    New graduate doctors' preparedness for practice: A multistakeholder, multicentre narrative study

Thus, no additional data are available.Objective While previous studies have begun to explore newly graduated junior doctors' preparedness for practice, findings are largely based on simplistic survey data or perceptions of newly graduated junior doctors and their clinical supervisors alone. This study explores, in a deeper manner, multiple stakeholders' conceptualisations of what it means to be prepared for practice and their perceptions about newly graduated junior doctors' preparedness (or unpreparedness) using innovative qualitative methods. Design A multistakeholder, multicentre qualitative study including narrative interviews and longitudinal audio diaries. Setting Four UK settings: England, Northern Ireland, Scotland and Wales. Participants Eight stakeholder groups comprising n=185 participants engaged in 101 narrative interviews (27 group and 84 individual). Twenty-six junior doctors in their first year postgraduation also provided audio diaries over a 3-month period. Results We identified 2186 narratives across all participants (506 classified as 'prepared', 663 as 'unprepared', 951 as 'general'). Seven themes were identified; this paper focuses on two themes pertinent to our research questions: (1) explicit conceptualisations of preparedness for practice; and (2) newly graduated junior doctors' preparedness for the General Medical Council's (GMC) outcomes for graduates. Stakeholders' conceptualisations of preparedness for practice included short-term (hitting the ground running) and long-term preparedness, alongside being prepared for practical and emotional aspects. Stakeholders' perceptions of medical graduates' preparedness for practice varied across different GMC outcomes for graduates (eg, Doctor as Scholar and Scientist, as Practitioner, as Professional) and across stakeholders (eg, newly graduated doctors sometimes perceived themselves as prepared but others did not). Conclusion Our narrative findings highlight the complexities and nuances surrounding new medical graduates' preparedness for practice. We encourage stakeholders to develop a shared understanding (and realistic expectations) of new medical graduates' preparedness.     Optic neuritis is associated with inner nuclear layer thickening and microcystic macular edema independently of multiple sclerosis

    BACKGROUND: Microcystic macular edema (MME) and inner nuclear layer thickening (INL) were described in multiple sclerosis (MS) and neuromyelitis optica (NMO) patients using optical coherence tomography (OCT). The cause of these findings is currently unknown and a relation to inflammatory or degenerative processes in the optic nerve is discussed. OBJECTIVE: The aim of our study was to investigate whether INL thickening and MME are related to optic neuritis (ON) in various neuro-inflammatory disorders causingON: MS, NMO and chronic inflammatory optic neuropathy. METHODS: We retrospectively analyzed data from 216 MS patients, 39 patients with a clinically isolated syndrome, 20 NMO spectrum disorder patients, 9 patients with chronic inflammatory optic neuropathy and 121 healthy subjects. Intra-retinal layer segmentation was performed for the eyes of patients with unilateral ON. Scanning laser ophthalmoscopy (SLO) images were reviewed for characteristic ocular fundus changes. RESULTS: Intra-retinal layer segmentation showed that eyes with a history of ON displayed MME independent INL thickening compared to contralateral eyes without previous ON. MME was detected in 22 eyes from 15 patients (5.3% of all screened patients), including 7 patients with bilateral edema. Of these, 21 had a prior history of ON (95%). The SLO images of all 22 MME-affected eyes showed crescent-shaped texture changes which were visible in the perifoveal region. A second grader who was blinded to the results of the OCT classified all SLO images for the presence of these characteristic fundus changes. All MME eyes were correctly classified (sensitivity = 100%) with high specificity (95.2%). CONCLUSION: This study shows that both MME and INL thickening occur in various neuro-inflammatory disorders associated with ON. We also demonstrate that detection and analysis of MME by OCT is not limited to B-scans, but also possible using SLO images

    Entwicklung von Strukturmodellen für Hydroxy-Interkalierte Tonminerale zur quantitativen Rietveld-basierten Phasenanalyse von Böden

