    Correlates of mothers' use of medications for their children

    This study examines the extent, determinants and quality of mothers' independent use of medications for treating their children's symptoms. Data on mother-initiated medication behavior (MIMB) were obtained from a stratified systematic random sample of 500 mothers of children presenting for a well child visit at two pediatric ambulatory care sites. Six expert pediatric judges each rated every reported medication use (N = 3908) along three dimensions (usefulness, correctness and harmfulness or helpfulness) and also evaluated the overall appropriateness of each mother's MIMB. Results indicate that: (1) mothers keep available and use for children a considerable variety of different medications and medical appliances; (2) a positive linear relationship exists between mothers' socioeconomic status (SES) and degree of MIMB; (3) mothers' attitudes toward medications and toward their children's health are associated with the number of categories of medications and appliances they possess and use; (4) the combination of SES and attitudinal characteristics accounts for substantial portions of the variance in MIMB; and (5) judges' ratings show only minimal-level support of MIMB (extending to mothers in all SES groups) and are linearly related to SES (P < 0.001).     We present the case of a patient, with previously unknown liver metastases, presenting with a liver abscess and Clostridium septicum septicaemia. C. septicum is known to be associated with both malignancy and immunosuppression and therefore in patients where this organism is isolated, efforts must be made to exclude an occult underlying malignancy or haematological disorder

    Fulminant Clostridium Septicum myonecrosis in well controlled diabetes: a case report

    Diabetic myonecrosis with Clostridium Septicum is uncommon but carries a high mortality rate. This commensal organism is part of the gastrointestinal tract flora and can become extremely virulent, often in the setting of immuno-suppression such as neutropenia, occult malignancy (commonly caecal) and poorly controlled diabetes. The case report is unusual in that there are few risk factors other than very mild neutropenia. This highlights the opportunistic character of the organism and recommends that a high index of suspicion and vigilance be carried out in the presence of fevers and sepsis, even in the well-controlled diabetic population

    Preoperative predictors of sternotomy need in mediastinal goiter management

    BACKGROUND: The objective of this study was to identify the preoperative risk factors for patients in need of a sternotomy in the management of mediastinal goiters in order to provide better preoperative planning and patient consent. METHODS: We analyzed 98 patients who underwent surgery for mediastinal goiters (goiters extending below the thoracic inlet > or =3 cm with the neck in hyperextension) between 1995 and 2008. Twelve (12.2%) of the patients required a sternotomy. The patients' features were analyzed by the surgical approach performed. Logistic regression analysis was used to study which variables were influencing the surgical strategy. The receiver operating characteristic (ROC) curves were designed when appropriate. RESULTS: The analysis disclosed the following risk factors: radiologic extension of mediastinal goiters below the aortic arch (odds ratio [OR] = 32.87; 95% confidence interval [CI] = 4.04-267.12; p 160 months: OR = 22.8; 95% CI = 5.28-98.53; p < .0001). CONCLUSIONS: Sternotomy need for mediastinal goiter removal can be predicted; in such cases surgeons should not hesitate to perform it for minimizing complications

    Current workout measurement devices such as pedometers are limited. There are inexpensive and easy to use solutions offering low accuracy, and accurate yet expensive designs. This project aims to produce a device fitting in the middle of the cost versus accuracy spectrum. The project design consists of a hardware prototype as well as final design with software. An accelerometer is used as the sensor in this device so the cost can be minimized while accurate distance measurements are recorded. This device will be worn on a user’s shoe, possibly integrated into a shoe itself in the future. If a device with these characteristics is offered, a user will have a better product which is simple to use and low in price. This report serves to outline the project development from design background to parts selection and the development and final device designs. By using a simple microcontroller connected to an accelerometer and display, the device is able to recognize and time strides with high accuracy and translate these values to a corresponding distance measurement
