    A Comparison of Oral Motor and Production Training for Children with Speech Sound Disorders

    Despite the many debates about the usefulness of nonspeech oral motor exercises (NSOMEs) in the treatment of speech disorders, few controlled experiments have evaluated their efficacy in the remediation of phonological/articulatory disorders (PADs). More importantly, the relative effect of NSOMEs compared with traditional production treatment (PT) has not been established. The current study employed an alternating treatment design to evaluate changes in production of sounds targeted by NSOMEs and PT in nine children with PAD. Each subject received treatment on two linguistically distinct sounds in which one sound was treated with NSOMEs and the second sound was targeted with PT. The difference in treatment efficacy, measured as the percentage change in target production for NSOMEs versus PT, was compared using a paired t test. Because NSOMEs typically are used to ready a child for subsequent PT, comparison of PT treatment accuracy was made between NSOME-first and PT-first sessions. Results demonstrated a statistically significant effect of treatment type with greater production gains with PT compared with NSOMEs. Further, no facilitative effect of NSOMEs on PT was noted; however, the choice of distinct treatment targets may have contributed to this null effect. Although additional investigation is warranted, the current investigation does not support the efficacy of NSOMEs in the treatment of PAD

    How young people find out about their family history of Huntington's disease

    Family communication about adult-onset hereditary illness can be problematic, leaving some relatives inadequately informed or ignorant of their risk. Although studies have explored the barriers and facilitators in family communication about genetic risk, questions remain about when, what, how and indeed whether to tell relatives. The process of disclosure is also dependent upon the way in which genetic information is realized and understood by recipients, but research here is limited. Our paper explores young people’s experiences of finding out about a family history of the hereditary disorder Huntington’s disease (HD). In-depth interviews explored how and when young people found out, their reactions to different communication styles and any impact on family relations. We recruited young people through the North of Scotland regional genetics clinic and the Scottish Huntington’s Association (SHA). Thirtythree young people (aged 9–28) were interviewed. A qualitative analysis was undertaken which revealed four types of disclosure experiences: (1) having always been told, (2) gradually told, (3) HD was kept a secret, or (4) HD as a new diagnosis. In particular, the timing and style of disclosure from relatives, and one’s stage of awareness, were fundamental in structuring participants’ accounts. This article focuses on questions of when, how and indeed whether to tell children, and sits within a broader set of research and practice issues about what professionals and families (should) tell children about parental illness and genetic risk.Wellcome Trust’s Programme in Biomedical Ethic

    Understanding Appalachian Microaggression from the Perspective of Community College Students in Southern West Virginia

    The term Appalachian is wrongly understood to represent a single culture of rural White poverty (Keefe, 2005). This conception contains stereotypical images that obscure hardships many rural White Central Appalachians face. Similar to other oppressed minorities in the U.S., what it means to be Appalachian is a social construction based on what differs them from the White hegemony. Recent scholarship on discrimination recognizes the importance of microaggression, small insults and slights experienced frequently by people from minority groups (Sue, et. al., 2007). Microaggression may be an especially insidious mechanism in the oppression of Appalachian people, since the derogatory stereotypes are broadly accepted while their oppressed status tends to not be acknowledged. This study applied qualitative focus group methodology to understand perceptions of microaggression and oppression among a sample of college students living in rural Central Appalachia. Results reveal themes of microaggression. Identifying Appalachian microaggression provides evidence of marginalized status and offers a framework for understanding how the social construction of White Appalachian perpetuates reduced status, stereotypes, and prejudice. Implications are discussed to consider how to foster resilience to oppression among rural White Central Appalachian people

    Evaluating the effects of a therapeutic day rehabilitation program and inclusion of gardening in an Australian rural community health service

    Therapeutic day rehabilitation (TDR) is a non-residential intensive structured program designed for individuals recovering from substance misuse. A weekly afternoon of therapeutic gardening was a new incentive initiated in a TDR program at one Australian community health service, designed to give participants the opportunity to spend time outdoors connecting with nature and each other. The aim of this study was to explore perceptions of participants enrolled in this program by employing a convergent parallel mixed-method design using qualitative individual, semi-structured interviews (n = 14) and longitudinal quantitative quality of life (QOL) data at three different intervals (n = 17). The analysis of the quantitative data showed that there was a statistically significant increase in the participants' QOL scores in three of four domains (physical health, psychological, social relationships) when comparing baseline and post completion of the TDR. These observed changes were maintained at the 4-week follow up. The key findings from the semi-structured interviews include positive effects for participants on social connectivity, structure and achievement, understanding of recovery and relaxation from contact with nature. This study shows that a combination of TDR and therapeutic gardening can improve participants' physical health, psychological health and social relationships. © 2021 La Trobe University

