Ezetimibe: New perspectives in lipid lowering treatment
Therapeutic goals for lipid lowering treatment in the prevention of ischemic heart disease are
often not reached in clinical practice. Even the highest doses of statins do not guarantee good
control of hypercholesterolemia in all patients. Therefore, new lipid lowering drugs are being
investigated. One of them is ezetimibe - intestinal cholesterol absorption inhibitor. Treatment
with ezetimibe results in significant reduction of total cholesterol and LDL cholesterol levels.
It is hoped that concomitant treatment with ezetimibe and other lipid lowering drugs (particularly
statins) will be more effective. In large randomized clinical trials, co-administration of
ezetimibe with atorvastatin and simvastatin proved to be more effective in lowering cholesterol
levels and reaching target therapeutic levels than treatment with statin alone. In addition,
combined treatment with ezetimibe and simvastatin was more effective compared to treatment
with today’s most effectively used statin (rosuvastatin) alone. Reduction of statin dose, due to
the combined treatment with ezetimibe, lowers the risk of adverse events. Results of the published
studies suggest that ezetimibe is safe and when administered together with statin does
not increase the risk of liver or muscle damage. Treatment with ezetimibe may be regarded as
a new option in the management of patients who need lipid lowering treatment. (Cardiol J

Association of free testosterone and sex hormone binding globulin with metabolic syndrome and subclinical atherosclerosis but not blood pressure in hypertensive perimenopausal women
Introduction: Data on the role of androgens as potential mediators of increasing cardiovascular risk in women at midlife are controversial. The aim
of the study was to analyze the relationship of free testosterone (FT) and sex
hormone binding globulin (SHBG) with blood pressure and subclinical organ
damage and metabolic syndrome (MS) in middle aged hypertensive women.
Material and methods: One hundred and fifty-two women with newly diagnosed arterial hypertension were included in the study. In all subjects blood
pressure measurements were performed as well as echocardiographic examination with left ventricular structure and function assessment (GE Vivid
7.0), carotid ultrasound with measurement of intima-media thickness (IMT),
and carotid-femoral pulse wave velocity (PWV) measurement (Sphygmocor).
A fasting blood sample was taken to measure glucose and lipid concentrations. Serum testosterone and SHBG were measured. Free testosterone was
calculated according to the Vermeulen formula. Metabolic syndrome was defined following the International Diabetes Federation (IDF) recommendations.
Results: Free testosterone was significantly higher and SHBG lower in women with MS independently of menopausal status. The odds ratio of MS per
quartile increment in FT after adjustment for covariates was 2.06 (95% CI:
1.16–3.65). There was no correlation between FT, SHBG and blood pressure.
Free testosterone was associated with decreased left ventricular diastolic
function (E/A ratio β = –0.19, p = 0.05) and subclinical atherosclerosis (IMT
β = 0.34, p = 0.009), but not arterial stiffness.
Conclusions: Free testosterone and SHBG independently of menopause status are related to MS. Free testosterone is associated with worse metabolic
profile, subclinical atherosclerosis and impaired diastolic function of the left
ventricl
Valsartan - its antihypertensive efficacy, tolerance and impact on the quality of life
Jednym z warunków skutecznego leczenia nadciśnienia
tętniczego jest uzyskanie równowagi pomiędzy
kontrolą ciśnienia, redukcją ryzyka sercowo-naczyniowego a tolerancją zastosowanego leczenia
i jego wpływem na jakość życia chorych. Wykazano,
że jakość życia chorych z nadciśnieniem tętniczym
jest niższa niż osób zdrowych, a ponadto niższa
u kobiet niż mężczyzn. Wdrożenie farmakoterapii
nadciśnienia tętniczego powinno przyczyniać się do
długotrwałej poprawy jakości życia, co pozwala na
utrzymanie dobrej współpracy pacjenta w czasie leczenia.
Jedną z klas leków przeciwnadciśnieniowych
bardzo dobrze tolerowanych przez pacjentów są antagoniści
receptora angiotensyny II. Pośród nich walsartan
jest lekiem bezpiecznym i skutecznym, zarówno
w monoterapii, jak i w leczeniu skojarzonym.
Walsartan powoduje całodobową kontrolę ciśnienia
tętniczego, bez istotnych działań niepożądanych.
