    Adequacy of phosphodiesterase inhibitor in prevention and treatment of LPS induced organ failure in BALB/c mice

    Background: Even though with immense improvement and extensive understanding of pathophysiology of sepsis induced organ failure and affected population, it continues to put hundreds of people worldwide to eternal sleep due to lack of targeted therapy. Newer treatment modalities is the dire need of time. The present study was aimed to ascertain the adequacy of phosphodiesterases inhibitor - pentoxifylline (75mg/kg i.p) in endotoxin/LPS induced hepatotoxicity in BALB/c mice.Methods: The number of animals in each group was six. Endotoxin/lipopolysaccharides induced hepatotoxicity was reproduced in mice by giving lipopolysaccharide of serotype E. coli intraperitoneally. To ascertain the Preventive role, pentoxifylline was administered forehand LPS injection whereas therapeutic potential adjuged via post LPS delivering. The extent of liver damage was evaluated through serum alanine aminotransferases (ALT) and aspartate aminotransferase (AST) estimation along with histopathological examination of liver tissue.Results: Results set forth that serum ALT, AST levels and histological alteration abated considerably (p ≤0.05) both in animals subjected to pentoxifylline pre and post-treatment.Conclusions: Pentoxifylline set up promising results in endotoxin induced hepatotoxicity and can be used therapeutic adjuncts to conventional treatment strategies in sepsis induced liver failure

    Electrohydrodynamic Processes and Their Affecting Parameters

    Electrohydrodynamic processes such as electrospinning and electrospraying are simple, flexible, and cost-effective. Both processes use electrically charged jet of polymer solution for the fabrication of micro- or nanofibers and micro- or nanoparticle. Both of these electrodynamic techniques have been receiving increasing attention not only in the scientific community but also in industry. These fibers and particles offer several morphological and functional features that are suitable for tissue engineering in biomedical applications. The main apparatus used for both of these processes is almost the same. Both need electric voltage to induce charge on the droplet, which at optimized electric field leads to micro- or nanofibers and micro- or nanoparticles. Rayleigh in 1882, for the first time, theoretically estimated the maximum amount of charge that a liquid droplet could carry to change in a jet. This theory is now known as the “Rayleigh limit.” He predicted that a droplet on reaching Rayleigh limit would move as fine jets of liquid. More than 100 years later, Rayleigh limit theory was confirmed experimentally.     Pulmonary hyalinising granuloma: a rare pulmonary disorder

    Pulmonary hyalinising granulomas are rare, noninfectious fibrosclerosing lesions of the lung which can mimic metastatic disease. It was first described in literature by Engleman et al in the year 1977. Its etiology is unknown but they may be caused by an exaggerated immune response. The patient typically presents with cough, chest pain. dyspnoea or haemoptysis in association with multiple bilateral parenchymal nodules. We report the case of a 20 years old male who presented with a 12-month history of worsening dry cough. His plain chest radiograph and subsequent CT scan revealed bilateral pulmonary nodules. A CT guided biopsy of the pulmonary lesions was consistent with Pulmonary Hyalinising Granuloma [PHG]

    Space Charge–Limited Current Model for Polymers

    Polymers have exceptional charge transport mechanism as a combination of delocalization and localization of charge carriers with intramolecular and intermolecular charge interaction, respectively, and most of the time, it is interpreted with Mott-Gurney space charge–limited current model. As polymers are full of traps, therefore, Mott-Gurney space charge–limited model is modified with various trap distributions as trapped space charge–limited model. The most crucial parameter affected by the nature and distribution of traps is the carrier mobility, and it is argued that space charge–limited model is an acceptable choice for the mobility measurement for polymer. Similarly, in order to account the commonly observed lowering of trap barrier height at higher electric field, the Mott-Gurney space charge–limited current is further modified with little variations, which are evaluated and discussed in detail

    Fibrolipomatous hamartoma of the median nerve with macrodystrophia lipomatosa

    Fibrolipomatous hamartoma (FLH) is a rare congenital condition that presents with a benign overgrowth of the bone and fibroadipose tissue termed as macrodystrophia lipomatosa (MDL). Although commonly seen in the median nerve, other peripheral nerves can be involved. Diagnosis can be made on magnetic resonance imaging (MRI) due to the characteristic coaxial cable appearance on axial images and the spaghetti appearance on sagittal images. Histology shows mature adipose and fibrous tissue infiltrating the epineural and perineural compartments. Multiple or debulking surgeries are often needed, with an emphasis on cosmetic aspects. We present one such case in which wide margin excision and sural nerve graft were carried out

    Treatment Outcome of Multi-Drug Resistant Tuberculosis Treated As Outpatient in a Tertiary Care Center

    INTRODUCTION: Community-based out-patient treatment for multidrug-resistant tuberculosis (MDR TB) is relatively new concept with reported successful outcomes. OBJECTIVE: To assess the treatment outcomes of HIV negative multidrug-resistant tuberculosis (MDR TB) patients treated as outpatient at a tertiary care center in Karachi, Pakistan. METHODS: A retrospective observational study of 53 consecutive, culture proven HIV negative MDR TB patients (resistant at least to both Isoniazid (H) and Rifampin(R) treated at Aga Khan University Hospital, Karachi between August 1999 and March 2007. Data were collected on predesigned performa regarding patient’s demography, clinical features, radiological findings, drug sensitivity, treatment and outcome. RESULTS: A total of 53 HIV negative patients (27 males), with mean age of 37±15 years (range 15-76 years), received treatment as outpatient for culture proven MDR TB. 51 patients (96.2%) had pulmonary while 3 patients (5.6%) had extra-pulmonary TB. History of exposure to tuberculosis patients was found in 36 (67.9%) patients. Treatment regimen with 2nd line drugs was decided on individual basis according to DST on sputum culture results. The mean duration of treatment was 18 months. Successful outcome was seen in 25 patients (47.2%), 25 patients (47.2%) were loss to follow up and defaulted while 3 (5.6%) patients remain smear positive at the end of treatment. Success rate was 89 .2% in those who completed the treatment. CONCLUSION: Community-based out-patient treatment strategy is both feasible and safe for the treatment of MDR-TB patients in resource limited country like Pakistan and this strategy should be integrated into the routine approach to treatment of MDR-TB patients in the country where the expertise are available. High default rate is this strategy is the main challenge which should be addressed