    Power,independance and worker democracy in the development of the National Union of Metalworkers of South Africa (NUMSA) and its predecessors: 1980-1995

    Student Number : 0376246 - PhD thesis - School of Humanities - Faculty of ArtsThis thesis examines the building of power and how workers’ control and union independence augmented or detracted from this process in the National Union of Metalworkers and its predecessors from the 1980s to the mid 1990s. These unions aimed to accrue power to improve both their members’ working conditions and to effect political and economic transformation. In this process the building of non-racial national industrial unions that cut across the ethnically constituted state, the promotion of workers’ control, and political independence from formal political organisations were central. This thesis demonstrates how Numsa and its predecessors overcame obstacles to the accrual of power and scrutinizes reasons for failures in achieving pivotal ideological goals. In the early 1980s Numsa’s predecessors constructed greater degrees of democratic organizational and bureaucratic power. The formation of Numsa in 1987 allowed for the further construction of an efficient bureaucracy to support organizational and bargaining activities. It successfully forged national bargaining forums and built hegemony across the industry. In 1993 Numsa adopted a programme through which it hoped to restructure its industries in the transitional period leading up to a new democracy. It failed however to successfully implement the programme in its entirety. Tensions emerged in union goals as membership remained focused on increased wages whilst leadership was attempting to restructure industry, enhance worker skills and augment workers’ control in the workplace. In the political sphere Numsa was largely unable to effect a deeper infusion of its socialist leanings. Though Numsa and other Cosatu unions made an important contribution to the birth of a non-racial democracy, the capitalist state succeeded in demobilizing the trade unions in their pursuit of more fundamental systemic change. By the time Numsa produced the concept of a Reconstruction Accord, later developed into the Reconstruction and Development Programme (RDP), the space to popularise a socialist perspective had been considerably reduced. Although Numsa forewent its early `party autonomous` position when Cosatu entered the ANC/SACP alliance, this was clearly far from a `state ancillary` stance. Though labour had won the right to be consulted in Nedlac and the right to strike, the possibility of dissent being diverted into bureaucratic chambers existed with a consequent loss of militant, strategic and ideological focus.     Strategic Behavior and Variation in the Supreme Court’s Caseload over Time

    Over the past sixty years, the size of the Supreme Court’s docket has varied tremendously, growing at some points in time and shrinking at others. What accounts for this variation in the size of the docket? We focus on two key strategic factors – the predictability of outcomes within the Court, and whether justices consider the potential actions of other political institutions – and assess whether these factors help to explain the variation in docket size over time. We discover that uncertainty and institutional constraints prevent the Court from choosing cases with complete freedom, even after accounting for other potential influences on the size of the Court’s docket

    Community Engagement: Using Feedback Loops to Empower Residents and Influence Systemic Change in Culturally Diverse Communities

    A multi-faceted approach to community engagement includes the need to involve the community members in the design, implementation and feedback of any program, services, or supports provided. Intentional participatory engagement of residents also requires shared responsibility for workload, shared recognition of achievement, thoughtful communication, engagement in robust discussions taking care not to internalize conversations as personal affronts and holding close the rules of effective decision making (Toms & Burgess, 2014). Implementing a feedback loop process can be used as a tool to foster intentional resident engagement. The findings presented in this article are from a case study of a neighborhood that implemented a feedback loop process. Analysis of data indicate that while transportation, housing, and places for people to safely be outside are real community concerns, acts of nature and choices made by city government also directly impact the quality of life of the residents in this community. The need for purposeful channels of communication to be established among residents, between neighborhood action groups, and city government is evident. Leveraging relationships with community partners to establish trust and organizational understanding of culture within community socio-economic context, along with the multiple layers of community engagement is discussed

    Statistical analysis of word-initial /k/ and /t/ produced by normal and phonologically disordered children

    The acoustic characteristics of voiceless velar and alveolar stop consonants were investigated for normally articulating and phonologically disordered children using spectral moments. All the disordered children were perceived to produce It/ for /k/, with /k/ being absent from their phonetic inventories. Approximately 82% of the normally articulating children's consonants were classified correctly by discriminant function analysis, on the basis of the mean (first moment), skewness (third moment) and kurtosis (fourth moment) derived from the first 40 ms of the VOT interval. When the discriminant function developed for the normally articulating children was applied to the speech of the phonologically disordered group of children, no distinction was made between the velar and alveolar stops. Application of the model to the speech of individual children in the disordered group revealed that one child produced distinct markings to the velar-alveolar contrast. Variability measures of target /t/ and /k/ utterances indicated greater variability in this disordered child's productions compared with the normally articulating children. Phonological analysis of this child's speech after treatment, in which the velar-alveolar contrast was not treated, revealed target appropriate productions of both It/ and /k/.
