    Comparison of high -performance thin layer chromatography/UV-densitometry and UV-derivative spectrophotometry for determination of trimetazidine in pharmaceutical formulation

    New methods for assaying trimetazidine dihydrochloride on the basis of thin layer chromatography and spectrophotometry are proposed and compared in the paper. In HPTLC/UV-densitometry, separation is achieved by using a mobile phase composed of ammonia-methanol (30:70, V/V) on silica gel HPTLC plates F254. Quantification using a non-linear calibration curve is accomplished by densitometric detection at 230 nm. Derivative spectrophotometric determination of trimetazidine dihydrochloride is carried out from the fourth derivative of the absorbance at 233 nm in peak-zero mode. Statistical comparison led to the conclusion that there is no significant difference between the two studied methods and, moreover, that they demonstrate satisfactory accuracy and precision for routine applications

    Herbal medicine has accompanied man since the dawn of time. For various ailments and health problems, help was sought in the surrounding nature. Ancient civilizations have greatly contributed to the development of phytotherapy, identifying and describing numerous species of medicinal plants. Plant raw materials have become the foundation of medieval medicine. The revival in herbal medicine has been observed since the sixth century with the emergence of monasteries, where the gardens in which the medicinal plants were grown were established. Monastic schools were also established, monks broadened their skills by studying and copying ancient books, preparing medicines of natural origin according to secret prescriptions used to heal the sick. Although the Middle Ages are often considered dark ages, many achievements of the then phytotherapy have been recorded on the pages of history and a man use them to this day

Chora zosta艂a poddana leczeniu operacyjnemu drog膮 trepanacji otworkowej. Przebieg oko艂ooperacyjny by艂 niepowik艂any. Dwa tygodnie po zabiegu zaobserwowano popraw臋 w zakresie funkcji poznawczych. Opisany przypadek dowodzi znaczenia, jakie przewlek艂e krwiaki podtward贸wkowe odgrywaj膮 w艣r贸d chorych leczonych na oddzia艂ach geriatrycznych.聽S艂owa kluczowe: przewlek艂y krwiak podtward贸wkowy, ot臋pienie, geriatria聽聽聽AbstractChronic subdural hematomas (CSDH) are typical for elderly people, its symptoms can be cognitive impairment, which in older patients require differentiation of the progressive dementia. In this publication, we presented a case of 85-year-old patient admitted to the Department of Geriatrics of CM UMK in Bydgoszcz with symptoms of progressive dementia. Psychological tests showed significant cognitive impairment. The CT scan revealed the presence of extensive CSH. The patient underwent surgical treatment through burrhole. Perioperatively no complitations happened. Two weeks later improvement in cognitive function was observed. This case improved the importance of CSDH in geriatrics.聽Key words: CSDH, dementia, geriatric

    A聽massive gross haematuria in an 85-year-old patient with acquired haemophilia A

    The authors herein present a聽case report of an 85-year-old female patient with massive gross haematuria in the course of acquired haemophilia A. Acquired haemophilia in the geriatric population is a聽rare but severe bleeding disorder. It is caused by suddenly appearing autoantibodies that interfere with coagulation factor VIII activity. The diagnosis of acquired haemophilia should be considered in any patient who presents with bleeding and a聽prolonged activated partial thromboplastin time (APTT)

    Results of conservative, surgical treatment and rehabilitation of entrapment neuropathies in elderly patients in geriatric practice

    Entrapment neuropahies of upper limbs can cause mainly nocturnal neuropathic pain and impaired manual dexterity. The most common entrapment neuropathy is carpal tunnel syndrome and ulnar groove syndrome - ulnar nerve entrapment at the elbow. Treatment of entrapment neuropathies is both analgetic therapy and physiotherapy. In the cases of conservative treatment inefficiencies surgical decompression of nerves is performed. Authors of this oublication present results of both conservative聽 and surgical of entrapment neuropathies in patients over 65 years old. Among the 17 patients with entraoment neuropathies 12 of them underwent surgical treatment. Achieved partial improvement in pain, mostly nocturnal and improving the quality of life and dexterity. In comparison, however, a group of younger patients who underwent surgery for the improvement was less spectacular, which may testify advancement and irreversibility of changes in older patients

Patients showed a weight loss during the last year, signs of depression in CES-D scale, gait downturn, muscle weakness and severe back pain - 9 VAS. By imaging study the spinal fractures were diagnosed and patients underwent vertebroplasty obtaining good recovery in pain. Then patients have been implemented rehabilitation and high-protein diet. After about 3 months for each patient a significant clinical improvement was achieved. Patients no longer meet the Fried鈥檚 criteria for frailty. This proves that this complex and often multi-disciplinary proceeding is beneficial for the treatment of elderly patients with frailty syndrome.聽Key words: frailty, spine copression fracture, diet, rehabilitaion, vertebroplast

    Assessment of the prevalence of nootropic drugs by elderly patients in geriatric practice

    So-called nootropics include a diverse group of drugs whose idea is to improve brain function. They are quite commonly accepted among the elderly. Although currently available data from the literature do not show unequivocally the efficacy of these drugs, in practice a large number of patients are in practice. These drugs are prescribed to patients fairly commonly and the range of indications for their use is wide and non-specific. This publication presents the mechanism of action of these nootropic drugs, historical outline of their use and epidemiological data on their use in elderly patients on the basis of patients treated in the Department of Geriatrics of Medical University in Bydgoszcz

It includes holistic approach to patients examination. In this systematic review we analyzed results of tests used in CGA and correlation of results. Moreover, we underlined some issues concerning homogeneity of tests used in CGA as well as scientific validity of some of its tools.Results of prospective studies show that some of results of examination used in CGA are good predictors of functioning in another health domain in next years.Systolic blood pressure as well as BMI could be good predictors for neuropsychological functioning, however results are equivocal.Low scores in dual task (walking and cognitive engaging task simultaneously) are good predictor for falls occurrence. Education measured by socioeconomic status is very often mentioned as one of the best predictors of cognitive functioning.Undergoing Mini Nutritional Assessment by patients could be an effective tool to improve their nutrition status, possibly by indirectly influencing on their diet habits. Moreover, genotype examination could be worth to include into CGA due to, inter alia, examination of possible causes of dietary deficits.Aerobic examination could be done by cost-free tests, which sensitivity and specify are on tolerable level. It could help lowering CGA costs, which will help make it widely available for patients.聽Keywords: Comprehensive Geriatric Assessment, cognitive functioning, older people

    Evaluation of Selected Parameters of Oxidative Stress and Adipokine Levels in Hospitalized Older Patients with Diverse Nutritional Status

    Malnutrition is classified as one of the Giant Geriatric Syndromes. It carries serious consequences, such as sarcopenia or depression, which lead to functional disability. The main objective of this study was to identify parameters of oxidative stress and adipokines, which may be potential biomarkers of malnutrition in hospitalized older patients. During the study, selected parameters were determined in 137 senile patients, taking into account their nutritional status determined according to the Mini Nutritional Assessment (MNA), as well as an additional tool, namely the Geriatric Nutritional Risk Index (GNRI). Leptin and resistin were determined as the parameters with statistically significant differences between the patients classified according to the MNA. This phenomenon was confirmed using the GNRI classification. However, additional parameters for which differences were observed include the oxidized low-density lipoprotein level and activity of glutathione peroxidase. In conclusion, the determination of the mentioned markers in hospitalized senile patients as an adjunct to the routine assessment of nutritional status might be suggested to identify the early risk of malnutrition so that a personalized nutritional therapy can be implemented as early as possible
