Utilization and spending trends for antiretroviral medications in the U.S. Medicaid program from 1991 to 2005
<p>Abstract</p> <p>Background</p> <p>HIV/AIDS incidence and mortality rates have decreased in the U.S. since 1996. Accompanying the longer life spans of those diagnosed with the disease, however, is a tremendous rise in expenditures on medication. The objective of this study is to describe the trends in utilization of, spending on, and market shares of antiretroviral medications in the U.S. Medicaid Program. Antiretroviral drugs include nucleoside reverse transcriptase inhibitors (NRTIs), protease inhibitors (PIs), nonnucleoside reverse transcriptase inhibitors (NNRTIs), and fusion inhibitors (FIs).</p> <p>Methods</p> <p>Utilization and payment data from 1991 to 2005 are provided by the Centers for Medicare & Medicaid Services. Descriptive summary analyses were used to assess quarterly prescription numbers and amounts of payment.</p> <p>Results</p> <p>The total number of prescriptions for antiretrovirals increased from 168,914 in 1991 to 2.0 million in 1998, and 3.0 million in 2005, a 16.7-fold increase over 15 years. The number of prescriptions for NRTIs reached 1.6 million in 2005. Prescriptions for PIs increased from 114 in 1995 to 932,176 in 2005, while the number of prescriptions for NNRTIs increased from 1,339 in 1996 to 401,272 in 2005. The total payment for antiretroviral drugs in the U.S. Medicaid Program increased from US 1.6 billion in 2005, a 49.8-fold increase. In 2005, NRTIs as a class had the highest payment market share. These drugs alone accounted for US1040) and Combivir<sup>® </sup>(750), Lexiva<sup>® </sup>(420), Viramune<sup>® </sup>(1914).</p> <p>Conclusion</p> <p>The tremendous growth in antiretroviral spending is due primarily to rising utilization, secondarily to the entry of newer, more expensive antiretrovirals, and, finally, in part to rising per-prescription cost of existing medications.</p
A Self-Reference False Memory Effect in the DRM Paradigm: Evidence from Eastern and Western Samples
It is well established that processing information in relation to oneself (i.e., selfreferencing) leads to better memory for that information than processing that same information in relation to others (i.e., other-referencing). However, it is unknown whether self-referencing also leads to more false memories than other-referencing. In the current two experiments with European and East Asian samples, we presented participants the Deese-Roediger/McDermott (DRM) lists together with their own name or other people’s name (i.e., “Trump” in Experiment 1 and “Li Ming” in Experiment 2). We found consistent results across the two experiments; that is, in the self-reference condition, participants had higher true and false memory rates compared to those in the other-reference condition. Moreover, we found that selfreferencing did not exhibit superior mnemonic advantage in terms of net accuracy compared to other-referencing and neutral conditions. These findings are discussed in terms of theoretical frameworks such as spreading activation theories and the fuzzytrace theory. We propose that our results reflect the adaptive nature of memory in the sense that cognitive processes that increase mnemonic efficiency may also increase susceptibility to associative false memories
Influence of a knot on the strength of a polymer strand
Many experiments have been done to determine the relative strength of
different knots, and these show that the break in a knotted rope almost
invariably occurs at a point just outside the `entrance' to the knot. The
influence of knots on the properties of polymers has become of great interest,
in part because of their effect on mechanical properties. Knot theory applied
to the topology of macromolecules indicates that the simple trefoil or
`overhand' knot is likely to be present with high probability in any long
polymer strand. Fragments of DNA have been observed to contain such knots in
experiments and computer simulations. Here we use {\it ab initio} computational
methods to investigate the effect of a trefoil knot on the breaking strength of
a polymer strand. We find that the knot weakens the strand significantly, and
that, like a knotted rope, it breaks under tension at the entrance to the knot.Comment: 3 pages, 4 figure
A New Approach for Adipose Tissue Treatment and Body Contouring Using Radiofrequency-Assisted Liposuction
A new liposuction technology for adipocyte lipolysis and uniform three-dimensional tissue heating and contraction is presented. The technology is based on bipolar radiofrequency energy applied to the subcutaneous adipose tissue and subdermal skin surface. Preliminary clinical results, thermal monitoring, and histologic biopsies of the treated tissue demonstrate rapid preaspiration liquefaction of adipose tissue, coagulation of subcutaneous blood vessels, and uniform sustained heating of tissue
ALPS: the Arbitrary Linear Plasma Solver
The Arbitrary Linear Plasma Solver (ALPS) is a parallelised numerical code that
solves the dispersion relation in a hot (even relativistic) magnetised plasma with an
arbitrary number of particle species with arbitrary gyrotropic equilibrium distribution
