    Early Origins of Adult Cancer Risk Among Men and Women: Influence of Childhood Misfortune?

    Objective—To examine the effect of five childhood misfortune domains—parental behavior, socioeconomic status, infectious diseases, chronic diseases, and impairments—on all-site and selected site-specific cancer prevalence and all-site cancer incidence. Method—Panel data from the Health and Retirement Study (2004–2012) were used to investigate cancer risk among adults above the age of 50. Results—Risky parental behavior and impairment in childhood were associated with higher odds of all-site cancer prevalence, and childhood chronic disease was associated with prostate cancer, even after adjusting for adult health and socioeconomic factors. Moreover, having one infectious disease in childhood lowered the odds of colon cancer. Cancer trends varied by race and ethnicity, most notably, higher prostate cancer prevalence among Black men and lower all-site cancer among Hispanic adults. Discussion—These findings underscore the importance of examining multiple domains of misfortune because the type and amount of misfortune influence cancer risk in different ways

    XTE J2123-058: A New Neutron Star X-Ray Transient

    We report on optical and RXTE observations of a new high-latitude bursting X-ray transient, XTE J2123-058. We identified the optical counterpart, measured the 5.9573 hr orbital period and constrained the binary inclination and the source distance. The distance lower limit indicates that the source is at least 2.6 kpc from the Galactic plane, which is unusual for an LMXB. RXTE observations were made between June and August 1998 during the first detected outburst from this source. We describe correlations between X-ray timing and spectral properties and discuss the possibility that the propeller mechanism turned on during the decay of the outburst. During one of the RXTE observations, we detect a pair of high frequency QPOs at 847.1 +/- 5.5 Hz and 1102 +/- 13 Hz simultaneously. According to the beat frequency model, the QPO separation implies a neutron star spin period near 3.9 ms. As the X-ray flux decreases at the end of the outburst, the amplitude of the optical modulation increases significantly.     High resolution measurements of the switching current in a Josephson tunnel junction: Thermal activation and macroscopic quantum tunneling

    We have developed a scheme for a high resolution measurement of the switching current distribution of a current biased Josephson tunnel junction using a timing technique. The measurement setup is implemented such that the digital control and read-out electronics are optically decoupled from the analog bias electronics attached to the sample. We have successfully used this technique to measure the thermal activation and the macroscopic quantum tunneling of the phase in a small Josephson tunnel junction with a high experimental resolution.     Oral dosing for antenatal corticosteroids in the Rhesus macaque.

    Antenatal corticosteroids (ACS) are standard of care for women at risk of preterm delivery, although choice of drug, dose or route have not been systematically evaluated. Further, ACS are infrequently used in low resource environments where most of the mortality from prematurity occurs. We report proof of principle experiments to test betamethasone-phosphate (Beta-P) or dexamethasone-phosphate (Dex-P) given orally in comparison to the clinical treatment with the intramuscular combination drug beta-phosphate plus beta-acetate in a Rhesus Macaque model. First, we performed pharmacokinetic studies in non-pregnant monkeys to compare blood levels of the steroids using oral dosing with Beta-P, Dex-P and an effective maternal intramuscular dose of the beta-acetate component of the clinical treatment. We then evaluated maternal and fetal blood steroid levels with limited fetal sampling under ultrasound guidance in pregnant macaques. We found that oral Beta is more slowly cleared from plasma than oral Dex. The blood levels of both drugs were lower in maternal plasma of pregnant than in non-pregnant macaques. Using the pharmacokinetic data, we treated groups of 6-8 pregnant monkeys with oral Beta-P, oral Dex-P, or the maternal intramuscular clinical treatment and saline controls and measured pressure-volume curves to assess corticosteroid effects on lung maturation at 5d. Oral Beta-P improved the pressure-volume curves similarly to the clinical treatment. Oral Dex-P gave more variable and nonsignificant responses. We then compared gene expression in the fetal lung, liver and hippocampus between oral Beta-P and the clinical treatment by RNA-sequencing. The transcriptomes were largely similar with small gene expression differences in the lung and liver, and no differences in the hippocampus between the groups. As proof of principle, ACS therapy can be effective using inexpensive and widely available oral drugs. Clinical dosing strategies must carefully consider the pharmacokinetics of oral Beta-P or Dex-P to minimize fetal exposure while achieving the desired treatment responses

    Increase in Caesarean Deliveries after the Australian Private Health Insurance Incentive Policy Reforms

    AbstractBackground: The Australian Private Health Insurance Incentive (PHII) policy reforms implemented in 1997–2000 increased PHI membership in Australia by 50%. Given the higher rate of obstetric interventions in privately insured patients, the reforms may have led to an increase in surgical deliveries and deliveries with longer hospital stays. We aimed to investigate the effect of the PHII policy introduction on birth characteristics in Western Australia (WA).Methods and Findings: All 230,276 birth admissions from January 1995 to March 2004 were identified from administrative birth and hospital data-systems held by the WA Department of Health. Average quarterly birth rates after the PHII introduction were estimated and compared with expected rates had the reforms not occurred. Rate and percentage differences (including 95% confidence intervals) were estimated separately for public and private patients, by mode of delivery, and by length of stay in hospital following birth. The PHII policy introduction was associated with a 20% (221.4 to219.3) decrease in public birth rates, a 51% (45.1 to 56.4) increase in private birth rates, a 5% (25.3 to 25.1) and 8% (28.9 to 27.9) decrease in unassisted and assisted vaginal deliveries respectively, a 5% (25.3 to 25.1) increase in caesarean sections with labour and 10% (8.0 to 11.7) increase in caesarean sections without labour. Similarly, birth rates where the infant stayed 0–3 days in hospital following birth decreased by 20% (221.5 to 218.5), but rates of births with .3 days inhospital increased by 15% (12.2 to 17.1).Conclusions: Following the PHII policy implementation in Australia, births in privately insured patients, caesarean deliveries and births with longer infant hospital stays increased. The reforms may not have been beneficial for quality obstetric care in Australia or the burden of Australian hospitals

    Two Galaxy Clusters: A3565 and A3560

    We report 102 new redshifts and magnitudes for a sample of galaxies to RF ~ 15.5 mag in a 2.17 deg x 2.17 deg region centered on the galaxy IC 4296, the most luminous member of the A3565 cluster. Up to the limiting magnitude we find 29 cluster members, and measure a velocity dispersion of 228 km/s. The estimated total mass for this system is ~ 3.0 x h**-1 10**13 Msun (where h = H0/100 km/s/Mpc), and its dynamical properties are quite typical of poor clusters presenting X-ray emission. We also find that galaxies with absorption lines are more concentrated towards the center of the cluster, while systems with emission lines are mainly located in the outer parts. The small velocity dispersion of the cluster, coupled to the known presence of an interacting pair of galaxies, and the large extent of the brightest cluster galaxy, could indicate that galaxy formation through mergers may still be underway in this system. The surveyed region also contains galaxies belonging to the Shapley Concentration cluster A3560. Within 30 arc min of the cluster center, we detect 32 galaxies, for which we measure a velocity dispersion of 588 km/s and a mass of ~2 x h**-1 10**14 Msun. However, because our sample is restricted to galaxies brighter than M*, these values should be considered only as rough estimates.Comment: 33 pages, including 6 tables and 9 postscript figures.
