    Decentralization and public services : the case of immunization

    The author studies the impact of political decentralization on childhood immunization, an essential public service provided in almost all countries. He examines the relationship empirically using a time-series data set of 140 low- and middle-income countries from 1980 to 1997. The author finds that decentralization has different effects in low- and middle-income countries. In the low-income group, decentralized countries have higher coverage rates than centralized ones, with an average difference of 8.5 percent for measles and DTP3 vaccines. In the middle-income group, the reverse effect is observed: decentralized countries have lower coverage rates than centralized ones, with an average difference of 5.2 percent for the same vaccines. Both results are significant at the 99 percent level. Modifiers of the decentralization-immunization relationship also differ in the two groups. In the low-income group, development assistance reduces the gains from decentralization. In the middle-income group, democratic government mitigates the negative effects of decentralization, and decentralization reverses the negative effects of ethnic tension and ethno-linguistic fractionalization, but institutional quality and literacy rates have no interactive effect either way. Similar results are obtained whether decentralization is measured with a dichotomous categorical variable or with more specific measures of fiscal decentralization. The study confirms predictions in the theoretical literature about the negative impact of local political control on services that have public goods characteristics and inter-jurisdictional externalities.     Immunization in developing countries : its political and organizational determinants

    The authors use cross-national social, political, economic, and institutional data to explain why some countries have stronger immunization programs than others, as measured by diphtheria-tetanus-pertussis (DTP) and measles vaccine coverage rates and the adoption of the hepatitis B vaccine. After reveiwing the existing literature on demand- and supply-side side factors that affect immunization programs, the authors find that the elements that most affect immunization programs in low- and middle-income countries involve broad changes in the global policy environment and contact with international agencies. Democracies tend to have lower coverage rates than autocracies, perhaps because bureaucratic elites have an affinity for immunization programs and are granted more autonomy in autocracies, althought this effect is not visible in low-income countries.     Scalable computing for earth observation - Application on Sea Ice analysis

    In recent years, Deep learning (DL) networks have shown considerable improvements and have become a preferred methodology in many different applications. These networks have outperformed other classical techniques, particularly in large data settings. In earth observation from the satellite field, for example, DL algorithms have demonstrated the ability to learn complicated nonlinear relationships in input data accurately. Thus, it contributed to advancement in this field. However, the training process of these networks has heavy computational overheads. The reason is two-fold: The sizable complexity of these networks and the high number of training samples needed to learn all parameters comprising these architectures. Although the quantity of training data enhances the accuracy of the trained models in general, the computational cost may restrict the amount of analysis that can be done. This issue is particularly critical in satellite remote sensing, where a myriad of satellites generate an enormous amount of data daily, and acquiring in-situ ground truth for building a large training dataset is a fundamental prerequisite. This dissertation considers various aspects of deep learning based sea ice monitoring from SAR data. In this application, labeling data is very costly and time-consuming. Also, in some cases, it is not even achievable due to challenges in establishing the required domain knowledge, specifically when it comes to monitoring Arctic Sea ice with Synthetic Aperture Radar (SAR), which is the application domain of this thesis. Because the Arctic is remote, has long dark seasons, and has a very dynamic weather system, the collection of reliable in-situ data is very demanding. In addition to the challenges of interpreting SAR data of sea ice, this issue makes SAR-based sea ice analysis with DL networks a complicated process. We propose novel DL methods to cope with the problems of scarce training data and address the computational cost of the training process. We analyze DL network capabilities based on self-designed architectures and learn strategies, such as transfer learning for sea ice classification. We also address the scarcity of training data by proposing a novel deep semi-supervised learning method based on SAR data for incorporating unlabeled data information into the training process. Finally, a new distributed DL method that can be used in a semi-supervised manner is proposed to address the computational complexity of deep neural network training

    Governance of communicable disease control services : a case study and lessons from India

    The authors study the impact of governance and administrative factors on communicable disease prevention in the Indian state of Karnataka using survey data from administrators, frontline workers, and elected local representatives. They identify a number of key constraints to the effective management of disease control in India, in misaligned incentives, and the institutional arrangements for service delivery. The authors discuss these under five headings: administrative issues; human resource management; horizontal coordination; decentralization, community involvement, and public accountability; and implementation of public health laws and regulations. They find that India's public health system is configured to be highly effective at top-down reactive work, such as bringing disease outbreaks under control, but not for the more routine collaborations required for proactive disease prevention.     Cytogenetic studies among Iranian infertile men: The first 20-year long-term report

    Chromosomal aberrations have been postulated to be one of the principal genetic factors in male infertility and occur in about 2 to 3% of unselected patients with proven sub-fertility. This rate is estimated to be 5 to 7% in patients with oligospermia, increasing to 10 to 15% in patients with azoospermia. The aim of this study was to report the frequency of all chromosomal aberrations among Iranian infertile men. In this 20-year retrospective study, we investigated 829 men which were referred to our department due to infertility. Karyotyping was performed on peripheral blood lymphocytes according to standard methods. Out of 829 patients, 557 patients (67.19%) had normal karyotype and 272 patients (32.81%) showed abnormal chromosomes. Klineferlter syndrome, found in 195 patients (23.52%), was the most frequent aberration in our study. The remaining 77 cases (9.29%) showed a variety of abnormal karyotypes.Keywords: Karyotyping, chromosome abnormality, male infertilit