    Chemische und physikalische Bodeneigenschaften werden entscheidend vom Tongehalt und der Zusammensetzung der Tonfraktion geprägt. Tonminerale pedogenen Ursprungs können dabei sehr komplex sein. Insbesondere unter sauren Bedingungen kann gelöstes Al in den Zwischenschichten quellfähiger 2:1 Tonminerale (Smectit, Vermiculit) durch die Bildung nicht austauschbarer Al-Polymere gebunden werden. So kann eine Reihe sogenannter Hydroxy-Interkalierter Minerale (HIM) entstehen. Gegenwärtig entziehen sich diese Phasen jedoch einer verlässlichen Quantifizierung mittels Röntgenbeugung und Rietveld-Analyse, da hierfür keine kristallografischen Strukturmodelle vorliegen. Ziel unseres Projektes ist daher die Entwicklung von Strukturmodellen für die quantitative Beschreibung pedogener HIM-Phasen, welche folgende Schritte umfasst: (1) Identifizierung wichtiger natürlich vorkommender HIM in Böden unterschiedlichen Verwitterungsgrades, (2) Abtrennung/Herstellung phasenreiner Proben zur Prüfung/Verbesserung des Strukturmodells, sowie (3) Überprüfung des Strukturmodells an natürlichen Proben (Phasengemischen). Die Identifizierung typischer HIM wurde anhand einer 120.000 Jahre alten Bodenchronosequenz (Franz-Josef Gletscher, Neuseeland) vorgenommen. Hierbei konnten diverse intermediäre Phasen wie z.B. Vermiculit, Chlorit und Chlorit-Hydroxy-Interkalierter Smectit identifiziert werden, weshalb Hydroxy-Interkalierter Smectit (HIS) als erstes zu erstellendes Strukturmodell ausgewählt wurde. Da sich HIM generell im gleichen Korngrößenbereich anderer Tonminerale befinden, bzw. mit diesen verwachsen sind, können sie nicht phasenrein abgetrennt werden, wie für den zweiten Schritt der Modellentwicklung notwendig. Aus diesem Grund wurde im Labor eine Probenserie von 7 HIS aus natürlichem Smectit (Bentonit von Milos, Griechenland) mit unterschiedlichem Grad der Al-Belegung (0-100%) hergestellt. Hierfür wurde zu 6 Teilproben 0,1 mol/L AlCl3 gegeben und mit 0,3 mol/L NaOH Lösung bis pH=5,5 titriert. Die Teilproben unterschieden sich anhand der Volumina 0,1 mol/L AlCl3 Lösung sowie den Reaktionszeiten. Eine siebte Teilprobe wurde nicht behandelt (0% Belegung). Alle Proben wurden mittels Multimethodeneinsatz charakterisiert. Der vorliegende Beitrag gibt einen Einblick in den Ablauf und die damit verbundenen Schwierigkeiten in der Entwicklung eines Strukturmodells für HIM, welche eine quantitative Beschreibung von Hydroxy-Interkalierten Tonmineralen in Böden zukünftig ermöglichen soll

    In this work, we obtain bound states for a nonrelativistic spin-half neutral particle under the influence of a Coulomb-like potential induced by the Lorentz symmetry breaking effects. We present a new possible scenario of studying the Lorentz symmetry breaking effects on a nonrelativistic quantum system defined by a fixed space-like vector field parallel to the radial direction interacting with a uniform magnetic field along the z-axis.     Can the stroma provide the clue to the cellular basis for mammographic density?

    Mammographic density is recognised as a useful phenotypic biomarker of breast cancer risk. Deeper understanding is needed of the cellular basis, but evidence is limited because of difficulty in designing studies to validate hypotheses. The ductal epithelial components do not adequately explain the physical and dynamic features observed. The stroma is thought to interact with ductal structures in cancer initiation. Stromal tissues might account for the mammographic features, and this interplay can be hypothesised to relate risk to density. In a paper in this issue of Breast Cancer Research, Alowami has shown a relationship between density and stromal proteins, which might provide useful insight into mammographic density

    NetMets: software for quantifying and visualizing errors in biological network segmentation

    One of the major goals in biomedical image processing is accurate segmentation of networks embedded in volumetric data sets. Biological networks are composed of a meshwork of thin filaments that span large volumes of tissue. Examples of these structures include neurons and microvasculature, which can take the form of both hierarchical trees and fully connected networks, depending on the imaging modality and resolution. Network function depends on both the geometric structure and connectivity. Therefore, there is considerable demand for algorithms that segment biological networks embedded in three-dimensional data. While a large number of tracking and segmentation algorithms have been published, most of these do not generalize well across data sets. One of the major reasons for the lack of general-purpose algorithms is the limited availability of metrics that can be used to quantitatively compare their effectiveness against a pre-constructed ground-truth. In this paper, we propose a robust metric for measuring and visualizing the differences between network models. Our algorithm takes into account both geometry and connectivity to measure network similarity. These metrics are then mapped back onto an explicit model for visualization

    Investigating Tissue Optical Properties and Texture Descriptors of the Retina in Patients with Multiple Sclerosis

    PURPOSE: To assess the differences in texture descriptors and optical properties of retinal tissue layers in patients with multiple sclerosis (MS) and to evaluate their usefulness in the detection of neurodegenerative changes using optical coherence tomography (OCT) image segmentation. PATIENTS AND METHODS: 38 patients with MS were examined using Stratus OCT. The raw macular OCT data were exported and processed using OCTRIMA software. The enrolled eyes were divided into two groups, based on the presence of optic neuritis (ON) in the history (MSON+ group, n = 36 and MSON- group, n = 31). Data of 29 eyes of 24 healthy subjects (H) were used as controls. A total of seven intraretinal layers were segmented and thickness as well as optical parameters such as contrast, fractal dimension, layer index and total reflectance were measured. Mixed-model ANOVA analysis was used for statistical comparisons. RESULTS: Significant thinning of the retinal nerve fiber layer (RNFL), ganglion cell/inner plexiform layer complex (GCL+IPL) and ganglion cell complex (GCC, RNFL+GCL+IPL) was observed between study groups in all comparisons. Significant difference was found in contrast in the RNFL, GCL+IPL, GCC, inner nuclear layer (INL) and outer plexiform layer when comparing MSON+ to the other groups. Higher fractal dimension values were observed in GCL+IPL and INL layers when comparing H vs. MSON+ groups. A significant difference was found in layer index in the RNFL, GCL+IPL and GCC layers in all comparisons. A significant difference was observed in total reflectance in the RNFL, GCL+IPL and GCC layers between the three examination groups. CONCLUSION: Texture and optical properties of the retinal tissue undergo pronounced changes in MS even without optic neuritis. Our results may help to further improve the diagnostic efficacy of OCT in MS and neurodegeneration
