    Smart Emission - Building a Spatial Data Infrastructure for an Environmental Citizen Sensor Network

This paper introduces the design and development of the data infrastructure for the Smart Emission initiative and discusses challenges for the future. The Spatial Data Infrastructure (SDI) is open and accessible on the Internet using open geospatial standards and (Web-) client applications.     The trajectory of caregiver burden and risk factors in dementia progression:A systematic review

    Background: Caring for patients with dementia at home is often a long-term process, in which the independence of the patient declines, and more responsibility and supervision time is required from the informal caregiver. Objective: In order to minimize and reduce caregiver burden, it is important to explore its trajectory and the accompanying risk factors as dementia progresses; the objective of this systematic review. Methods: PRISMA guidelines were followed in this systematic review. Three databases, PubMed, PsycINFO, and EMbase, were systematically searched in November 2019 using specific keywords. Results: 1,506 hits emerged during the systematic search but only eleven articles actually met the inclusion criteria for this review. The trajectory of caregiver burden is highly variable and depends on multiple factors. Important risk factors included: patients’ behavioral and neuropsychiatric symptoms, and their decline in functioning in (I)ADL; the caregiver’s age, gender, and physical and mental health; and, within the dyads (patient/caregiver), cohabitation and kinship. Conclusion: There is no one-size-fits-all for predicting how caregiver burden will change over time, but specific factors (like being a spouse and increased behavioral impairment and decline in functional status in the patient) may heighten the risk. Other factors, not yet comprehensively included in the published studies, might also prove to be important risk factors. Future research in the field of reducing caregiver burden is recommended to integrate the patient, caregiver, and context characteristics in the trajectory of caregiver burden, and to assess more clearly the phase of the dementia progression and use of external resources

    Serum magnesium, hepatocyte nuclear factor 1β genotype and post-transplant diabetes mellitus: a prospective study

    BACKGROUND: Retrospective studies suggest that tacrolimus-induced hypomagnesaemia is a risk factor for post-transplant diabetes mellitus (PTDM), but prospective studies are lacking. METHODS: This was a prospective study with measurements of serum magnesium and tacrolimus at pre-specified time points in the first year after living donor kidney transplantation (KT). The role of single nucleotide polymorphisms (SNPs) in hepatocyte nuclear factor 1β (HNF1β) was also explored because HNF1β regulates insulin secretion and renal magnesium handling. Repeated measurement and regression analyses were used to analyse associations with PTDM. RESULTS: In our cohort, 29 out of 167 kidney transplant recipients developed PTDM after 1 year (17%). Higher tacrolimus concentrations were significantly associated with lower serum magnesium and increased risk of hypomagnesaemia. Patients who developed PTDM had a significantly lower serum magnesium trajectory than patients who did not develop PTDM. In multivariate analysis, lower serum magnesium, age and body mass index were independent risk factors for PTDM. In recipients, the HNF1β SNP rs752010 G > A significantly increased the risk of PTDM [odds ratio (OR) = 2.56, 95% confidence interval (CI) 1.05-6.23] but not of hypomagnesaemia. This association lost significance after correction for age and sex (OR = 2.24, 95% CI 0.90-5.57). No association between HNF1β SNPs and PTDM was found in corresponding donors. CONCLUSIONS: A lower serum magnesium in the first year after KT is an independent risk factor for PTDM. The HNF1β SNP rs752010 G > A may add to this risk through an effect on insulin secretion rather than hypomagnesaemia, but its role requires further confirmation

    The impact of climate change on groundwater recharge: National-scale assessment for the British mainland

    Groundwater systems provide an important source of water supply as well as contributing baseflow to rivers, lakes and dependent ecosystems and so the impact of climate change on these systems needs to be understood. Calculating recharge to groundwater systems is, therefore, necessary to quantify what is typically one of the largest components of the groundwater balance. This study uses the national-scale recharge model developed for the British mainland and the 11 ensemble members from the Hadley Centre for rainfall and potential evaporation created by the Future Flows and Groundwater Levels (FFGWL) project to investigate the impact of future climate on groundwater resources. Changes to seasonal and monthly recharge for the 2050s and 2080s time slices have been produced for the whole modelled area and for river basin districts for England and Wales. Areal summaries and monthly time series of recharge values show a generally consistent trend of increased recharge in winter, decreased recharge in summer, and mixed pattern in autumn and spring. The work shows that increased winter rainfall is the main factor in increasing recharge. Water balance calculations reveal that over the 2050s and 2080s, the climate change “signal” predominates over the annual variability, which results in a clearer pattern of more recharge being concentrated in fewer months. This finding should prove useful for water resources planners to assess the resilience of groundwater resources to climate change. Further work is recommended to understand the sequencing of flooding and drought events and to the effects of soil health and land cover changes in the future analysis