Przyczynia się również do poprawy funkcjonowania
w zakresie ważnych sfer życia pacjentów z nadciśnieniem
tętniczym, co prowadzi do poprawy jakości
życia, zarówno mężczyzn, jak i kobiet.One of the main conditions essential for an effective treatment
of hypertension is obtaining an equilibrium between
blood pressure control, cardiovascular risk reduction, tolerance
of treatment and its impact on the patient’s quality of
life. It was shown that in hypertensive subjects the quality of
life is lower than in healthy subjects and it is particularly low
in hypertensive women as compared with men. Pharmacotherapy
of hypertension should contribute to a lasting improvement
in the patient’s quality of life, which in consequence
leads to an improved compliance with treatment. Angiotensin
II receptor antagonists are among well tolerated
antihypertensive drug classes. One of these drugs, valsartan, is
a safe and effective agent, both when used in monotherapy or
in combined therapy. Valsartan allows twenty-four hour blood
pressure control, without important adverse events. It also
contributes to a better functioning of hypertensive patients’ in
important aspects of their lives, which translates itself into an
improved quality of life, both in men and in women

Wstęp Celem pracy była ocena wpływu eprosartanu
stosowanego w monoterapii lub w leczeniu skojarzonym
na ciśnienie tętnicze oraz jakość życia chorych
na nadciśnienie tętnicze.
Materiał i metody Do badania włączono 85 osób
z łagodnym lub umiarkowanym nadciśnieniem tętniczym
- 47 mężczyzn i 38 kobiet w wieku 45,1 ±
13,9 roku. Wśród badanych 16 osób (18,6%) wyjściowo
nie otrzymywało dotąd leku obniżającego
ciśnienie tętnicze, a 70 osób (81,4%) stosowało monoterapię:
diuretykiem tiazydopodobnym (14 osób),
b-adrenolitykiem (13 osób), inhibitorem ACE
(20 osób), antagonistą wapnia (18 osób), a-adrenolitykiem
(2 osoby) i lekiem działającym centralnie
(3 osoby).
Pomiarów ciśnienia tętniczego i tętna dokonywano
na każdej wizycie, co 4 tygodnie przez 16 tygodni.
W leczeniu stosowano metanosulfonian eprosartanu
w dawce 600 mg dziennie. Jeśli nie uzyskano kontroli
ciśnienia (> 140/90 mm Hg), po 8 tygodniach
do leczenia dołączano hydrochlorotiazyd 12,5 mg raz
dziennie. Jakość życia oceniono dwukrotnie: wyjściowo
oraz po 16 tygodniach za pomocą kwestionariusza
- Psychologiczny Wskaźnik Dobrego Samopoczucia
(PGWB).
Wyniki Po 16 tygodniach stosowania eprosartanu
w całej grupie stwierdzono redukcję ciśnienia tętniczego
(154,0/99,2 vs. 129,4/85,1 mm Hg, p <
0,01) u mężczyzn i u kobiet. Dodatkowego leczenia
hydrochlorotiazydem wymagało 14 chorych
(16,5%). Największą redukcję ciśnienia tętniczego
uzyskano w grupach pacjentów leczonych przy
użyciu leków skojarzonych: eprosartanu z diuretykiem
tiazydopodobnym, eprosartanu z antagonistą
wapnia oraz eprosartanu z inhibitorem ACE.
Po 16 tygodniach uzyskano normalizację ciśnienia
tętniczego u wszystkich chorych oraz stwierdzono
poprawę jakości życia leczonych (PGWB 88,4 ±
9,1 vs. 101,4 ± 10,3 punktów, p < 0,01). Na końcu
badania uzyskano poprawę w zakresie wszystkich
podskal kwestionariusza PGWB, z wyjątkiem
skali „samokontrola”, zarówno u mężczyzn,
jak i u kobiet.
Wnioski Eprosartan stosowany w monoterapii lub
leczeniu skojarzonym nadciśnienia tętniczego jest
dobrze tolerowany i poprawia jakość życia leczonych
- mężczyzn i kobiet. Poprawa jakości życia podczas
leczenia eprosartanem wiąże się z normalizacją ciśnienia
tętniczego.Background The goal of the study was to evaluate antihypertensive
efficacy and the quality of life (QoL) in hypertensive
patients treated with an angiotensin II receptor antagonist
- eprosartan, both in mono- or in combination therapy.
Material and methods The study population consisted of
85 patients with mild to moderate essential hypertension
(47 men and 38 women), age 45.1 ± 13.9 yrs. Mean SBP
was 154.0 mm Hg and DBP was 99.2 mm Hg. At baseline
16 patients (18.6%) were untreated, and 70 (81.4%) were
treated with monotherapy: thiazide-like diuretic (14 pts),
beta-blocker (13 pts), ACE inhibitor (20 pts), calcium antagonist
(18 pts), alpha-blocker (2 pts), centrally acting
drug (3 pts).
