    DECOMPOSITION ANALYSIS OF FACTOR COST SHARES: THE CASE OF GREEK AGRICULTURE

    An alternative version of decomposition analysis, based on factor cost shares rather than input demand functions, is presented and applied to Greek agriculture. Decomposition analysis shows that most of the changes in factor cost shares during the period from 1973 to 1989 are attributed to technical change and factor substitution, while the role of the scale effect is small, except that of fertilizer.     The Process-Goal Alignment (PGA) modeling method is a domain-specific modeling language that aims to achieve strategic fit of the business strategy with the internal infrastructure and processes. To ensure the acceptance and correct understanding of PGA models by business-oriented end-users, an intuitively understandable notation is of paramount importance. However, the current PGA notation was not formally tested up to now. In the paper at hand, we apply an evaluation technique for testing the intuitiveness of domain-specific modeling languages to bridge that research gap. Based on an analysis of the tasks, we propose improvements to six elements of the initial PGA notation. Our research contributes a comprehensive description of the empirical modeling language evaluation, which enables the reproducibility of the evaluation procedure by the conceptual modeling community

    Public attitudes towards Chinese medicine in Melbourne, Australia

    PURPOSE - To investigate the public attitudes towards Chinese medicine and the impact of the introduction of statutory practitioner registration in Melbourne, Australia. METHOD - A convenience sample of 575 participants (18-45 years of age) from three locations in Melbourne completed a self-administered, 60-question survey in August 2002. RESULTS - In the previous 12 months, Chinese medicine had been used by 30.9% of the respondents and 17.3% had visited a Chinese medicine practitioner. For each of eight common conditions listed in the survey, between 61.2% and 92.7% of participants indicated that western medicine was their preferred therapeutic option, rather than Chinese medicine or combination of Western and Chinese medicine. However, substantial proportions of participants preferred Chinese medicine for stomach or internal problems (28.4%), cold/flu (18.8%) and dizziness (17.6%). Although 78.2% were aware that statutory practitioner registration had recently been introduced in the State, only 29.9% indicated that they would only visit a registered practitioner. IMPLICATION - More stringent surveillance of Chinese medicine service providers and a public education campaign may be necessary to protect against unauthorized Chinese medicine practice

    Photodynamic therapy of choroidal neovascularization with enlargement of the spot size to include the feeding complex

    This is a case report of a 83-year-old man with choroidal neovascularization (CNV), due to age-related macular degeneration (AMD) in his right eye. Digital fluorescein (FA) and indocyanine green angiography (ICG) were performed, which disclosed predominantly classic subfoveal CNV and a dilated and tortuous feeding complex. The visual acuity was 20/800. Anti-vascular endothelial growth factor (anti-VEGF) treatment was suggested, however, the patient was not keen to receive an intraocular injection. Modified photodynamic therapy (PDT) with spot size enlarged, to include not only the CNV lesion but the feeding complex as well, was performed. Ten days after one session of PDT, ICG showed absence of leakage from the CNV and complete occlusion of the feeding complex. The visual acuity gradually improved to 20/100 and remained stable during the following 23 months. No evidence of CNV leakage was seen in the FA and ICG during the follow up period. Adjustment of the PDT spot size to include the detectable by ICG feeding complex might be an additional option in order to close the subfoveal CNV and might be considered as an alternative to intravitreal injection of anti-VEGF in selected cases where anti-VEGF treatment is not available

    Is there an additional benefit from coronary revascularization in diabetic patients with acute coronary syndromes or stable angina who are already on optimal medical treatment?

    Cardiovascular disease (CVD) is common in patients with diabetes mellitus (DM) and related clinical outcomes are worse compared with non-diabetics. The optimal treatment in diabetic patients with coronary heart disease (CHD) is currently not established. We searched MEDLINE (1975-2010) using the key terms diabetes mellitus, coronary heart disease, revascularization, coronary artery bypass, angioplasty, coronary intervention and medical treatment. Most studies comparing different revascularization procedures in patients with CHD favoured coronary artery bypass graft (CABG) surgery in patients with DM. However, most of this evidence comes from subgroup analyses. Recent evidence suggests that advanced percutaneous coronary intervention (PCI) techniques along with best medical treatment may be non-inferior and more cost-effective compared with CABG. Treatment of vascular risk factors is a key option in terms of improving CVD outcomes in diabetic patients with CHD. The choice between medical therapy and revascularization warrants further assessment
