonal relationships and practical difficulties that may underlie hoarding cases, and points the way to more person-centred practice and analysis
Hole concentration and phonon renormalization in Ca-doped YBa_2Cu_3O_y (6.76 < y < 7.00)
In order to access the overdoped regime of the YBa_2Cu_3O_y phase diagram, 2%
Ca is substituted for Y in YBa_2Cu_3O_y (y = 7.00,6.93,6.88,6.76). Raman
scattering studies have been carried out on these four single crystals.
Measurements of the superconductivity-induced renormalization in frequency
(Delta \omega) and linewidth (\Delta 2\gamma) of the 340 cm^{-1} B_{1g} phonon
demonstrate that the magnitude of the renormalization is directly related to
the hole concentration (p), and not simply the oxygen content. The changes in
\Delta \omega with p imply that the superconducting gap (\Delta_{max})
decreases monotonically with increasing hole concentration in the overdoped
regime, and \Delta \omega falls to zero in the underdoped regime. The linewidth
renormalization \Delta 2\gamma is negative in the underdoped regime, crossing

Bonding, Moment Formation, and Magnetic Interactions in Ca14MnBi11 and Ba14MnBi11
The ``14-1-11'' phase compounds based on magnetic Mn ions and typified by
Ca14MnBi11 and Ba14MnBi11 show unusual magnetic behavior, but the large number
(104) of atoms in the primitive cell has precluded any previous full electronic
structure study. Using an efficient, local orbital based method within the
local spin density approximation to study the electronic structure, we find a
gap between a bonding valence band complex and an antibonding conduction band
continuum. The bonding bands lack one electron per formula unit of being
filled, making them low carrier density p-type metals. The hole resides in the
MnBi4 tetrahedral unit and partially compensates the high spin d^5 Mn moment,
leaving a net spin near 4 \mu_B that is consistent with experiment. These
manganites are composed of two disjoint but interpenetrating `jungle gym'
networks of spin 4/2 MnBi4^{9-} units with ferromagnetic interactions within
the same network, and weaker couplings between the networks whose sign and
magnitude is sensitive to materials parameters. Ca14MnBi11 is calculated to be
ferromagnetic as observed, while for Ba14MnBi11 (which is antiferromagnetic)
the ferro- and antiferromagnetic states are calculated to be essentially
degenerate. The band structure of the ferromagnetic states is very close to