    Serum magnesium and the risk of death from coronary heart disease and sudden cardiac death

    Background-Low serum magnesium has been implicated in cardiovascular mortality, but results are conflicting and the pathway is unclear. We studied the association of serum magnesium with coronary heart disease (CHD) mortality and sudden cardiac death (SCD) within the prospective population-based Rotterdam Study, with adjudicated end points and long-term follow-up. Methods and Results-Nine-thousand eight-hundred and twenty participants (mean age 65.1 years, 56.8% female) were included with a median follow-up of 8.7 years. We used multivariable Cox proportional hazard models and found that a 0.1 mmol/L increase in serum magnesium level was associated with a lower risk for CHD mortality (hazard ratio: 0.82, 95% CI 0.70-0.96). Furthermore, we divided serum magnesium in quartiles, with the second and third quartile combined as reference group (0.81-0.88 mmol/L). Low serum magnesium (=0.80 mmol/L) was associated with an increased risk of CHD mortality (N=431, hazard ratio: 1.36, 95% CI 1.09-1.69) and SCD (N=217, hazard ratio: 1.54, 95% CI 1.12-2.11). Low serum magnesium was associated with accelerated subclinical atherosclerosis (expressed as increased carotid intima-media thickness: +0.013 mm, 95% CI 0.005-0.020) and increased QT-interval, mainly through an effect on heart rate (RR-interval: -7.1 ms, 95% CI -13.5 to -0.8). Additional adjustments for carotid intima-media thickness and heart rate did not change the associations with CHD mortality and SCD. Conclusions-Low serum magnesium is associated with an increased risk of CHD mortality and SCD. Although low magnesium was associated with both carotid intima-media thickness and heart rate, this did not explain the relationship between serum magnesium and CHD mortality or SCD. Future studies should focus on why magnesium associates with CHD mortality and SCD and whether intervention reduces these risks

    Non-conservative Evolution of Cataclysmic Variables

    We suggest a new mechanism to account for the loss of angular momentum in binaries with non-conservative mass exchange. It is shown that in some cases the loss of matter can result in increase of the orbital angular momentum of a binary. If included into consideration in evolutionary calculations, this mechanism appreciably extends the range of mass ratios of components for which mass exchange in binaries is stable. It becomes possible to explain the existence of some observed cataclysmic binaries with high donor/accretor mass ratio, which was prohibited in conservative evolution models.Comment: LaTeX, 32 pages, to be published in Astron.     Reliable identification at the species level of Brucella isolates with MALDI-TOF-MS

    <p>Abstract</p> <p>Background</p> <p>The genus <it>Brucella </it>contains highly infectious species that are classified as biological threat agents. The timely detection and identification of the microorganism involved is essential for an effective response not only to biological warfare attacks but also to natural outbreaks. Matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS) is a rapid method for the analysis of biological samples. The advantages of this method, compared to conventional techniques, are rapidity, cost-effectiveness, accuracy and suitability for the high-throughput identification of bacteria. Discrepancies between taxonomy and genetic relatedness on the species and biovar level complicate the development of detection and identification assays.</p> <p>Results</p> <p>In this study, the accurate identification of <it>Brucella </it>species using MALDI-TOF-MS was achieved by constructing a <it>Brucella </it>reference library based on multilocus variable-number tandem repeat analysis (MLVA) data. By comparing MS-spectra from <it>Brucella </it>species against a custom-made MALDI-TOF-MS reference library, MALDI-TOF-MS could be used as a rapid identification method for <it>Brucella </it>species. In this way, 99.3% of the 152 isolates tested were identified at the species level, and <it>B. suis </it>biovar 1 and 2 were identified at the level of their biovar. This result demonstrates that for <it>Brucella</it>, even minimal genomic differences between these serovars translate to specific proteomic differences.</p> <p>Conclusions</p> <p>MALDI-TOF-MS can be developed into a fast and reliable identification method for genetically highly related species when potential taxonomic and genetic inconsistencies are taken into consideration during the generation of the reference library.</p
