    Contact allergies are complex diseases, and one of the important challenges for public health and immunology. The German 'Federal Institute for Risk Assessment' hosted an 'International Workshop on Contact Dermatitis'. The scope of the workshop was to discuss new discoveries and developments in the field of contact dermatitis. This included the epidemiology and molecular biology of contact allergy, as well as the development of new in vitro methods. Furthermore, it considered regulatory aspects aiming to reduce exposure to contact sensitisers. An estimated 15-20% of the general population suffers from contact allergy. Workplace exposure, age, sex, use of consumer products and genetic predispositions were identified as the most important risk factors. Research highlights included: advances in understanding of immune responses to contact sensitisers, the importance of autoxidation or enzyme-mediated oxidation for the activation of chemicals, the mechanisms through which hapten-protein conjugates are formed and the development of novel in vitro strategies for the identification of skin-sensitising chemicals. Dendritic cell cultures and structure-activity relationships are being developed to identify potential contact allergens. However, the local lymph node assay (LLNA) presently remains the validated method of choice for hazard identification and characterisation. At the workshop the use of the LLNA for regulatory purposes and for quantitative risk assessment was also discussed

    Managing water in rainfed agriculture—The need for a paradigm shift

    Rainfed agriculture plays and will continue to play a dominant role in providing food and livelihoods for an increasing world population. We describe the world’s semi-arid and dry sub-humid savannah and steppe regions as global hotspots, in terms of water related constraints to food production, high prevalence of malnourishment and poverty, and rapidly increasing food demands. We argue that major water investments in agriculture are required. In these regions yield gaps are large, not due to lack of water per se, but rather due to inefficient management of water, soils, and crops. An assessment of management options indicates that knowledge exists regarding technologies,management systems, and planning methods. A key strategy is to minimise risk for dry spell induced crop failures, which requires an emphasis on water harvesting systems for supplemental irrigation. Large-scale adoption of water harvesting systems will require a paradigm shift in Integrated Water Resource Management (IWRM), in which rainfall is regarded as the entry point for the governance of freshwater, thus incorporating green water resources (sustaining rainfed agriculture and terrestrial ecosystems) and blue water resources (local runoff). The divide between rainfed and irrigated agriculture needs to be reconsidered in favor of a governance, investment, and management paradigm, which considers all water options in agricultural systems. A new focus is needed on the meso-catchment scale, as opposed to the current focus of IWRM on the basin level and the primary focus of agricultural improvements on the farmer’s field. We argue that the catchment scale offers the best opportunities for water investments to build resilience in smallscale agricultural systems and to address trade-offs between water for food and other ecosystem functions and services

    Altered postural sway in patients suffering from non-specific neck pain and whiplash associated disorder - A systematic review of the literature

    To assess differences in center of pressure (COP) measures in patients suffering from non-specific neck pain (NSNP) or whiplash-associated disorder (WAD) compared to healthy controls and any relationship between changes in postural sway and the presence of pain, its intensity, previous pain duration and the perceived level of disability. Summary of Background data: Over the past 20 years, the center of pressure (COP) has been commonly used as an index of postural stability in standing. While several studies investigated COP excursions in neck pain and WAD patients and compared these to healthy individuals, no comprehensive analysis of the reported differences in postural sway pattern exists. Search methods: Six online databases were systematically searched followed by a manual search of the retrieved papers. Selection Criteria: Papers comparing COP measures derived from bipedal static task conditions on a force plate of non-specific neck pain and WAD sufferers to those of healthy controls. Data collection and analysis: Two reviewers independently screened titles and abstracts for relevance. Screening for final inclusion, data extraction and quality assessment were carried out with a third reviewer to reconcile differences

It was simple - boost demand and more people would find work. But the mid 1970s the economic regency enjoyed by those advocating demand sided policies fell into disrepute as the OPEC nations raised prices dramatically and brought in a new era of both rising prices and unemployment. The laws of economics, which previously had viewed policy decisions as the choice between lower unemployment and higher inflation were now redundant. Both unemployment and inflation were moving in the same direction. The era of stagflation had begun

    Adult height and risk of breast cancer: a possible effect of early nutrition

    The relationship of breast cancer to early reproductive development and height suggests that fetal and childhood nutrition may be important in its aetiology. Caloric restriction sufficient to reduce adult height may reduce breast cancer risk. During World War II (WWII) there was a marked reduction in average caloric intake in Norway that resulted in greater nutritional diversity. We hypothesized that a positive association between height and risk of breast cancer would be stronger among women who were born during this period than among women born before or after the war. A total of 25 204 Norwegian women were followed up for approximately 11 years, and 215 incident cases of breast cancer were registered. We found the strongest positive association between height and breast cancer among women born during WWII: women in the tallest tertile (>167 cm) had a relative risk of 2.5 (95% confidence interval = 1.2–5.5) compared with the shortest (≤ 162 cm). Among women born before or after the war we found no clear association with height. The association with height in the WWII cohort may imply a role for early nutrition in breast cancer aetiology. © 2001 Cancer Research Campaignhttp://www.bjcancer.co

    Catch-up growth up to ten years of age in children born very preterm or with very low birth weight

    BACKGROUND: Improved survival due to advances in neonatal care has brought issues such as postnatal growth and development more to the focus of our attention. Most studies report stunting in children born very preterm and/or small for gestational age. In this article we study the growth pattern of these children and aim to identify factors associated with postnatal catch-up growth. METHODS: 1338 children born with a gestational age <32 weeks and/or a birth weight of <1500 grams were followed during a Dutch nationwide prospective study (POPS). Subgroups were classified as appropriate for gestational age and <32 weeks (AGA) or small for gestational age (<32 wks SGA and ≥32 wks SGA). Data were collected at different intervals from birth until 10 years for the 962 survivors and compared to reference values. The correlation between several factors and growth was analysed. RESULTS: At 10 years the AGA children had attained normal height, whereas the SGA group demonstrated stunting, even after correction for target height (AGA: 0.0 SDS; SGA <32 wks: -0.29SDS and ≥32 wks: -0.13SDS). Catch-up growth was especially seen in the SGA children with a fast initial weight gain. BMI was approximately 1 SD below the population reference mean. CONCLUSION: At 10 years of age, children born very preterm AGA show no stunting. However, many children born SGA, especially the very preterm, show persistent stunting. Early weight gain seems an important prognostic factor in predicting childhood growth