functions for any direction of wave propagation with respect to the background
field. ALPS reads the background momentum distributions as tables of values on
a (p⊥, pk) grid, where p⊥ and pk are the momentum coordinates in the directions
perpendicular and parallel to the background magnetic field, respectively. We present
the mathematical and numerical approach used by ALPS and introduce our algorithms
for the handling of poles and the analytic continuation for the Landau contour
integral. We then show test calculations of dispersion relations for a selection of
stable and unstable configurations in Maxwellian, bi-Maxwellian, κ-distributed and
Jüttner-distributed plasmas. These tests demonstrate that ALPS derives reliable plasma
dispersion relations. ALPS will make it possible to determine the properties of waves
and instabilities in the non-equilibrium plasmas that are frequently found in space,
laboratory experiments and numerical simulations
Testing the paradox of enrichment along a land use gradient in a multitrophic aboveground and belowground community
In the light of ongoing land use changes, it is important to understand how multitrophic communities perform at different land use intensities. The paradox of enrichment predicts that fertilization leads to destabilization and extinction of predator-prey systems. We tested this prediction for a land use intensity gradient from natural to highly fertilized agricultural ecosystems. We included multiple aboveground and belowground trophic levels and land use-dependent searching efficiencies of insects. To overcome logistic constraints of field experiments, we used a successfully validated simulation model to investigate plant responses to removal of herbivores and their enemies. Consistent with our predictions, instability measured by herbivore-induced plant mortality increased with increasing land use intensity. Simultaneously, the balance between herbivores and natural enemies turned increasingly towards herbivore dominance and natural enemy failure. Under natural conditions, there were more frequently significant effects of belowground herbivores and their natural enemies on plant performance, whereas there were more aboveground effects in agroecosystems. This result was partly due to the “boom-bust” behavior of the shoot herbivore population. Plant responses to herbivore or natural enemy removal were much more abrupt than the imposed smooth land use intensity gradient. This may be due to the presence of multiple trophic levels aboveground and belowground. Our model suggests that destabilization and extinction are more likely to occur in agroecosystems than in natural communities, but the shape of the relationship is nonlinear under the influence of multiple trophic interactions.
Evidence for Anthropogenic Surface Loading as Trigger Mechanism of the 2008 Wenchuan Earthquake
Two and a half years prior to China's M7.9 Wenchuan earthquake of May 2008,
at least 300 million metric tons of water accumulated with additional seasonal
water level changes in the Minjiang River Valley at the eastern margin of the
Longmen Shan. This article shows that static surface loading in the Zipingpu
water reservoir induced Coulomb failure stresses on the nearby Beichuan thrust
fault system at <17km depth. Triggering stresses exceeded levels of daily lunar
and solar tides and perturbed a fault area measuring 416+/-96km^2. These stress
perturbations, in turn, likely advanced the clock of the mainshock and directed
the initial rupture propagation upward towards the reservoir on the
"Coulomb-like" Beichuan fault with rate-and-state dependent frictional
behavior. Static triggering perturbations produced up to 60 years (0.6%) of
equivalent tectonic loading, and show strong correlations to the coseismic
slip. Moreover, correlations between clock advancement and coseismic slip,
observed during the mainshock beneath the reservoir, are strongest for a longer
seismic cycle (10kyr) of M>7 earthquakes. Finally, the daily event rate of the
micro-seismicity (M>0.5) correlates well with the static stress perturbations,
indicating destabilization.Comment: 22 pages, 4 figures, 3 table
Laser treatment in diabetic retinopathy
Diabetic retinopathy is a leading cause of visual impairment and blindness in developed countries due to macular edema and proliferative diabetic retinopathy (PDR). For both complications laser treatment may offer proven therapy: the Diabetic Retinopathy Study demonstrated that panretinal scatter photocoagulation reduces the risk of severe visual loss by >= 50% in eyes with high-risk characteristics. Pan-retinal scatter coagulation may also be beneficial in other PDR and severe nonproliferative diabetic retinopathy (NPDR) under certain conditions. For clinically significant macular edema the Early Treatment of Diabetic Retinopathy Study could show that immediate focal laser photocoagulation reduces the risk of moderate visual loss by at least 50%. When and how to perform laser treatment is described in detail, offering a proven treatment for many problems associated with diabetic retinopathy based on a high evidence level.
