Magnetic and charge transport properties of the Na-based Os oxide pyrochlore

Na-based osmium oxide pyrochlore was synthesized for the first time by an ion-exchange method. KOs2O6 was used as a host compound. Elelectron probe micro-analysis, synchrotron x-ray diffraction analysis, and thermo-gravimetric analysis confirmed its structure not as the beta-type but as the defect-type pyrochlore. The composition was identified as Na1.4Os2O6.H2O. Electrical resistivity, heat capacity, and magnetization measurements of the polycrystalline Na1.4Os2O6.H2O clarified absence of superconductivity above 2 K, being in contrast to what were found for the beta-pyrochlore AOs2O6 (A = Cs, Rb, K). Sommerfeld coefficient of 22 mJ K-2 mol-1 of Na1.4Os2O6.H2O was smallest among those of AOs2O6. Event-free survival at 36 months is a suitable endpoint for diffuse large B-cell lymphoma patients treated with immunochemotherapy: real-world evidence from the North Japan Hematology Study Group

Information regarding follow-up duration after treatment for newly diagnosed diffuse large B-cell lymphoma (DLBCL) is important. However, a clear endpoint has yet to be established. We totally enrolled 2182 patients newly diagnosed with DLBCL between 2008 and 2018. The median age of the patients was 71 years. All patients were treated with rituximab- and anthracycline-based chemotherapies. Each overall survival (OS) was compared with the age- and sex-matched Japanese general population (GP) data. At a median follow-up of 3.4 years, 985 patients experienced an event and 657 patients died. Patients who achieved an event-free survival (EFS) at 36 months (EFS36) had an OS equivalent to that of the matched GP (standard mortality ratio [SMR], 1.17; P=0.1324), whereas those who achieved an EFS24 did not have an OS comparable to that of the matched GP (SMR, 1.26; P=0.0095). Subgroup analysis revealed that relatively old patients (>60 years), male patients, those with limited-stage disease, those with a good performance status, and those with low levels of soluble interleukin 2 receptor already had a comparable life expectancy to the matched GP at an EFS24. In contrast, relatively young patients had a shorter life expectancy than matched GP, even with an EFS36. In conclusion, an EFS36 was shown to be a more suitable endpoint for newly diagnosed DLBCL patients than an EFS24. Of note, younger patients require a longer EFS period than older patients in order to obtain an equivalent life expectancy to the matched GP

Mesodiverticular bandによる絞扼性イレウスの一例

症例は14歳,男児.開腹歴を含め,既往歴に特記すべきものなし.下痢,腹痛を主訴に来院し,抗生剤投与で一時軽快したが,その後腹部膨満感出現し,緊急入院となった.入院時血液検査所見で炎症反応の異常高値とCT検査上絞拒性イレウスの所見を認めたため緊急開腹手術を施行した.術中所見は回腸から約60cm口側にメッケル憩室を認め,そこに付着したmesodiverticular bandにより回腸が内ヘルニアを形成していた.小腸は広範囲に壊死しており,小腸部分切除を行った.mesodiverticular bandによるイレウスは稀で,術前診断は困難であるため手術歴のないイレウスの原因として考慮すべきであると考えられた.The patient was a 14-year-old boy who was admitted to our hospital because of abdominal pain and diarrhea. Blood tests showed strong inflammation, and plain CT showed the presence of air and fluid in the ileum. In physical findings, pan-peritonitis was suspected with muscle guarding, and so on emergency operation was performed. During the operation, we found that the ileum was compressed by a herniation of the mesodiverticular band and Meckel\u27s diverticulum. The a long segment of the ileum was necrotic, so these portions were resected and an end to side anastomosis was made. In a pathological study, neither gastric mucosa nor pancreatic tissue were observed, but microvessels were seen. Diagnosis is difficult before operation, so we should consider the mesodiverticular band when we see young ileus cases
