    Talar body fracture combined with medial malleolar fracture: a case report and literature review

    Fracture of the talar body is rare, it reaches articular congruence and determines the ankle's functional prognosis. The authors report a case of a combination of a sagittal talar body fracture with fracture of the medial malleolus in a 52-year-old following a road traffic accident. The association talus body fracture with a medial malleolus fracture is exceptional. Stable internal fixation can yield good functional results

    FRACTURE STRENGTH OF PALATAL DENTURE BASE CONSTRUCTED FROM DIFFERENT ACRYLIC DENTURE BASE MATERIALS

    Background: Acrylic resins constitute about 90% of polymeric materials used in prosthetic dentistry. However, they are not devoid of drawbacks. Their weakness includes poor resistance to fracture and abrasion, as well as changes in volume and shape during fabrication and use. Objectives: The aim of this study is to evaluate and compare the mechanical strength in fracture force test of maxillary palatal denture bases, fabricated from different acrylic resins. Materials and Methods: Twenty five maxillary edentulous stone casts were constructed from a standard silicon model of maxillary edentulous jaw. Five palatal denture bases were made from each of the following materials; conventional heat cure acrylic (Control Group), rapid cure acrylic, high impact acrylic and Biostar pure acrylic, according to the manufacturer’s instructions. The fracture force test was carried out on a universal testing machine (GUNT HAMBURG) at a crosshead speed of 10mm/min. The results were analyzed using ANOVA and t- test to determine the significant differences between tested groups at a significant level (P<0.05). Results: The high impact acrylic resin showed the highest fracture force value, while the Biostar pure acrylic resin showed the lowest value as compared with other tested materials. The results revealed highly significant differences between the high impact acrylic and conventional heat cure acrylic (P<0.001), and other tested denture base materials (P<0.05). Also, rapid heat cure acrylic resin showed significantly higher fracture force value than those obtained by both conventional heat cure acrylic resin and Biostar pure acrylic resin (P<0.05),(P<0.001) respectively. Finally, significantly lower fracture force value was obtained by the Biostar acrylic resin as compared to the conventional heat cure acrylic (P≤0.05) and other tested denture base materials (P<0.001). Conclusions: Under the limitation of this study for those patients who fracture their dentures on regular basis, it is possible to consider a high impact and rapid cure acrylic resins denture base, which shows a higher degree of resistance to fracture in comparison with conventional heat cure acrylic resin. However, this is unlike the Biostar pure acrylic resins that showed undesirable results as compared with conventional heat cure acrylic and other tested denture base materials

    Multistage Fragmentation of Ion Trap Mass Spectrometry System and Pseudo-MS 3

    A new approach was recently introduced to improve the structure elucidation power of tandem mass spectrometry simulating the MS3 of ion trap mass spectrometry system overcoming the different drawbacks of the latter. The fact that collision induced dissociation in the triple quadrupole mass spectrometer system provides richer fragment ions compared to those achieved in the ion trap mass spectrometer system utilizing resonance excitation. Moreover, extracting comprehensive spectra in the ion trap needs multistage fragmentation, whereas similar fragment ions may be acquired from one stage product ion scan using the triple quadrupole mass spectrometer. The new strategy was proven to enhance the qualitative performance of tandem mass spectrometry for structural elucidation of different chemical entities. In the current study we are endeavoring to prove our hypothesis of the efficiency of the new pseudo-MS3 technique via its comparison with the MS3 mode of ion trap mass spectrometry system. Ten pharmacologically and synthetically important (E)-3-(dimethylamino)-1-arylprop-2-en-1-ones (enaminones 4a–j) were chosen as model compounds for this study. This strategy permitted rigorous identification of all fragment ions using triple quadrupole mass spectrometer with sufficient specificity. It can be used to elucidate structures of different unknown components. The data presented in this paper provide clear evidence that our new pseudo-MS3 may simulate the MS3 of ion trap spectrometry system

    Individual-level determinants of waterpipe smoking demand in four Eastern-Mediterranean countries