BP and heart rate were measured at baseline and on each
control visit (every 4 weeks for 16 weeks). In all patients
eprosartan 600 mg once daily was used. In patients who
did not reach BP control (BP < 140/90 mm Hg) after
8 weeks, hydrochlorothiazide (HCTZ) 12.5 mg daily was
added. QoL was evaluated at baseline and after 16 weeks
by the Psychological General Well-being Index questionnaire
(PGWB).
Results After 16 weeks of treatment with eprosartan. BP
was significantly decreased (154.0/99.2 vs. 129.4/85.1 mm
Hg, p < 0.01), both in men and in women. At the end of
the study in all patients blood pressure control was obtained.
HCTZ was added to 14 patients (16.5%) during
the study. Most pronounced reduction of BP was observed
in patients treated with eprosartan + thiazide-like diuretic,
eprosartan + calcium antagonist and eprosartan +
ACE inhibitor. After 16 weeks significant improvement of
QoL (PGWB 88.4 ± 9,1 vs. 101.4 ± 10,3 points, p < 0.01) was observed. The total score of the PGWB and its
subscales improved significantly in both sexes, except of
“self-control” subscale.
Conclusions Eprosartan provides good antihypertensive
efficacy and significantly improves quality of life, irrespective
of gender. Higher QoL during treatment with
eprosartan is strictly related to better blood pressure control
Arterial Compliance in Arterial Hypertension. From Pathophysiology to Clinical Relevance
Termin „podatność tętnic” funkcjonuje jako pojęcie w patofizjologii układu krążenia od początku XVII wieku. Przez długie lata jej znaczenie pozostawało niedoceniane ze względu na panujący pogląd, że podstawową rolę w rozwoju nadciśnienia tętniczego odgrywa opór systemowy, a będąca jego odbiciem wysokość ciśnienia rozkurczowego
jest główną determinantą wystąpienia powikłań sercowo-naczyniowych. Pogląd ten uległ radykalnej zmianie w ciągu ostatnich 10 lat w związku z opublikowaniem wyników dużych randomizowanych badań osób z izolowanym
nadciśnieniem skurczowym. W grupie wiekowej powyżej 55 rż. najsilniejszym wyznacznikiem ryzyka okazało się ciśnienie tętna, będące pośrednim wskaźnikiem sztywności tętnic.
Przeniosło to zainteresowanie badaczy z oporu naczyniowego na własności sprężyste tętnic. W artykule przedstawiono podstawy morfologiczne i patofizjologiczne procesów związanych ze zmianami własności sprężystych tętnic (sztywnienie). Zdefiniowano
podstawowe wskaźniki wyrażające podatność tętnic i opisujący je aparat matematyczny. Szczególną uwagę poświęcono następstwom wzrostu sztywności tętnic dla patologii
serca oraz klinicznemu znaczeniu badania jej parametrów. Przedstawiono bieżący stan wiedzy na temat związków między podatnością tętnic a istotniejszymi czynnikami, takimi jak: płeć, starzenie się, nadciśnienie tętnicze, zaburzenia regulacji wewnątrzwydzielniczej i metabolizmu oraz możliwości ingerencji terapeutycznej w podatność tętnic.Arterial compliance as a term has been known in the pathophysiology
of the cardiovascular system since the beginning of the 17th century. For many years it has been underrated because of the predominant view that the development of arterial hypertension is determined by the systemic resistance, which is reflected in diastolic blood pressure level, the
main factor for cardiovascular complications.
This view has been modified in the recent decade after the publication of the results of large randomized trials in patients with isolated systolic hypertension. In patients over
55 years of age blood pressure, an indirect marker of arterial stiffness has been found to be the strongest risk factor. For this reason the focus of interest has been shifted from vascular
resistance to elastic properties of the arteries.
The present paper discusses the basics of morphology and pathophysiology of changes in the elastic properties of the
arteries (stiffening), defines basic indices of arterial compliance and mathematical background. A chapter will be devoted to the effects of increased arterial stiffness on heart pathology and clinical implications of measuring this parameter. A state-of-the-art section will describe the relationship
between arterial compliance and several physiological and pathological factors (sex, ageing, arterial hypertension,
endocrine and metabolic dysfunction) as well as therapeutic interference with arterial compliance
