    A middle-aged woman was admitted with a diagnosis of liver cysts. The patient was symptomatic and was treated by injecting absolute ethanol into the largest cyst every week, but the secretion from the cyst persisted. The patient was then treated by absolute ethanol injection every day with good results. CA 19-9 was measured in the cystic fluid. The secretion was related to CA 19-9 activity

    Dental metal allergyand dental focal infection are possible causes of dermatological diseases, but have been the subjects of few reports to date. We have been treating such patients in our special clinicfor more than 20 years.The purpose of the present study was to investigate the mouths of patients visitingour dental hospital over an 8-year period, with the aim of clarifyingwhether dental metal allergy andjor dental focal infection affects their dermatologic conditions.We surveyed all clinicalrecords of the 185 patients who visited Niigata UniversityMedicaland Dental Hospitalwith chiefcomplaints of dental metal allergysince 2002. Diagnosticsof skin diseases, periodontal records, periapical lesions, dental caries, dental metal series patch test results and Electron Probed Micro-Analysis(EPMA)data were investigated. Ninety-two(49%) patients were sufferingfrom pustulosis palmaris et plantaris and 20 (11%)patients had lichen planus. Eighty-two(49%)patients showed positive reactions on patch testing. Based on the result of patch tests, Nishowed the highest positivity rate (62%,51 patients), but on EPMA,the number of patients with Ni as an allergen was 14 (27%).On the other hand, more than 98%of patients who showed positive reactions on patch test to Pd and Au had these metals in their dental prostheses. In addition, 112 (60%)patients showed the possibilityof dental focal infections

    Review Article Immunogenetic Heterogeneity of Type 1 Diabetes in Japan

    Type 1 diabetes is an organ-specific autoimmune disease characterized by T-cell mediated destruction of pancreatic ホイ-cells . In Japanese population, the incidence of type 1 diabetes in children is very low compared to European countries. However, there are more patients with type 1 diabetes in adults, including latent autoimmune diabetes in adults (LADA). A variety of environmental and genetic factors are involved in the development of the disease. The human leukocyte antigen (HLA) class II genes (termed IDDM1) are the major genes associated with susceptibility to type 1 diabetes. HLA-DRB1*0405-DQB1*0401, HLA-DRB1* 0901-DQB1*0303 and HLA-DRB1*0802-DQB1*0302 are three major haplotypes in Japanese patients with type 1 diabetes. Other genetic factors reported in Japanese type 1 diabetes include the polymorphisms in insulin gene (IDDM2), CTLA-4 gene (IDDM12), MICA gene, Neuro D/Beta 2 gene, and IL-10 gene. The circulating autoantibodies to multiple islet autoantigens including GAD, insulin, and IA-2 are the important immunological features of type 1 diabetes. The prevalences of anti-islet autoantibodies in patients with Japanese type 1 diabetes are 60-70% for GAD autoantibodies, 45-50% for insulin autoantibodies (IAA), and 60-65% for IA-2 autoantibodies at disease onset, which are similar to those reported in Caucasian patients. With combinatorial analysis of these autoantibodies ninety percent of patients express at least one of these autoantibodies and are classified as immune-mediated type 1 diabetes. Although the majority of patients with type 1 diabetes are young, lean, and ketosisprone, there are number of patients with type 1 diabetes initially diagnosed as having type 2 diabetes at disease onset. These slow-onset diabetic patients with anti-islet autoantibodies often progress toward insulin-deficient state within several years after diagnosis. High level of GAD autoantibodies has a high predictive value for future insulin deficiency in slow-onset patients with type 1 diabetes. In conclusion, Japanese patients with type 1 diabetes are clinically heterogenous and the determination of immunological and genetic features are helpful to clarify the characteristics of the Japanese type 1 diabetic syndrome

    磁気共鳴は医学におけるイメージングにおいて重要なモダリティーであり,高度な数学的手法が画像の再構成に取り入れられている。画像再構成の数学的手法および画像再構成にいたるまでの数学的な過程を理解するこは,MRIを学ぶものにとって非常に重要であるが,成書において明確な説明はなされていない。ここでは,MR画像の再構成法を,複素関数と複素フ-リェ係数を用いて導いたので報告する。Magnetic resonance is an important modality for medical imaging, and advanced mathematical techniques are introduced into the reconstruction of images. Clearly understandable explanations of the reconstruction of the MR image and of the mathematical derivation process have not been presented in the texts. Reconstruction of MR image is presented using the complex function and its relevant Fourier coefficients

    Measurement of sampling time in MRI and reconstruction of MR image

    MRIにおいてk-spaceを構築するためにはエコー信号をサンプリング時間に従って採取する必要がある.このサンプリング時間を測定するために,FOV(field of view)の左半分にファントムを置くことにより階段関数となるスピン分布を生成した。得られたエコー信号を,階段関数をフ-リェ変換したときに得られるフ-リェ係数と比較することによりサンプリング時間を求めた。得られたサンプリング時間を用いて,MRI MULTI-TEST PHANTOMをMRI装置でスキャンすることにより得られたエコー信号をサンプリングしk-spaceを構築した。このk-spaceをフ-リェ逆変換することによりMRI MULTI-TEST PHANTOMの画像を再構成することができた。Regarding the construction of k-space in MRI, it is necessary to extract the required data from the echo signals with the rate of sampling time. In order to measure the sampling time, a rectangular phantom was placed on the left side of the FOV in the body coil of an MRI equipment, and got analog signals in the sequence of spin echo. The sampling time was determined by comparing the measured signals with the Fourier coefficients obtained by transforming the step function which was similar to the profile of the FOV. The MRI MULTI-TEST PHANTOM was scanned, and the k-space was constructed with the extracted data from the echo signals with the rate of the determined sampling time. The MRI images of the MULTI-TEST PHANTOM were reconstructed by inverse-transforming the obtained k-space

    The relationship between auditory ERP and neuropsychological assessments in schizophrenia

    金沢大学附属病院To clarify the cognitive significance of event-related potential (ERP) abnormalities in schizophrenia, we examined the relationships of amplitudes and latencies of ERP components with performance on neuropsychological tests in schizophrenic patients. Twenty patients underwent the Trail Making B Test (TM-B), which is sensitive to frontal lobe dysfunction, and the logical memory, verbal paired-association, and visual reproduction subtests of the Wechsler Memory Scale (WMS), which are sensitive to temporal lobe dysfunctions, and ERP recordings during performance of an oddball auditory discrimination task. Pearson product–moment correlations indicated that an increased P200 amplitude was correlated with poor performance on the TM-B, whereas a decreased P300 amplitude was correlated with poor performance on the verbal paired-association subtest of the WMS. These findings suggest that a P200 abnormality represents the frontal lobe dysfunction, and a P300 abnormality represents the left temporal lobe dysfunction in schizophrenia