The prevalence of waterpipe tobacco smoking in the Eastern Mediterranean Region is at alarmingly high levels, especially among young people. The objective of this research was to evaluate the preferences of young adult waterpipe smokers with respect to potential individual-level determinants of waterpipe smoking using discrete choice experiment methodology. Participants were young adult university students (18-29 years) who were ever waterpipe smokers, recruited from universities across four Eastern Mediterranean countries: Jordan, Oman, Palestine and the United Arab Emirates. The Internet-based discrete choice experiment, with 6 × 3 × 2 block design, evaluated preferences for choices of waterpipe smoking sessions, presented on hypothetical waterpipe café menus. Participants evaluated nine choice sets, each with five fruit-flavored options, a tobacco flavored option (non-flavored), and an opt-out option. Choices also varied based on nicotine content (0.0% vs. 0.05% vs. 0.5%) and price (low vs. high). Participants were randomized to receive menus with either a pictorial + text health-warning message or no message (between-subjects attribute). Multinomial logit regression models evaluated the influence of these attributes on waterpipe smoking choices. Across all four samples (n = 1859), participants preferred fruit-flavored varieties to tobacco flavor, lower nicotine content and lower prices. Exposure to the health warning did not significantly predict likelihood to opt-out. Flavor accounted for 81.4% of waterpipe smoking decisions. Limiting the use of fruit flavors in waterpipe tobacco, in addition to accurate nicotine content labeling and higher pricing may be effective at curbing the demand for waterpipe smoking among young adults

    Abstract We report a case of mucoid degeneration of the anterior cruciate ligament (ACL). Mucoid degeneration of the ACL is a very rare cause of knee pain. There have been only some reported cases of mucoid degeneration of the ACL in the English literature. We reviewed previous reports and summarized clinical features and symptoms, including those found in our case. Magnetic Resonance Imaging is the most useful tool for differentiating mucoid degeneration of the ACL from an intraligamentous ganglion or other lesions in the knee joint. If this disease is considered preoperatively, it can be diagnosed easily based on characteristic findings

    Abstract Isolated palmar dislocation of the fifth carpometacarpal joint is an uncommon injury and classified as radio-palmar or ulno-palmar according to the direction of displacement of the fifth metacarpal base . This very rare injury is often difficult to recognize. A careful neurologic assessment of the patient is a necessity, as well as obtaining proper radiographs of the hand. The purpose of this report is to present a patient with a pure isolated volar dislocation of the fifth carpometacarpal joint that was satisfactorily treated with closed reduction and casting. A review of the literature is presented

    Exploring the effect of khat (Catha edulis) chewing on the pharmacokinetics of the antiplatelet drug clopidogrel in rats using the newly developed LC-MS/MS technique

    Clopidogrel (CLOP) is widely used worldwide for cardiovascular complications. CLOP is highly metabolized in the liver to its active metabolite by cytochrome P450 enzymes. Studies have shown that khat, an addictive substance, is a powerful inhibitor of cytochrome P450 enzymes and can influence the metabolism of drugs that are concomitantly used. Therefore, this study was designed to evaluate the effects of khat on the pharmacokinetics of CLOP in rats. In this study, rats were administered either CLOP alone or CLOP combined with khat and their plasma were obtained at different time intervals and analyzed using the newly developed and validated liquid chromatography with tandem mass spectrometry (LC-MS/MS) method using foretinib (FTB) as the internal standard. The corresponding peak area of the analyte versus FTB was used for calculating the peak ratio. The validated LC-MS/MS method resulted in the separation of the well-defined quantifiable peaks of CLOP, FTB, and CLOP metabolite within 7 min. Results showed a significant influence of khat on the peak ratio of CLOP metabolite, which was found to be significantly decreased (P < 0.05) in comparison to CLOP alone, suggesting significant decrease in the conversion of CLOP to its active metabolite due to the inhibition of CYP450 enzymes by khat. Therefore, there might be a need for dose adjustment for regular khat chewers using CLOP
